Employee was jack hammering during clean up and felt a piece of material or dust fall in EE's eye. Immediately washed-out eye but swelled up overnight and following morning took EE to the clinic to be seen and employee had a scratch to EE's eye. "Abrasion of left cornea"
Employee was performing housekeeping on a spill in an open tunnel when coworkers observed pale, heavy sweating and abnormal behavior within 15 to 20 minutes. Supervisor was notified and then EHS manager was called. Manager rushed the employee to the ER nearest to plant job site as employee was showing seizure like symptoms, the cause was determined to be dehydration.
Employee was de-icing sidewalk flatfooted on both feet. EE slipped on ice and landed on EE's back. EE was wearing EE's hard hat and as EE fell the hard hat bill forced EE's head to go forward straining EE's neck.

Employee was grinding with a corded 4 1/2" grinder on a dust collector manifold replacement at the maintenance fabricating shop and got a piece of metal in their eye due to flying debris after removing their auto darkening hood and safety glasses.
Employee was cleaning out discharge end of quencher and hurt their ankle in the process, tripping over clinker balls.
While installing a deflection shield for under the redimat, employee was notching the belt to fit correctly around the gate foot with a box cutter. Cutting towards the foot/body, the box cutter slipped and cut employee through EE's work boot and cut into inside part of EE's R foot. Seeked medical care and no stitches or restrictions.

As employee was completing their normal job duties, they were walking up the stairs and they lost their footing and at the same time their knee buckled and went backwards causing their knee to hyperextend back.
Employee was trying to move impact dolly when EE strained EE's Right shoulder
Employee was trying to get the Gypsum feeding unplugged on the bottom of Silo #17. EE was working from a small portable platform. As EE performed the task EE slipped and EE's mouth on the silo cone, causing EE to break EE's Left front tooth.

Employee was helping unchoke south side air slide at the packhouse. EE climbed up on the handrail to hit the side of the air slide box with hammer. The handrail broke causing employee to fall and catch self with right hand. Causing employee to tear/rupture the bicep.
Cut to middle finger on left hand during installation of clay pump housing
Miner was shooting cardox in kiln, inserted the drill bit into the port and when the air valve was turned on hot clinker dust blew back into the miner's face and arms. The miner received first, and second degree burns to face and arms. Miner was treated at ER Clinic and returned to work the same day.
Miner was walking by the bag house and slipped in the wet dust and twisted left ankle when trying to catch self. Weather was cold and wet.

Employee was in the quencher dryer are helping clear a choke up in the kiln throat. Employee was using a sledgehammer to hit the side of the chute. When they struck the chute they felt a discomfort in their upper body.

The miner was removing duct tape from a coupling for KWF-1 using a knife and when the shaft accidentally rolled EE cut EE's left knuckle on EE's index finger and EE needed stitches

The injured miner was removing broken bags from under the palletizer area when EE injured EE's lower back.
The injured miner was jack hammering out a choke up in the throat that goes into the quencher dryer when received chemical burns from clinker dust. EE did seek immediate medical attention but the pain got worse later on and they went to an urgent care clinic the next day.
The Injured Miner (IM) was striking a air line when a build up of mud fell striking the IM's hard hat and right shoulder causing neck trauma and chipping a tooth.

The injured process foreman was assisting in shooting out a ring in the kiln with a shot when some material got into right eye.
The injured process attendant IV was washing down the raw mill basement area floor when they slipped on green algae injuring their right knee.
The Injured employee was assisting with removing screens from a raw ball mill when the screen dislodged fell and struck EE's right foot and wrist.
The Miner was removing bars from a cement mold when the screwdriver ee was using slipped and struck left thumb and nail.
The Injured Employee was opening a rail hopper car door with a bar when the bar slipped causing the IE to fall to the ground landing on right wrist.

The injured miner was jackhammering in the quencher dryer and removing the loose material with a shovel. When EE bent over and twisted to remove the material, EE felt a pull in lower back. EE did not seek medical attention until 10/07/2018.
The consultant was eating breakfast in a small conference room when the consultant suddenly collapsed in the chair. CPR was administered until emergency responders and EMS arrived.
The Miner was working on the finish mill chute choke up with a rivot buster. The air hose of the rivet buster moved knocking a 6' bar over striking miner's back causing a bruise. Miner did not seek medical attention until 6/13/2018.
Employee was removing discharge head liner when the liner slid causing the employee to move and lose balance causing EE to fall over on left side. Employee received a laceration to left leg below the knee.
The Maintenance Journeyman was helping to replace a dust collector bag and was pushing in two clips with EE's two index fingers when the other maintenance journeyman lifted the bag the clips mashed both fingers. EE did not seek medical attention until 3/28/2018
The Serviceman was shoveling up cement at the rail unloading station when the Serviceman felt back go out
The Maintenance Journeyman A was bent down vacuuming the compressor room and when the Maintenance Journeyman A raised up felt a pain in lower back. The Maintenance Journeyman A did not seek immediate medical attention until 01/26/2018

While checking a clinker inlet chute the miner was hitting the side of the chute to remove any buildup and had hand resting on the edge of the inspection door when a chunk fell striking right hand against the chute causing a laceration
The injured miner was replacing a door on the side of a screw and was replacing the bar that held the door in place with a hammer when the miner hit left index finger causing a laceration.
The Injured miner was exiting a forklift and when turning to get off felt a sharp pain in the middle of back on the left side
The Maintenance Journeyman A was lifting a troughing roll for a belt conveyor and strained back.

After shoveling around LWF-1 weighfeeder, the miner took a break then complained about a back strain and reported it to a supervisor. Employee did not seek immediate medical attention until 12/27.
The Maintenance Journeyman A was climbing an affixed ladder at the Quencher Dryer Pier when the anchor point broke loose causing EE to fall landing on right shoulder.
The injured employee was cleaning the raw mill area. While shoveling material into the gehl loader bucket, the injured employee turned EE's body to dump the shovel into the bucket, EE felt a sharp pain in EE's upper right hip.
The injured employee was operating a forklift to stack pallets of packaged cement when a fluid cooler fan started vibrating. EE went to adjust the fan guard when finger was cut by the plastic fan blade requiring stitches.
The Quality Control Tech(QC Tech) was assisting with charging a raw mill with grinding media using a 5 ga. bucket. 4/25 ee reported that Dr. diagnosed ee with a hernia. EE was sent to the worker's comp Dr. with an umbilical Hernia. The hernia was cosmetically corrected on July 8.
The injured employee was moving pallets of cement with a forklift. Due to stronger winds from storms sawdust from a pallet blew up under employee's safety glasses and got into employee's eyes requiring prescription drops.
Process Attendant IV was making rounds inspecting the finish mill equipment when they made contact with a beam just under the finish mill area. They reported it immediately to their foreman and their scheduled day off was Monday. They came in Tuesday with a stiff neck asked for medical attention.
The injured employee(IE)was be refreshed in import cement unloading. The IE was assisting opening the hopper door on a railcar. The IE was holding open a locking device on the door inertia caused the door to open and crush is left index finger to smashed by the locking device.

Miner was walking out of the elevator when he bumped his right elbow on the scissor gate. The miner was carrying a box in his arm. He did not have symptoms of swelling until later that night. He called his foreman the next day 7/25/2015.
The Injured Miner was changing the housing cover of CSP-8 pump. It was badly worn and had sharp edges around the bottom of the cover. He grabbed a hold of it to slide it out of the way and the sharp edge sliced his left finger
Employee had just got a sample of flexicoke from a flexicoke trailer and was descending when his foot landed on a chain used to secure the discharge cap to the trailer. This caused him to lose the 3 points of contact and lose his balance causing him to put his weight on his right arm causing pain. As a process attendant he had his required PPE including his fall protection
A Serviceman IV temporarily assigned to the pack house stepped off the forklift to refuel it with propane when he felt pain in his left knee. He was sent back with no restrictions. On 8/6/2015 a specialist reviewed his MRI and put him restrictions.

Injured Employee (IE) removing a chunk of material when some fell off hitting a bar and smashing his left thumb breaking the bone. IE did not request medical attention until December 8th.
Injured Employee (IE) was moving rail car from c-silos to plant; came up on switch and dropped to ground. As IE turned to walk to back of car the gravel gave way and he twisted his left knee, left ankle, left wrist and right ankle
Miner was removing castable from a quencher spout and piece of castable blew into his eye.
Injured employee (IE) was taking off parts from the K5 when the follower plate suddenly came loose and smashed the IE's finger between the pump shaft
Injured Employee(IE) was removing raw mill spout cut bolt and was driving out bolts to remove the spout. The spout moved and made the alignment bar come in contact with his left and the hopper of the raw mill causing bruising. Subsequently a knot or cyst has come up requiring more attention.

Injured Employee (IE) was starting the Cemex unloading system and saw that the system was dusting. He went around to the Cemex Dust Collector while it was still running and opened the inspection port and stuck his right hand inside and contacted with a rotary feeder catching the tips of his two fingers on the right hand and pinching tissue on the ends.
Injured Employee (IE) was removing coating from the kiln throat using a rivet buster. He aggravated his old back injury by binding and pushing the coating down with the throat.
Aqueous Ammonia 19% dripped into his left eye, air lines were contaminated with aqueous ammonia, trying to remove plug on the run under pressure, wind blew it into his eye
Injured Employee (IE) was working on a stretch hood machine. When he stepped off the machine from an access staircase, he twisted his knee. The IE was sent to a specialist which the IE underwent an MRI the MRI came back inconclusive. The IE was sent back to the specialist after reviewing the MRI again and again saying the MRI was inconclusive the specialist recommended therapy.

Injured Employee (IE) was cleaning out a dust collector with his left hand up in dust collector trying to unplug it. The cement in the dust collector broke loose pushing his hand down and his finger got caught in the rotary feeder below.
Injured employee was doing an assembly of a finish mill cement pump parts. The pump is below waist line alot of leaning over lifting parts and strained back
Injured Employee (IE) was cleaning chunks out a 3" hole from a cement silo with a bar and chunk broke up then cement come out and knocked the IE's glasses off and got in his eyes.

Miner was moving a vending machine and smashed his right little finger between machine and door jamb.
Employee was trying to get KCD #2 drag running when some hot raw clinker splashed on to employees left arm causing second degree burns.
Injured Employee (IE) was helping to charge a raw mill with steel balls and was tightening the chain on a Morse drum handler when he felt the pain. He wanted a report filled out but did not seek medical attention until 8/16/2011.
Miner was handing up cages for dust collector bags to re-install them when he felt something in his back.
Miner was opening a door on a cement hopper car and was breaking a cable tie with a large bar. When the cable snapped it jarred both his elbows causing pain.
Miner was repairing a Hercules roller mill plow and was removing a bolt from one of the plows. when he struck the bolt with a hammer cement shot out of the hole and into his face and left eye which was scratched.
Miner was rebuilding a hercules mill roll for installation and was using a 3# hammer to drive a bolt into place and after several minutes of swinging the hammer he felt a pop in his lower back.
Miner was removing broken parts of a die ring from a hercules mill and strained his lower back
While doing a brick job and putting in the key row the brick moved and smashed his right hand small finger breaking the tip of the finger.
Miner was helping to remove a guard at the end of a Raw Grinding Mill and said he felt some discomfort in his back but did not want medical attention at the time. 1/14/2011 Miner was having some back spasms and requested to see a dr. Dr diagnosed a sprain on the right side of his back.

Miner was inspecting outfall #1 from the source, slipped and fell in the mud and felt pain in both shoulders and arms. Dr. released him with no restrictions 8/26. 9/2 Dr. released with no restrictions set up MRI appointment 9/4/10. Specialist visit 10/5 Dr put him on restricted duty of use of right arm.
Miner was hauling clinker from end of quencher dry with a skid steer loader and had a chemical reaction burn from the clinker dust.

Miner was manually moving by hand, a bucket elevator by sheave and pulley to position elevator sprocket into a workable position when his pinky finger was caught between the sheave and the belt and the tip of the finger was broken.
Miner was charging Raw mill with steel balls. He cut the band that held the lid of the barrel and when the band came loose it hit his forearm cutting it requiring 2 stitches.
Due to repetitive motion which includes loading the cement packing machine with empty bags and operating a forklift. carpal tunnel syndrome was deemed as a work related injury on May 18th. Day surgery was done on May 20th.
Miner was using a 50 lb weight to calibrate a scale belt when he said he felt something in his back. The initial visit did not reveal anything and he returned to work. During an economic lay-o ff he went to a third doctor who discovered 2 disc protrusions in his back and he did not return to work from lay-off on December 14th.
Miner was tracing air line for a dust collector slide gate actuator. He closed a valve to begin removing a cam locked air hose fitting. When he grabbed the connection, the air line broke at the valve and blew debris that was in the line into his face.

Finish Mill work was being done, when some equipment was being removed, creating a hole. The miner was tracing water lines, when he stepped into the hole falling to the same level, causing a fracture in the wrist and bruised his chin and leg. They were in the process of covering the hole when the accident occurred.

Crush & laceration of right ring finger, no broken bones, loss of fingernail. Required 5 stitches. Gentleman had 2 beams rigged together & was flagging with other hand. Had one hand on beams as lift was being made & finger was caught between beams.
Miner was holding a shaft for a 2" slurry with a pipe wrench while another miner removed the nut on the impeller end. The wrench came loose striking the miner's middle finger breaking it in three (3) places.
While removing a coupling on a shaft of cement pump, miner used a jackhammer bit as punch. As he was striking the bit, a piece of metal sliver splintered off and struck victim in the arm. The doctors original diagnoses stated that nothing needed to be done, but later due to discomfort, the miner requested that the object be removed and the surgery was performed on July 30th.
Miner was stepping down off a clinker platform after trying to locate a clinker chunk; when he was about 4' off of the platform level the miner fell on his side. He was helped up by the process attendant. Within the next couple of minutes he passed out and hit his head on the platform grating causing a laceration requiring stitches
While replacing screens in the raw mill a screen fell on the back of the victim's leg and it took assistance from another miner to lift it off. The strain of lifting the screen caused a herniated stomach. He is on light duty at this time but will require surgery at a later date. The screen was placed improperly on the uneven ball charge causing it to fall over.

The victim was observing a auto stretch film operate to determine why the knife was not cutting the film when he went to check the knife because it was jammed. Before de-energizing the equipment or checking for stored energy the victim unjammed the blade from the guard causing the knife to cut his thumb and thumb nail requiring 3 stitches.

Miner was replacing 3/4" steel air line and valve. When he was replacing damaged valve, compressed air in the line blew rust into his left hand. He had isolated the damaged valve with a second valve. The second valve appeared to be completely closed, but was not closed.
Miner was installing a 3/8" metal plate on the bottom of a drag conveyor. The injured was using an aligning bar to put bolts in the bottom. He was laying on his side & moving the bar when he felt a burn. It was not reported that day to a foreman or supervisor. The next day he came in & complained about the pain.
Miner was removing a 10" pipe elbow that was originally installed using silicone as as a gasket. While using pressure with a forklift fork to break the elbow loose, it broke loose and fell onto the miners right hand cutting the right hand knuckle requiring stitches and broke his small finger on his right hand.
