DIGGING INTO A 10 FT FACE WALL WITH A FRONT END LOADER, THE OPERATOR DUG OR REMOVED MATERIAL UNTIL THE BERM ABOVE FELL IN. THEN THE OPERATOR TRAVELED AROUND TO THE TOP OF THE WALL AND BACKED O VER IT.

WHEN RAISING A LOADED BUCKET ON A WHEEL LOADER A HYDRALIC LINE BURST CAUSING THE BUCKET TO OCMEDOWN THE IMPACT FORCED THE OPERATOR UP IN HIS SEAT. HE CLAIMED THE SEAT BELT RESTRAINT IS WHAT P UT PRESSURED ON HIS BACK. THE OPERATOR WENT TO A CHIROPRACTOR AND WAS TOLD NOT TO WORK FOR ABOUT A WEEK. THIS WAS ON 9/13/99
