EE STATED THAT HE HURT HIS BACK WHEN HE RAN OVER A LARGE ROCK WHILE PUSHING IN SHOT MATERIAL. HE DIDN'T REPORT ANY INJURY UNTIL 11-22-99. HE WORKED REGULAR SHIFT ON 11/19/99 AND 11/20/99.

EE WAS HOLDING THE TOOL USED TO DRIVE THE PIN OUT ON THE BUCKET. A PIECE OF THE HAMMER CHIPPED OFF AND IMBEDDED IN HIS HAND. SIX STITCHES WAS REQUIRED TO CLOSE THE WOUND.
