EMPLOYEE WAS CLIMBING A LADDER IN THE DRY ROOM TO THE LOFT. AS HE REACHED THE TOP, THE TOP 18" OF THE LADDER BROKE OFF AND THE LADDER PULLED AWAY FROM THE WALL. HE FELL 6 TO 8 FEET TO THE CE MENT FLOOR LANDING ON HIS HANDS AND KNEES, STRIKING HIS HEAD ON THE FLOOR. STICHES WERE REQUIRED IN HIS FOREHEAD.
