Employee was walking on uneven ballast setting the unit train railcar brakes when EE stepped wrong and over extended EE' right knee. The employee saw a doctor and was not placed on restrictions and had not missed work. On Oct 13, the employee saw a surgeon and it was determined EE would need surgery. The employee is still able to work full duty, but did miss a full shift on Oct 13.
Employee was rebuilding a dodge gear box. As the employee turned over the box, it fell on the tip of EE's right hand index finger, crushing the tip. The employee requested only first aid treatment at the time of the injury. The finger continued to bother EE and EE went to the Dr. on 2/10/22 and the diagnosis was a broken bone at the tip of the index finger.

During the activity of removing the adjustment bolts from the bearing housing, a punch was used to support one side of the bearing, the employee pulled out the support punch and stored energy caught EE's hand between the bearing and adjustment housing.
Employee was shoveling off a belt that was overloaded.

Operator was operating a haul truck when they had an asthma attack possibly triggered by second hand smoke.
Cleaning mirror on truck and EE slipped. Grabbed for the mirror and cut EE's hand as a result (stitches required).
Fork had come off the skid steer. Employee was lifting and pulling to reinstall the detached fork. While performing this activity, the employee strained EE's right arm bicep
Roughly noon on Tuesday, Feb 20, ee were on top of the new water tank checking the heat trace to make sure it was working. EE was at one inspection hole and as ee stood up to walk to the other inspection hole, feet slid out from underneath. EE landed on bottom and didn't think much of it the time. As the day progressed ee became more sore.
Taking an empty bag and filling with 50 lbs limestone product, lifting full bag off a table (waist height) and pivoting 180 degrees, bending to place at ground level onto a wood pallet. Repeat process until pallet is 10 rows high of full bags (shoulder height). Injury to neck is undetermined at this time.

The employee was clearing a beam of built up limestone with employee's foot to install expanded metal to prevent pigeons from accessing the plant, when employee twisted and sprained employee's left knee.

Employee was cutting the temporary bracing out of the pin mill frame when a leak occurred in the oxygen hose causing EE's glove to fill with oxygen. A spark from the cutting ignited the glove.

Contractor was replacing the cable on his drill. As he was trying to get the cable off the pulley, he used a pry bar to pull out the cable. While he was applying force, the pry bar slipped and struck him on the left eye brow. The brow line was cut and he received two stitches.
employee was shoveling material on to a conveyor belt, turned his body wrong and strained the left shoulder and a possible partial tear of the left bicep.
Mechanic was removing cutting edges on the 988H loader. As he was holding the bolts in place with a gloved hand, the bolt head spun, cutting thru the glove into the palm of his hand.

Bearing out of return roller on conveyor allowing the roller to become dislodged from the pin. Roller fell approx 20' grazing employee's hard hat and left shoulder impacting hand on hand rail. Laceration to palm of left hand requiring seven stitches.

EE was operating production FEL loading haul truck w/shotrock. A rock 2" in diameter rolled down muckpile between L. front & rear wheel. He did not see rock and ran over the rock W/L front wheel while backing out of the muckpile W/ a loaded bucket. The resulting impact jarring motion hurt his back. Miner reported need for medical attention on 10/17/2007.

Employee climbed onto stationary equipment & shoveled rock from equipment decks throughout the day. He did not seek medical treatment until 9/7/06 at which time it was determined he has a hernia. Last working day was Wed. 9/6/06. Is taking antibiotics for a related infection, then will schedule appointment with surgeon.
EE was walking down a ramp area after checking to make sure his trailers were lined up under the customer product load out.EE slipped and fell backwards using his left arm to catch his fall. EE left shoulder was sore however he did not go to a clinic until 02/12/2006 received pain pills and continued his normal work schedule.
