Washington Mine: 1 recorded fatality Metal/Non-Metal

Employee reported late, and said he overslept. The following day, he reported unfit for work, due to having taken medication and being emotionally unstable due to personal problems. He was sent home to get some sleep. Nov. 23, he showed up late and demanded to go to the doctor, which he did. He was given two weeks of restricted duty that would preclude him from performing his job.

Small rock came out of the back, striking the miner in the back. Miner did finish the shift, but sought medical attention the next day because of pain when the spot was touched. No pain otherwise. Skin not broken. No visible bruise. Clinic doctor limited the miner's mobility, pending an MRI to determine if undetected damage has occurred to the spine.

While operating a jackleg to install bolts, a miner felt a pop in his right knee. The work deck was level and clear of obstructions and tripping hazards. Primary contributing factor is recurrent injury to knee from other non-work related accidents.

The driller was operating a wire line core drill to retrieve a stuck core barrel. He tensioned the cable to free the stuck barrel when the sheave wheel dislodged from the mounting bracket and struck his right hand and the thumb joint.

The miner was using a jackleg to drill back holes in a scram drift. When he energized the leg he had his left hand on top of the machine which pinched his index finger between the machine and the roof.

A miner was descending an equipment ladder on a surface haul truck. When he reached the bottom, he stepped on the ground and twisted at the same time causing his knee to "pop." This miner has a history of non-work related injuries to the same knee. This previous injury combined with the twisting motion contributed to the injury.

On 5/20/13 a miner was blowing in the end of a drill steel to see if it was plugged. His partner turned around and accidentally hit the steel, injuring the miner's tooth. First aid was given initially, but the miner requested further treatment on 6/3/13 after the condition did not improve.

While installing runners to attach lagging boards in the 2047 raise a miner missed the nail he was driving and struck the knuckle of his right hand index finger.

Miner "ran" up the haulage to get out of the way of a haul truck. First aid applied. Went to Dr on 2/18/13 for continued pain, possible hernia.

He dismounted a LHD and slipped on a rock, hyper-extending his knee. The accident occurred at approximately 0800 however the employee continued to work on the injury until he reported it at 1430. This potentially aggravated the injury beyond the treatment of basic first aid.

The miner was walking down the main decline and heard a pop in his left knee. The decline was graded prior to the start of shift and was clear of tripping hazards. The miner did not trip or fall. The miner has a prior his history of injury.

The mill operator was freeing compacted material from the top of a feed hopper when a large rock dislodged and struck this left leg.

While lifting a trashcan full of shop waste a "pop" sound was heard from the right elbow/bicep area of the miner's arm.

The miner noticed a lump in his left groin area on May 10th 2012. He visited his family doctor on May 25th 2012 and it was determined to be a hernia. A specific incident or action that could have caused the hernia is unknown, nor is it known if the hernia was caused by activities related to the miner's tasks.

While shoveling rock, a boulder fell out of the rib and rolled over the miners foot, fracturing two bones in the miners foot.

The miner was moving a mat when it contacted the rib, dislodging a small piece of rock which fell behind his safety glasses and into his eye.

The miner was slushing in the 371 Stope, when the handle of the slusher he was operating kicked back and hit him in the mouth splitting his upper lip.

Miner was collaring an up hole with a Jackleg Drill, with his right hand on top of the steel puller. His work glove became stuck to the drill steel, pulling his glove and hand into the steel puller, rapping his glove and hand around the drill steel.

A crew of miners were working off the main raise of 275 Stope. Miner went down the main raise manway to the sill level. He began picking up some items from under the ore pass. He had not covered the grizzley of the ore pass. A rock rolled down the ore pass, and hit the miner in the hardhat causing injury to the miners head.

Two miners were installing rock bolts for ground support after scaling and bolting the back. As they were drilling a rock bolt hole a large slab of rock slid down from the hanging wall side breaking the timber supports, supporting the two miners. One miner fell 14' to a rock pile and was seriously hurt with broken ribs and collar bone, the other did not fall and was uninjured.

The employee was cutting a piece of belting with a razor knife. He looked away for a moment, the knife sliped and he cut his thumb.

Drilling with a jack-leg when bit caught and twisted employee's shoulder. Strain to shoulder.

Closing the door to pulveriser and did not have my thumb out of the way, smashing the tip of my thumb.

Pulling a jackleg steel out of the hole, when it came out he pinched his finger between the drill and the leg.

Moving jackleg to face of drift, turned wrong and strained my back muscle.

Stepping down off of the 928 loader. Landed wrong on the ground and broke bone in his foot.

Miner was putting up utilities off of scissor deck. He did not put chain across the back of the scissor deck, slipped and fell off scissor deck landing on his butt. He has a compression fracture of his tail bone.

He was cleaning up some mud with a shovel. He said his knee popped and he felt a lot of pain.

EE was getting off the IT28 loader with his lanyard still on. He caught his lanyard on the step and fell off the loader and hit his lower back on a wooden rail strain/sprain to lower back

Was setting timber and some rock fell from the back. When the miner jumped down he caught his arm on a slusher cable and tore his rotor cuff.

He was walking down the muck pile and stepped wrong. He did not even fall down.

He was taking a sample at the face of the Washington drift. It was not yet bolted. A slab fell from the back hitting his right shoulder, lower back, and left lower leg. Just strain and bruses.

The miner was bolting with a jack-leg when the steel caught. When it spun around he grabbed the machine and his thumb was pinched between the stops for the hammer and the leg. It cut and broke his thumb.

MINER WAS CLEANING OFF BULKHEAD WHEN A ROCK 2' OUT OF THE BACK SMASHING THE MINER'S THUMB.

WORKER WAS LEARNING HOW TO DRILL WITH THE JACKLEG. A ROCK FROM THE HANGING WALL CAME DOWN HITTING THE WORKER IN THE SHOULDER KNOCKING HIM DOWN. THE ROCK LANDED ON HIS RIGHT ANKLE.

ON THE DATES 6-2-94 & 6-3-94 EMPLOYEE CUT & WELDED 6" H BEAM TO 7' LENGTHS & MOVED THEM OUT OF THE MILL BLDG. WEIGHING APPROX. 175 LB. TOTAL OF 6. ON SUNDAY 6-5-94 HE HAD SEVERE BACK PAIN. ADM ITTED TO THE HOSPITAL 6-7-94.

WHILE INSTALLING AIR BAG, NEAR THE LUCKY SEVEN WINZE FREDDY WILCOX LOST HIS FOOTING AND FELL. HIS LEFT ELBOW CAUGHT ON LADDER. HIS LEFT SHOULDER WAS DISLOCATED.

HAULING LOAD OF CARS-TRAIN-OUT OF ADIT EMPL GOT BETWEEN RIB AND LOCI ON TIGHT CORNER.ATTRIBUTE EMPL CARELESSNESS .ONE RIB WAS CRACKED,RIGHT SHOULDER BRUISED.

5 STITCHES REQ FOR GASH ON UPPER RIGHT THIGH CAUSED BY CLIMBING PIECE OF 1/4"PLATE CUT IN NUMEROUS PLACES AND TRIPPING OVER IT.
