Employee was holding on to a piece of metal with one arm to place it on the plant when he twisted his arm and felt pain in his right shoulder.

Employee was routinely checking the bin capacity of the stone bin on the plant. He opened the overhead door (chain pulley) then stuck his head under the door to look in the bin. He accidentally let go of the chain and the door came down on his head.
