EMPLOYEE WAS PUTTING DOG HOLES IN STONE HE WAS PINNED BETWEEN THE PLUG DRILL WHICH HE WAS OPERATING & THE COUNTERWEIGHTS ON THE LOADER WHICH WAS BACKING UP CRUSHING HIM TO DEATH

ON 7-15-95 EE WAS PUSHING ON A BEAM AND HURT HIS BACK. DID NOT GO TO THE DR AT THE TIME BECAUSE PAIN STARTED TO GO AWAY. THEN PAIN SLOWLY STARTED MOVING TO ARM AND SHOULDER. EE WENT TO DR ON 8 -18-95. INJURY REPORTED TO COLD SPRING FIRST AIDOFFICE ON 8-24-95.

EMPLOYEE WAS BLOWING THE MUD OFF THE ROCK, USING THE AIR HOSE AND THE BLOW PIPE. THE EMPLOYEE SLIPPED OFF THE ROCK. THE ROCK WAS WET AND AT A 40 DEGREE ANGLE. THE EMPLOYEE CAUGHT HIMSELF WITH HIS HANDS AND INJURED HIS RIGHT THUMB AND PALM. THERE IS EXTREME SWELLING AND PAIN. THE EMPLOEE DID NOT PUT ICE ON INJURY UNTIL HE WAS BACK IN GREEN LAKE, HE FELT IT DIDN'T REALLY NEED IT.
