EMPLOYEE STATED HE WAS OPERATING THE LOADER WHEN A 12" ROCK ROLLED DOWN THE HILL AND BROKE THE SIDE WINDOW OUT OF LOADER H ITTING EMPLOYEE ON BACK OF LEFT SHOULDER. HE WASTAKEN FOR MEDICAL TREATMENT: X-RAYS. THE NEXT DAY HE WAS SENT FOR A MRI OR SCAN, HIS DR. IS WAITING FOR RESULTS.

ON 9-15-95, EE WOKE UP WITH A VERY SORE EYE--HE THOUGHT HE HAD PINKEYE AND WOULD NOT SEEK MEDICAL ATTENTION. AFTER APPROX 3 WKS, EE'S EYE WAS NOT BETTER SO HE SOUGHT MEDICAL TREATMENT ON 10-09 -95, DR, REMOVED A FOREIGN OBJECT FROM EE'S EYE.
EMPLOYEE WAS LIFTING A LARGE ROCK THAT WAS STUCK IN THE CRUSHER JAWAS WHEN HE FELT PAIN IN HIS BACK. HE SOUGHT MEDICAL TREATMENT (CHIRO) ON 7-18-95 AND RETURNED TO WORK ON NEXT SCHEDULED SHIF T.
