EE was inspecting a noise on a shaft under sand classifier. EE placed right hand on the guard as they leaned in to take a closer look and the key stock on the end of the shaft caught the tip of a glove and pulled a finger into the guard. EE received a 1/4" laceration to the side of the right pointer finger.

EE felt pop in knee when starting to get on the ladder to exit the cab of the unit.

Employee states that repetitive movement of pressing the diesel pedal on the 980G Cat loader he was operating, was causing his right knee to hurt.

Employee was attempting to make repairs on 980H Cat loader. He climbed up the loader to the battery box. He then stretched his right leg out to step on the rear axle of the loader, when his left foot slipped off the battery box and caused his left shin to hit the corner of the battery box, causing a contusion.
Employee was installing rear transmission cover plate on D8 Cat dozer. While installing rear cover, the 6-point socket he was using slipped off the bolt, causing the back of his right hand to strike against the rear frame plate on the dozer.
Employee's thumb became pinched between a section of pipe and handles on bale resulting in a laceration on his thumb. This occurred as the employee was loading pipe as part of the bail drilling process.

Employee was installing bucket teeth on a 375 Cat excavator. While doing this he felt a pinch on his lower left abdomen. As he went to pickup teeth off shop floor, felt pinch again. Diagnosed with abdominal wall strain.
Employee had open wound on right middle finger. While repairing equipment, grease got into wound, causing it to become red and swollen.
As the maintenance crew was attempting to unclog the sand line, the unsecured pipe used for the come-along slipped off the pump side, striking the employee on the back of right shoulder. As this happened, the come-along slipped off the pipe, causing the pipe to come back up and strike the employee across the nose and forhead.
On 4/19/11 at approximately 11:57am, EE was operating the crane to relocate the pump in the west pit. As he swung to the left, the crane turned on its left side. EE was taken to the hospital.
Employee smelled hydraulic oil while operating 375 Cat excavator. Employee stopped running the machine and went to the back of the machine to check the fluid level. He stepped on the battery box with his right foot and while raising his left foot, he hooked it on the battery box cover latch. This caused his left foot to bend under creating a foot strain.
On 1/24/11, at 10:39am, injured EE & another EE were moving the fusion machine to weld poly pipe. As injured EE was pulling on the machine, he felt a pain in his lower back.

As employee was driving his B30 Euclid haul unit, he hit a bump in the road, causing soreness/stiffness in his neck and right shoulder.
While the employee dismounted his Cat 740 haul truck, he twisted his right knee when his foot slipped off the last step. The employee did not seek medical attention at this time. On 12/07/10, employee requested medical attention. Light duty was issued by a doctor which made it MSHA reportable.

EE clocked out of work and drove his personal vehicle down to the southeast pit area. He stated that the sun blinded him for a moment and he drove his personal vehicle over a berm and into the pit. When this happened the car rolled over and EE's collar bone was broken.
