Alleyton Resource Company, LLC. · Struck by powered moving object

An employee was fatally injured during maintenance work on the hopper. EE was operating a CAT forklift in close proximity to a Volvo excavator. EE exited the vehicle without notifying the other employees working in the area. Other employees stated they heard a "pop" and found the forklift operator unconscious next to the fork of the forklift.

Multisource Sand & Gravel Co., LTD · Struck against stationary object

Employee was in the process of using a pry bar to roll the roll-away box back into place after performing maintenance on the shaker. The pry bar slipped out of the pivot point and EE's right hand contacted the right edge of the roll-away box. Although EE was wearing gloves, the force from the contact partially tore EE's right pinky finger nail and drew blood from the nail bed.

EE was using a cordless drill to drill a hole in a piece of rubber. While holding the rubber down with right hand, the drill slipped. The bit ripped through the glove and cut EE's middle finger on the right hand. EE was taken to the hospital and received stitches and medication.

Employee was using a weed eater around the scalehouse. After they were done with the task, they noticed their left eye was irritated.

While moving pipe for hopper support, the pipe clamp came loose causing the pipe to fall. Upon top end of pipe hitting the ground. The bottom end rose into the air, striking employee in the face. Flipped the employee and they landed on their stomach.

Two employees were returning the two piece "Auto Crane" boom on a service truck to the traveling position. One employee pushed on the boom extension and the injured employee's right finger got caught between the two pieces of the boom.
