EE WAS ATTEMPTING TO REPLACE DRIVE BELTS ON FEEDER OF PRIMARY CRUSHER. DURING THIS PROCEDURE EE S 5TH FINGER ON LEFT HAND BECAME CAUGHT BETWEEN SHEAVE AND BELT RESULTING IN LACERATION AND BROK EN BONES OF LEFT HAND FIFTH FINGER.

EMPLOYEE CLIMBED ONTO CONVEYOR BELT WITH NO KNOWLEDGE OF THIS ACTION TO THE OPERATOR. START SEQUENCE WAS ALREADY INACTED. EMPLOYEE TRAVELED UP BELT AND WAS EJECTED ONTO STOCKPILE APPROX 5'. AN OTHER EMPLOYEE TRIED TO WARN AND INTERVENE.

WHILE CLEANING TAIL PULLEY, A ROCK FELL FROM AN OVERHEAD CONVEYOR THAT RUNS ABOVE THE AREA ACTUALLY BEING WORKED ON. MINER WAS STRUCK ON LEFT THIGH, RESULTING IN A CONTUSION.
