ADJUSTING CONVEYER BELT AND BOLT ADJUSTING GOT TIGHT. USED SOCKET AND BREAKER BAR TO LOOSEN, BOLT STARTED TO TURN FREELY AND PINCHED FINGER BETWEEN BREAKER BAR AND STEEL PLATE.

TAKING PLASTIC PIPES APART WITH WRENCH & HAMMER,HAND SLIPPED AND PUNCHED LEFT RING FINGER AT THE CREASE WHERE FINGER & HAND JOIN.ONE STITCH.
LOOSENING UP A LOCKNUT, HAND SLIPPED AND HE HIT HIS KNUCKLE ON THE LEFT RING FINGER AGAINST THE BRAKE HOUSING COVER. EMERGENCY ROOM DOCTOR CLEANED IT OUT, PUT IN TWO STITCHES AND GAVE HIM A TE TANUS SHOT.

FROM A LEVEL AREA THE GROUND SLOPES DOWNWARD TOWARD THE JAW CRUSHER AREA. A THROUGH, APPROX. 2' WIDE BY 6" - 8" DEEP. CAUSED BY RAIN WATER RUN-OFF HAS EXPOSED NUMEROUS SIZES OF ROCKS. EMPLO YEE WAS WALKING TOWARD THE JAW CRUSHER AREA (AT TIMES IN THE PLANT WAS NOT IN OPERATION AT THE TIME OF INJURY.
