October 30, 2025 TX · Metal/Non-Metal crusher attendant/operator, pan feeder operator/worker SLIP OR FALL OF PERSON
Vulcan Construction Materials, LLC · Fall from machine

On 10/30 employee had filled crusher with diesel & was descending the ladder. As EE was stepping down their foot slipped causing them to hang onto the handrail by their right arm. First aid was administered from that day forward. On 11/14 EE requested to go to the clinic. Diagnosed with strain of their right shoulder rotator cuff. Prescribed medication and physical therapy.

Employee stepped over a trailer hitch and caught boot on the hitch which caused employee to trip and fall to the ground. Employee was diagnosed with a fracture in the left humerus.
