ROOF BOLT HELPER WAS BENDING A RESIN ROD AND PUSHING IT UP IN THE HOLE WHEN HE PULLED ON IT TO STRAIGHTEN IT IT TURNED IN THE HOLE AND THE PLATE CAME AROUND AND HIT HIM IN THE NOSE
