CREW OF MEN LEFT SURFACE AT 7:00 AM. 7:04 HAD TRAVELED 250' INBY ROADWAY DRIFT #3 ENTRY AND FOUND THE TOP HAD SWAGGED ABOUT 6". MEN BACK ON SURFACE AT 7:15. CHECKED THE INBY SIDE OF SWAG AND F OUND TOP WORKING. CALLED FEDERAL AT ABOUT 9:15 AM. CHECKED AGAIN AT 10:30 AM, FOUND THE TOP HAD FALLEN.

MAN WA USING A HYDRALIC HAND DRILL IN THE FACE DRILLING A CUT OF COAL, PREPARING IT TO BE SHOT. HE WA WITH DRAWING THE DRILL AFTER HE HAD DRILLED THE HOLE UP AND HIS ELBOW HIT HIM IN THE RIBS .

THE TRAM CHAIN BROKEON JOY 16 RB CUTTER. THE REPAIRMAN WAS SPLICING BACK THE CHAIN WITH A HAMMER AND A PIECEOF THE HAMMER CHIPPED AND FLEW INTO HIS INDEX FINGER.

MINE FOREMAN WAS MAKING AN INSPECTION OF MINES BEING REOPENED, FOUND A ROOF FALL ON THE #1 BELT INBY THE 4TH OPEN X-CUT FROM THE BELT DRIFT. FALL IS ABOUT 100' LONG AND 20' WIDE, 3-4' HIGH.
