Pioneer Aggregates, Inc. · Fall to the walkway or working surface

The employee was doing a ground level repair to a water valve. While exiting the repair area, they attempted to step over a water pipe and a connected hose. Their feet did not clear this structure, and they lost balance and fell onto a concrete slab causing injury to their right shoulder. No safety rule violations were attributed to this incident.

The miner was picking up a 4FT wire rope sling. As EE slid EE's gloved hand through the end loop, it pushed EE's shirt sleeve up, exposing EE's arm. When EE lifted upward, the cable rotated on EE's forearm. A broken wire strand on the sling loop penetrated the skin, became lodged and required medical treatment to remove it. The injury was minor.
