On June 6, 2014 a dragline slid into the water filled pit. No injury or illness was suffered, and no lost work day or restricted days were incurred. The North American Coal Corporation is and was contractually obligated to and is and was legally responsible for operating and maintaining dragline with its own employees.
Employee was pulling a hose from the service truck to the portable crusher diesel tank. The hose became entangled on the stairs. As he pulled on the hose it became entangled causing his elbow to strike the diesel tank.

A welder was working on the bucket of a dragline. It was a hot day so the welder decided to set up a blower motor aimed at him to cool off. While using the air chipping hammer a small flake of metal was blown under his welding hood and into his right eye by the fan. The employee was taken to workers comp where a doctor removed an embedded metal fragment from his eye.

We experienced a fire on our 6400 Dragline in the area of the main engine. Two operators began to fight the fire & then firefighters from the Fire Department extinguished the blaze & pumped foam to avoid re-ignition. No injuries were sustained. Investigations showed a lost 24 volts carrier from starters near fuel lines could have caused it.

WHILE OPERATING THE CRUSHER, DIGGING ROCKS OUT O THE CRUSHER, THE LEFT ARM STARTED SWELLING FROM THE LOWER PART INTO THE HAND CAUSING PAIN.
WHILE GETTING OFF MACHINE, WAS ON TRACK STEPPING ONTO LADDER, FOOT SLIPPED, HIT HIS KNEE ON THE TRACK PADS.
EE FELL BACK OVER A BOULDER, INJURING HIS LOWER BACK.
WHILE STANDING ON THE STABILIZER ARM OF THE CURSHSER FUELING IT, HE REACHE DUP HIHG, LOST HIS BALANCE. THE WIND WAS VERY STRONG THAT DAY EE FELL HITTING THE OUTSIDE CORNER OF THE STABILIZER A RM. THE FALL WAS APPROX 3' LANDIN GON HIS BACK, INJURING HIMSELF.
EE WAS AT THE TOP OF THE MAST ON THE CRUSHER ATTEMPTING TO PUT THE BOOM CABLE BACK IN THE SHEAVE AND USING A PRY BAR WITH HIS RIGHT HAND. THE LEFT HAND WAS BALANCING THE SHEAVE WHEN THE CABLE CAME DOWN THE OUTSIDE LACERATIN HIS LEFT HAND.

THE WELDING TRUCK WAS STARTED WHILE IN GEAR BY ANOTHER EE , WHO HAS SINCE BEEN TERMINATED, CAUSING IT TO LUNGE FORWARD AND PIN EE BETWEEN IT AND ANOTHER WORK TRUCK SITTING IN FRONT OF THE WELD ING TRUCK. HE SUSTAINED BRUISES ON HIS LEFT LEG AND UP TO HIS STOMACH, ALONG WITH A CHEST WALL SCONTUSION.
CAUGHT FINGER LEFT HAND, BETWEEN THE TAILGATE AND BUMPER ON SUPERINTENDENTS PICK UP TRUCK.
