There was a loose guard over a pulley, and the Miner didn't follow company and MSHA Safety Policies to shut down and LOTO the machine before attempting to repair. He attempted to make the repair while the conveyor was running, and got his fingers caught in the pulley, cutting two of his fingers badly.
