While operating the scaling machine W/GS-2, jarring of the scaler cab had resulted in back strain/pain. The employee immediately reported this to his supervisor, requesting medical attention.
EE AND COWORKER WERE PLACING A METAL RAMP INTO POSITION TO LOAD A TRUCK. METAL PLATE SLIPPED OFF FORKLIFT AND STRUCK EE'S UPPER RIGHT FOOT. EE WAS WEARING STEEL-TOED SAFETY SHOES AT THE TIME.

WHEN DIGGING POST HOLES WITH TRACTOR DRIVEN AUGER, EE S GLOVE WAS SNAGGED BY PROTRUDING BOLT ON ROTATING SHAFT. EE WAS SNATCHED AROUND ONCE BY AUGER BEFORE POWER WAS REMOVED.
EE AND OTHER EMPLOYEE WERE INSTALLING FLEXIBLE PUMP LINE USING METAL BANDS. A BAND HAD BROKEN OFF IN THE BANDER AND EE WAS ATTEMPTING TO REMOVE THE BROKEN PIECE, USING A PAIR OF PLIERS. WHEN T HE PIECE FINSLLY CAME OUT SUDDENLY, THE SHARP END OF THE BAND CUT ACROSS HIS HAND WHERE THE THUMB MEETS THE HAND. REQUIRED 5 STITCHES.
