SUBJECT WAS STANDING ALONG SIDE A SCOOP LOADED WITH A CONVEYOR BELT DRIVE. BELT DRIVE WAS BEINGRAISED WITH A WIRE CABLE ATTACHED TO TURNBUCKLESTHE HOOK OF ONE TURNBUCKLE BROKE CAUSING IT TO R ELEASE SUDDENLY. SUBJECT PLACED HIS HANDS IN A PROTECTIVE MANNER AND WAS STRUCK WITH THE TURNBUCKLE.
A ROOF FALL OCCURE DOUTBY THE 001-0 WORKIGN SECITON. THE FALL BEGAN APPROX 80' INBY SPAD 15320 IN THE NO. 8 ENTRY. THE FALLW AS FROM 8' TO 6' IN HEIGHT, 18' WIDE AND APPROX 80' IN LENGTH. N O EMPLOYEES OR EQUIPMENT WERE IN THE AFFECTED AT THE TIME OF THE FALL. A CONTINUATION OF THI SFALL BEGAN ON THE WEEKEND OF 5-21,22-94 APPROX 140' INBY SPAD 15321 AND 15322 ALSO AT APPROX 210'
