Winston Gold Mining Corp. · Caught in, under or between a moving and a stationary object

The Articulation Stop Arm on an LHD was not securely fastened to stop it from articulating. The arm popped off causing the LHD to bend knocking the miner to the ground and pinning EE's legs under the right rear tire.
