EMPLOYEE SAW THIS HAPPEN BUT EMPLOYEE DID NOT COMPLAIN OF BEING HURT UNTIL THURSDAY 5-9-96 PM EMPLOYEE WENT TO DR ON 5-13-96.

FOREAM WAS BREAKING OUT A LARGE BOULDER WITH A WEDGE WHICH IS A PIECE OF TOOTH OFF END-LOADER & SLIVER OF WEDGE FLEW OFF AND HIT HIS HAND. AS HE WAS STANDING ON THE PLATFORM WITH HIS HAND LAYI ING ON THE GUARD RAIL.
