OEPRATOR REPORTED ON FRI 7-19-96, THAT HIS EYE BOTHERED HIM ON THE PREVIOUS NIGHT SHIFT. HE WENT TO A LOCAL DR. THE DR SAID IT APPEARED HE HAD RUST IN HISEYE. WAS SENT TO ANOTHER DR FOR A S ECOND OPINION. SECOND DR AGREED AND WASHED EYE OUT AND SIAD IT WOULD BE OK. OPERATOR THOUGHT IT HAPPENED.

OPERATOR WAS FUELING DOZER WHEN HIS FOOT SLIPPED. HE FELL AND/OR JUMPED TO THE GROUND AND LANDED ON HIS FEET WITH MOST OF WEIGHT ON HIS LEFT LEG. HE WORKED THE REMAINDER OF THAT SHIFT AS WELL AS FRI AND SUNDAY BEFORE REPORTING PAIN IN HIS HIP AND BACK ON 7-15-96.
