ON THE ABOVE D5TE THE DECEASED IS PRESUMED TO HAVE INJURED HIMSELF FATALLY BY OPERATING A FRONT END LOADER WITHOUT PERMISSION OR ANY KNOWNLEDGE BY PLANT PERSONAL. THE DECEASED WAS FOUND LAYIN G ON THE GROUN8 NEAR THE LOADER & THERE WERE NO WITNESSES TO THE ACTUAL ACCIDENT.
