THE VICTIM LEFT HIS NORMAL WORKPLACE AND WENT OUTSIDE TO FRONT OF CONVEYOR CHAIN THAT LED TO PICKBREAKER. SOME LOOSE MATERIAL GAVE WAY CAUSING HIM TO BE CAUGHT IN COAL FEED CONVEYORS. THAT CAU GHT HIS BOOT DRAGGING HIM INTO PICKBREAKER. ACCIDENT RESULTED IN HIS DEATH.
