EMPLOYEE (FOREMAN) DID NOT TURN POWER OFF AND LOCK OUT 486 VOLT ELECTRIC BOX. CUT CABLE AND MADEA SPLICE. WAS ELECTRICUTED. TAKEN TO MOUNT CARMEL HOSPITAL. WE WERE NOTIFIED OF HIS DEATH APPROX 2:00 PM.
