THE VICTIM RECEIVED FATAL CRUSHING INJURIES WHILE WORKING ON A STAMLER COAL FEEDER BREAKER. THE EMPLOYEE FAILED TO LOCK AND TAG OUT THE CIRCUIT. HE WAS A CERTIFIED ELECTRICIAN AND WAS WELL AWA RE OF THE REQUIREMENTS CONCERNING LOCK OUT/TAG OUT PROCEDURES.

EE, SHUTTLE CAR OPR, WAS FATALLY INJURED WHEN HE WAS STRUCK FROM THE INBY SIDE OF THE NO. 2 SHUTTLE CAR IN AN INTERSECTION USED FOR HAULAGE EQUIPMENT. HE WAS SEATED IN THE DECK OF THE SHUTTLE CAR & WAS NOT SEEN BY THE NO. 2 S. CAR OPERATOR, AS THE INVESTIGATION REVEALED.
