A FATAL ACCIDENT OCCURRED AT AN ABANDONED SURFACE MINE WHERE LAND RECLAMATION WAS BEING PERFORMED. THE ACCIDENT OCCURRED WHEN THE D9N DOZER OPERATOR LOST CONTROL OF THE MACHINE AND ROLLED OVER (1 1/2 TIMES) THE SLOPED EMBANKMENT (APPROX. 60 FT) WHERE HE HAD BEEN PUSHING DIRT OVER A 35 FT HIGHWALL.
