THE TRAILING CA6LE ON #2 BOLTER WAS DEFECTIVE IN THAT THE GROUND WIRE WAS INTO, A SPLICE IN THE CABLE. FOR SOME UNDETERMINED REASON THE CIRCUIT BREAKER DID NOT TRIP. WHEN THE VICTIM ATTEMPTED TO START THE M5CHINE, HE WAS ELECTROCUTED. THIS OCCURRED IN #2 ENTRY, ONE CROSS CUT OUTBY THE FACE.
