El titular minero no presentó la información requerida durante la supervisión mediante requerimiento documentario que forma parte del acta de supervisión, de acuerdo al siguiente detalle: i) información referida a precipitaciones del periodo de octubre a noviembre del 2017; ii) registro de monitoreo diario del control del dique de contención y terraplenes; iii) registro de monitoreo programado y ejecutado del dique de protección del periodo comprendido entre enero y setiembre del 2017; iv) reportes diarios de turnos de la planta de relaves y del depósito de relves de Yuracyacu; v) Plan sostenido de capacitación del personal mediante cursos, charlas, seminarios y prácticas de entrenamiento referido al Plan de Contingencia y vi) Registro de simulacros de emergencias ante accidentes en el manejo de relaves y fugas o rotura de línea de tubería de conducción de relaves.
