A FATAL ROOF FALL ACCIDENT HAS OCCURRED IN RIVER MAINS SECTION AT 36 ROOM BETWEEN 3RD AND 4TH ENTRIES TO THE RIGHT OF THE BELT ENTRY. VICTIM WAS HELPING TO SET POSTS ACROSS ENTRY AFTER MINER H AD MOVED OUT OF PILLAR. VICTIM WALKED INBY CORNER , TO RETRIEVE A CRIB BLOCK, AND THE AREA CAVED IN, COVERING VICTIM.

BELT STAND WAS WELDED TO BEAM TO ACT AS GUIDE FOR WINCH LINE. COGER WAS STANDING ON TIGHT SIDE OF OPERATION. LEG ON BELT STAND BROKE, CABLE HIT COGER & PROPELLED HIM INTO BRICK WALL.
