BELT STAND WAS WELDED TO BEAM TO ACT AS GUIDE FOR WINCH LINE. COGER WAS STANDING ON TIGHT SIDE OF OPERATION. LEG ON BELT STAND BROKE, CABLE HIT COGER & PROPELLED HIM INTO BRICK WALL.
