Man basket popped up with employee inside and they struck their chin on the railing.

EE spotting haul truck under loader, the loader operator dropped full bucket on left bedrail, jostling the EE in the cab of the haul truck. Seen at clinic, xrays negative for fracture, released to modifed duty, worked one day modified, seen on 8/26, released to full duty.

Employee was assisting with a Vac-Truck hose in deep, muddy tundra conditions when unstable footing restricted movement and body positioning. While applying force to control the hose, their grip slipped and the hose contacted their right shoulder. Injury resulted in right anterior shoulder dislocation, restricted work duty.

A haul truck driver was traveling on the right side of the haul road and veered hard left rolling the truck onto its side. The EE stated the steering wheel pulled hard left.

Miner walked in front of loader as it was backing up not knowing they put it in drive. Miner made contact with the bucket of the loader and was pushed to the ground. Miner has scrapes and is sore, miner was taken to the Emergency Room as a precaution because they were on the ground and in pain.

Employee was operating Gehl Forklift to move crushing equipment at the Pit when forklift tipped onto side. Window broke, causing a cut to the employee's hand

Employee was assisting with spotting the Clinker Pan Conveyor, and changing damaged pans. As they squatted to spot the next damaged pan, they placed their hand on the conveyor frame to steady themselves when the clinker pan conveyor roller wheel rolled over the employees right pinky finger.

The 3 team members were using the telehandler to pick up the block and chose to use a rope instead of the rated chains on site. The team member was wrapping the rope around the rebar eye in the concrete block and the telehandler forks and as the telehandler operator lifter the forks the team members left middle finger got caught in between the rope and the rebar.

Truck driver was using weight cart to move weights to calibrate their truck scale, the cart flipped and the truck driver hit ground and broke EE's right arm at EE's wrist.

Haul truck operator reported not feeling well after the truck had been rough loaded.

Air ride seat in John Deere 844L loader. Employee stated the seat was not holding air, management inspected the seat and found it was. Employee clocked out and left the site, contacted HR stating it was a work related spine injury. Safety manager was contacted and is reaching out to employee to schedule medical evaluation.

Mechanics were attempting to pull Sag feed cart from Sag due to plugged cart. A shackle pin snapped and struck employee in right shin, causing a fracture. Employee was taken to ER, referred to orthopedic. Currently ONGOING.

Employee lost supplies off of the mantrip and stopped approx. 10" passed the supplies to reload them. The employee then was struck by the mantrip and knocked to the ground. The employee failed to set the parking brake on a slight incline on the haul road. The mantrip stopped with them underneath of it till help arrived.

Water truck operator stated EE blacked out while traveling west from the loading area. The truck traveled over the berm, tipped onto its side, slid down the pond slope, and came to rest upside down in the water. The operator regained consciousness, exited the truck, swam to shore, and walked up the hill for assistance. EE was transported to the hospital.

An employee stated that they received a jolt to their back when a heavy load of material was dropped into their haul truck from a loading unit. The mine site was not made aware of this until 8/26/2026. The employee returned to work on 9/1/2026 after calling in sick multiple times.

Miner reported having sore/stiff neck from ramp conditions and looking sideways throughout shift.

Brakes failed on our crane after being tested resulting in the operator jumping from the crane. EE injured EE's left ankle, Xray showed a slight fracture. Was released with a walking boot and Advil back to work on light duty.

EE was signaling pump truck operator during concrete pour and lost hold of the hose. As the EE tried to regain control of the hose it whipped and hit the EE's left hand. EE received basic first aid for injury and refused further medical treatment. EE requested medical evaluation due to continued pain on 8/13 and was diagnosed with a distal radial fracture.

The employee was spotting empty barges forward when a wire rope became caught on the wrong side of a kevel. While attempting to reposition the wire rope, the rope under tension struck the employee in the ankle, causing the employee to fall to the deck.

A miner pulled up in company pickup to the sign at the back gate that is used to communicate when blasting is in process or no Blasting in process that day. Employee left vehicle in drive while correcting the sign and as they pulled away from sign the bottom of the sign struck their left thumb causing a laceration to their Left Thumb, closest joint to tip of left thumb.

Employee placed their hand on a stationary safety ladder positioned on the forks of a forklift. Forklift operator was instructed to extend the boom. Ladder struck a piece of steel, ladder bounced and struck EE on the left side of head, resulting in lacerations to left cheek and behind ear. EE was transported for treatment.

EE was on one knee at the lolo and attempted to stand up. As EE placed EE's right hand on the lolo top rail to steady self, EE did not realize that the belt guide rail was so close to EE's hand. The belt guide wheel caught the thumb on EE's right hand. EE did seek medical treatment where EE received stitches in EE's thumb. EE has not missed any work.

An employee was loading a metric ton bag onto a container trailer and their foot was caught between the forklift and the container wall as they were sitting the bag down in the container. The employee did not have their foot completely inside of the forklift cab.

Employee stated EE and other drivers were congregated at the portal to clock out at the end of their shift when the foreman drove through the congested area in EE's F-250 pickup and struck the employee on the left shoulder with the truck's mirror. The employee continued working until 8/17/26, when a physician removed EE from work.

The employee sustained a laceration requiring five staples after guarding contacted them when it was struck by a supply trip.

EE went to a chiropractor with lower back pain on 7/30/26. EE was treated by the chiropractor for the lower back pain. The chiropractor diagnosed the EE with lower back pain due to a repetitive injury from operation of a forklift truck.

Miner was performing housekeeping duties when their arm was sucked into the vac truck hose

Employee was driving a concrete mixer truck approx. 15-18 mph on the plant road toward the river shop. The driver's side front steer tire blew out causing the truck to immediately pull to the left and struck an abandoned fix support structure. This caused the employee to strike the steering wheel, resulting in laceration that required sutures.

On 7/28/26, an employee reported the seat in a haul truck losing air. The employer offered they operate a different piece of equipment, but they did not change equipment. On 7/31/26, the employee notified the employer they were diagnosed with a slipped back disc from the seat not working properly. The cause of the injury is under investigation.

Ee removing feed screw chute when crane overran and caught pinky between snatch block and handrail. Seen at clinic, sutures requred to close wound, no fracture, released to modified duty. Seen at ortho, released to full duty 8/5/26.

TM backed scraper into 2nd scraper.

While pulling forward with the bed raised to plug out, the front wheels lifted off the ground. When the front end came back down, the operator began to feel pain in their back. Went reportable 07/26/2026.

While in #2 entry waiting on miner, EE set down behind #18 stinger (4 wheeler). Section boss backed stinger up, and bumped EE with corner of stinger to back. EE was able to finish shift. On Monday 7/27/26 EE went to get x-rayed, they have EE off work till EE follows up with doctor the following week.

Employee was cleaning the C-3 Conveyor Deck with a shovel while the plant was running. EE's Left Hand got caught by a trough Idler and was pinched between the belt & idler.

At Ostrander portable EE was using 25-30 lb spud bar to dislodge buildup between conveyers when the bar struck a rock awkwardly. Spud bar continued forward and EE's hand went into the conveyor and crushed/pinched L pinky. EE was not wearing gloves and first aid was performed. EE was given stitches and antibiotics at hospital and diagnosed with tip/nail of left pinky tuft fracture.

Ee working from scissor deck. Deck tipped, ee hung onto the upper rail, but reports striking ground when deck landed. Reported the incident, no injury reported. Worked 2 shifts, went on 5 days off. On 7/30 EE reported to NGM that EE was seen by a doctor on 7/28 and diagnosed with a fractured rib. Put off work as of 7/30, filed WC claim 8/4, follow-up 8/17.

Holding on to upper rail of boomlift basket when coming down the basket made contact with the silo pinching their finger causing a fracture.

Employee was operating a skid steer and while backing away from the pile the machine bucked due to uneven ground. When the equipment bucked it caused the employees feet to fly up and come down outside the cab and was pinched between the bucket and the entrance step. This caused injury to both the employee's feet.

EE was cleaning a cross beam (in the redzone) at the head of C25 when EE was pulled into a return roller.

Employee was moving scaffold racks with a sky track forklift when EE drove through a pile of CKD dust and it blew into EE's eyes. EE was wearing safety glasses at the time.

The employee was driving the manlift from tower 7 to the tower 80 work area. During the trip, the manlift traveled over an uneven section of the floor, causing the employee to twist and injure their right ankle.

IE was transporting a concrete block with a forklift and chain when the chain slipped, causing the suspended block to fall onto the employee's right foot.

An employee was rehanging a curtain as a battery hauler started to pull through the other side of the same curtain in a crosscut. The hauler contacted the injured in the lower right leg and knocking down the employee. The employee sustained bruising of the tissue to the lower right leg.

Employee was driving a mantrip when received a call on the radio. Employee looked down to answer radio when they struck their head on a low hanging bolt. PPE was not a factor, mining systems was not a factor, compliance with rules and regulations was not a factor.

Operator on HT456 complained of neck pain and ringing in ear after loading at S59. IP was seen by EMT and sent off site to clinic for further medical evaluation.

Newly hired employee was in a man-basket torquing nuts on Nevada bolts. EE placed EE's hand over the rear of the basket and under the impact torque gun. The operator of the forklift lifted the man-basket thus pinching EE's hand and causing the injury. There was not direct line of sight so effective communication was lost.

While a miner operating a haul truck was parked in a passing bay, another haul truck traveling down the ramp made contact with the bed of the parked haul truck. The miner of the parked truck received first aid for neck pain and was later transported for further evaluation. The miner was given time away from work.
