While a miner was docking the boat in rainy, windy conditions, the motor failed at idle, causing loss of control. Wet, slippery surfaces and an attempt to prevent collision led to a slip and fall, resulting in the miner's right forearm being caught and crushed between the boat and dredge. The miner was assisted and transported to the hospital at - 2:26 AM.

Employee was directed to retrieve a part from another on-site location. A loader blocked the roadway, requiring travel around it on uneven terrain in a side-by-side. While driving, rough ground caused the vehicle to jerk toward the loader. The employee's left hand, holding the safety handle, was caught in a pinch point during impact, causing an open fracture to the left wrist area

Employee was a passenger in a mantrip. While on the ME travelway @ XC 76-77 they ran over an 8" rock, causing the employee to strike their hard hat on the canopy of the mantrip causing pain in neck and top of head.

ind was stacking cable when a johnson super car was backing up struck the individual on EE's right side which resulted in a right hip fracture and bruised knee

Driver and Passenger (IE). Both employees were wearing seat belts when traveling in a Polaris UTV. Took a corner too sharp, turned UTV onto it's side on concrete surface. Passenger sustained elbow laceration.

Was traveling in shuttle car back to miner when a bent rib strap entered the kitchen striking EE on the back of the head. Compliance with rules/regs, PPE, mining equip/systems not a factor. Miner proficiency may be a factor.

Miner was conducting EE's plant walk facing the sand screw, which is a noisy area due to the shaker decks above, EE stated EE heard the skid steer operating, but when EE turned to EE's left to see where it was the skid steer knocked EE off EE's feet and into the bucket, causing shin and elbow injuries.

On May 13, 2026, at 1:15pm, an employee ignored an operator's warning, removed a guard, and reached into a moving conveyor to apply dressing. EE's arm caught, pulling EEs forearm into the headroller & pillow block bearing. Bolt in bearing caused a deep laceration. First aid was given & EMS responded. The worker was hospitalized for surgical repair of the laceration & a muscle tear.

The operator of HT487 was hauling loads out of the 98 shovel for a majority of the night. The operator believed the floor of the pit was rough and while spotting experienced some neck and back pain.

While performing routine cleaning duties, the operator of a Bobcat MT100 mini track skid steer reversed the equipment and made contact with a protruding motor. This resulted in an impact to the operator's lower back

Employee started Light vehicle, EE had a medical episode and accelerated through several berms coming to rest on the bottom of a reclaimed waste dump.

Haul truck operator was involved in a tip-over incident while moving material, resulting in the truck overturning onto the driver's side. Following the incident, the employee received a medical evaluation after beginning to experience blurred vision and nausea. The employee was diagnosed with a neck muscle strain and a lumbar muscle strain.

EE was driving EE's shuttle car in #9 entry when EE hit a hole causing EE to jar EE's back. EE was seen at OA and placed off work beginning 5/1/2026 for a possible pinched nerve.

Employee was walking ahead of a scoop when EE bent over to tie EE's boots and EE's foot was ran over by a scoop tire.

EE was operating the shuttle wagon while loading cement rail cars. EE had EE's head out the window, watching for signals from the ground man. A gust of wind, blew some debris up and under EE's safety glasses and into EE's right eye.

EE was operating the WA 500 loader and ran into a concrete block, causing injury to EE's right shoulder and chip tooth

Employee suffered soft tissue injuries to the foot as a result of foot becoming rolled over by passenger vehicle.

Operator informed their supervisor before start of shift that they were experience back discomfort. After operating a haul truck they informed their supervisor they needed to leave and seek medical attention. Preliminary evaluation indicated potential spine injury. Secondary evaluation diagnosed a sprained/strained musculoskeletal.

While backing up haul truck to shovel, employee had neck pain when the truck struck the shovel dipper which caused the contents to drop in truck bed.

EE was loading ventilation tubes into battery scoop bucket while it was de-energized. EE asked the operator to lower the bucket on float while dragging the tube into the bucket, not realizing EE's foot was under the bucket. The bucket was lowered onto EE's right foot. All X rays were negative, the EE is off due to swelling.

Employee was operating a forklift steering wheel with EE's left hand when EE felt a pop in EE's shoulder.

Haul truck operator hit a couple of potholes and tensed and applied brakes while traveling haul road. Soon after the operator saw a truck sliding towards them, so they tensed up and applied brakes again. In tensing up, the operator felt pain in left shoulder. The operator ran two more loads and, on the way, to tie down, the pain increased.

Driver was returning empty to the pit when EE ran up a berm and laid the cab of truck over

IE was a passenger in a mantrip that was travelling into a track spur. The mantrip struck a dolly that was unseen by the driver, and IE's back struck EE's seat hard. Employee continued to work without restriction until seeking medical attention on 4/24/2026, and began missing days after that. Injury was not the result of a training or compliance issue.

A millwright was moving parts with a knuckle boom when the controller was bumped resulting in an injury to the finger that required stitches.

Shoveling underneath the belt in the beltline. When the shovel caught the bottom roller pulling their hand in.

While operating a John Deere 315 Skid Steer to transfer cement powder out of the mill building as part of a cleaning task, an operator wearing steel-toed boots rested their feet outside the footwell with the bucket slightly raised. This resulted in injuries to the right big toe.

While adjusting the telehandler forks EE pinched EE's finger, resulting in a laceration.

Employee got finger pinched in belt.

Ee was inspecting the end of a parked Genco mantrip, when it rolled forward and Ee was caught between vehicle and tool cart. Ee was seen at local ER and released to full duty. The next day the Ee was experiencing some pain and was taken to local clinic. Dr. advised Ee to stay home the next shift, and return on next regular shift after weekend with no further restrictions.

On 4/14/26 Operator was in 980 front end loader 3301 EE hit a hole operator stated that EE started having pain in left shoulder went to Dr and never missed work Operator was steel having pain went back to the Dr on 4/29/2026 and the Dr put EE off until 5/6/2026

Hauling longwall components, was adjusting tow chains, load shifted causing fracture to the right ankle.

Employee was a front seat passenger in mantrip when mantrip hit coal rib, causing lacerated bottom lip, requiring stitches and fracturing some teeth.

EE was tracking Conveyor belt, and saw an Idler not rotating. EE used their hands to try to break the Idler free, The roller starting rolling which resulted in them being pulled into the Idler. their left arm was pulled between the Idler and belt.

While traveling to the primary crusher, Haul Truck 17 veered into the berm and came to rest on its non-cab side.

Employee had placed the remote control atop the mobile stacker machine's engine compartment box to check stackers position. Employee left remote on box, went to make final adjustments with the remote, and when EE hit the down control, it smashed EE's left pointer finger between the frame and the remote. Stiches required to pointer finger.

An employee was injured while crossing under an unguarded area of moving conveyor belt. The hood on their sweatshirt was pulled into a bottom belt roller, and as they attempted to free it, their left arm was drawn in between the roller and the belt.

While making a turn on SP11 dump in HT 62, the employee bumped their right elbow on the Haul truck steering wheel spoke and caused swelling and right contusion

Employee passed out while driving dump truck.

Employee was loading a truck with 200 lbs rip rap and one boulder fell off and they didn't notice it. They pulled forward running over the top of the boulder, that caused them to jar their back. Went back to work but as the day progressed so did the pain in their lower back and they notified their supervisor.

At -12:45 PM, EE cleaned a return idler while a conveyor was running, without supervisor approval. Despite a coworker's warning, EE continued and was pulled into the belt. Coworkers stopped the conveyor and freed EE. EE suffered minor abrasions. Cause: working on moving equipment without lockout/tagout.

While changing out gearbox on head pulley of 453 belt. While doing so the employees were adjusting the gearbox on the forks of telehandler when employees finger was smashed in between the motor base and a prybar they were using to adjust the placement of the motor base. Employees right index finger received a laceration and fractured fingertip.

Employee was operating an open cab forklift outside when the wind speed increased. The employee got dust underneath their safety glasses and into their eye. They flushed their eye and the following morning had irritation. They sought medical treatment and were given a prescription.

While scaling in 33 West, a rock fell from the back and struck the scaler boom jolting the cab causing the operator to hit the windshield with EE's forehead and right shoulder. The operator was wearing a seat belt. EE as a mild concussion and a strained A/C shoulder joint. Scaler equipped with a lap belt style seat belt.

Employee was helping switch cars in belt spur when motorman #2 pushed cars towards EE. EE was standing near motor when EE's foot was injured. PPE was not a factor, mining systems was not an issue, compliance with rules and regulations was not an issue. Body placement was an issue.

Employee was operating mantrip dragging slick line when rope that was attached to pipe broke and piece of metal from rope struck EE's elbow. PPE was not a factor, mining systems was not a factor, compliance with rule and regulations was not a factor.

Emp was driving mantrip from A6 to lang shop when EE veered off the road and came into contact with rib. Emp. taken the emergency room and EE was given a prescription for Ibuprofen 800mg. No restrictions.

EE reported a strained back from being loaded while operating end dump

Employee was sitting in a track spur when another mantrip came into ride and struck EE's ride. Causing contusion to EE's back and shoulder. PPE was not a factor, mining systems was not a factor.
