Employee was collecting trash from receptacles in employee parking lot and putting the trash in a pickup truck. They exited the truck to pick up trash and while employee was outside the vehicle, the truck began to roll backwards. They tried to jump into the truck to stop it and while doing so, tripped. The truck rolled over their right leg and fractured their right foot.
The employee applied force to the ignition key without maintaining proper grip and control. Contributing factors: use of a key ring with multiple keys; failure to follow supervisory instructions; lack of hazard recognition. Root cause: noncompliance with operating instructions and use of an inadequate key system for safe equipment operation.
The IE was operating a stand-up skid loader on a concrete pad removing spillage. Employee was dumping a bucket over the edge of the pad, machine traveled too far over the edge of pad. Machine tilted forward causing the right ankle to become wedged in a gap on the machine.
Customer tanker truck did not follow loading procedure and pulled forward causing the tanker lid to push the employee who was loading the truck's foot into the access ladder. The event resulted in a high ankle sprain.
Employee was operating a locomotive (LM43) on #4 Section (C-6) track at XC #42, when another motorman pulling a J-Loader on locomotive (LM85) lost control, slid backwards down the hill, and collided with EE's locomotive (LM43) causing two (2) fractured ribs and lacerations of the spleen.
At around 11:30 IP suffered an injury to the left shoulder after the haul truck was jarred driving. The force transferred from the steering wheel to the shoulder, which began to hurt. Medical interventions included a cold pack to help relieve shoulder pain and applying a sling. IP was transported to clinic by their supervisor.

Injured Worker was walking between 2 buses in parking lot prior to work. EE walked out into driveway and was struck by truck going at higher than allowed speed. EE's shin was fractured, ambulance took EE to hospital, and was treated and released for follow up with Orthopedic Dr
While a CAT 777 truck was pulling away after loading, a CAT 992G loader bucket contacted the top of the truck's box. The unexpected impact caused the truck operator to tense up and shift abruptly in their seat, resulting in reported mild back pain.
Material from a broken bag was on the conveyor line. The employee stopped the conveyor and was pushing material down between two conveyors. The employee turned on the conveyor while EE's hand was still in the material. The employee's hand and arm were caught in the conveyor. The employee's hand and arm sustained swelling, bruising, and an abrasion.
Employee was holding the corner strap on a metric ton bag to assist the forklift operator with picking up the bag when their finger was caught between the strap and fork.
At shift change, employee was riding in the back of the man van when the driver hit a pothole and employee was jostled in their seat, claiming bruises to the head and leg. Employee was taken to the ER and currently off work. ONGOING
Was driving down pit ramp and got jarred when hit a soft spot. Did not report till after shift. Did not seek medical attention until 06/29/26. was given full release. follow up on 07/06/2026 was put on TTD.
EE was operating an aerial lift around the plant when they ran over some uneven ground the platform produced a sudden movement causing the EE to strike their left knee against the railing frame.
EE was dumping a load of coal when the trailer started leaning. EE attempted to let it down but it was too late and truck turned over. Injuries to left shoulder and back.
Employee was kneeled by a battery golf cart at a outby sub when a scoop came by, striking the cart and pinching EE's right leg between the cart and the sub. EE was taken by ambulance to the ER where surgery was required to repair fractures in EE's leg.
Employee was working on an airslide that transports kiln feed. When they went to close the port on the airslide kiln feed went up left sleeve and some dust got into right eye. They were given restrictions on 06/26/26 for their left wrist.
Employee was attempting to enter the Genie lift man basket when EE received an injury to EE's right third finger. Employee clipped EE's fall protection harness onto the basket and then gripped the basket railing and entered the basket. As employee's weight entered the basket, the basket slightly suspended, and EE's finger was pinned between the basket and platform railing.
Loader operator ran over a rock in the roadway, jarring the operator.
Employee was operating a rubber-tired personnel carrier when EE entered a crosscut and inadvertently drove over a belt scraper that was lying on the mine floor. As the vehicle passed over the scraper, it cantilevered upward and struck the employee on the left side of EE's face and shoulder, resulting in injury.
EE driving a vehicle hit a berm and received a laceration to the face.
Employee was loading out shot rock and was making berms to contain the boulders as they would fall while EE was digging the shot. Operator was digging on a 45-degree angle to get around a high spot in the floor and a massive boulder fell with a rockslide. The boulder jumped over the berm and hit the right front tire of the loader causing damage to the front wheel and the operator.
The employee identified a material blockage in a rotary feeder and attempted to clear the obstruction using an air lance. While maneuvering the air lance inside the chute, the running rotary feeder pulled the air lance and the employee's hand toward the feeder. The employee's hand became caught resulting in a laceration. No LOTOTO was applied.
IE was a passenger in a mantrip headed outby on the inby side of the mantrip, headed toward the elevator bottom. The mantrip entered a switch that had been left for the turn by an unknown individual. IE was thrown from the mantrip and suffered injury to EE's right knee. Injury was not the result of a training or compliance issue.
Mid-shift truck did hour-long regen burn. Bad smell caused headache. Soon afterward another burn started up. This time threw up. Finished shift in another truck then went to ER.
EE reports backing over a drop-off and experiencing neck pain as a result of the whipping motion that it caused. EE went to ER after incident and was contacted by Clinic the next morning to obtain documentation for the incident.
Miner drove freightliner truck off haul road hit highwall and injured EE's back.
EE was cleaning under secondary with dingo. EE pinched left wrist between the dingo and plant structure. Our telehealth service was called, and they determined the injury first aid. The telehealth followed up with EE several times. On 6/15, they recommended EE go to clinic. Clinic put EE on restricted duty with EE's left arm until 6/30. X-ray showed no broken bones.

Operating dump truck they hit a hole and felt pain in their right knee that went up their right thigh to their lower right back. Initially refused medical. 6/10 complained of pain. 6/11 claim investigation by insurance. 7/10 deemed work related. 7/13 saw Doctor out until 8/24.
During normal loading operations, the Shovel was swinging back into the bed of Haul Truck with the second dipper when the dipper dropped into the bed. The dipper landed half on, half off of the dovetail causing the bed to rise. The front of the truck rose into the dropping back to the ground. Employee went on restrictions on 06/03/2026.
Employee was attempting to track the return side of the conveyor belt by striking the frame of the roller with a hammer to move it backwards. EE missed and EE's hand got caught between the conveyor belt and roller.
An employee was cleaning accumulated mud from around a machine guard when their tool contacted the moving conveyor belt. This contact caused the tool to pull forward, pinching the employee's hand against the gearbox. The incident resulted in lacerations that required medical treatment, including stitches.
EE started down the hill toward the new portal and parking lot EE was loaded at this time with water and the truck started sliding EE pulled the retarder and mashed the fuel and tried to regain control EE was heading toward a dry patch on the good side of the road when made it to the dry stuff the truck was sideways it cause the truck to go up on its offside
I am writing to formally report a dirt bike accident that occurred on the Sand pit property on May 31, 2026, at approximately 4:00 PM. The individuals involved were trespassing on the property at the time of the incident. During the accident, a young man was injured.
Employee was helping load belt structure from the scoop bucket onto a flatcar. The scoop rammed out, the structure slid off the flatcar and one of the chairs accidentally struck employee in EE's foot (fracture). Employee has not missed work. Also, EE was not diagnosed with a fracture to EE's toes until June 15, 2026 after the review of an MRI.
IE was riding on a mantrip when EE hit EE's head on a hanging deluge pipe causing a strain in EE's neck.
Injured employee was attempting to cross the Longwall face conveyor chain at the tailgate end of the face. As EE was stepping across, EE's right foot went down in the opening at the end of the Eicotrack and a chain flight struck EE's right leg, amputating it below the knee.
Employee was shoveling around the secondary plant we a rock fell and struck the employee in the nose and face area.
**** This injury was only a 1st Aid incident initially, but EE was prescribed PT on 6/17/26. While digging out large rock on quarry floor, the loader bounced and EE's shoulder popped. This was not a result from any deficient equipment, simply shifted material while properly operating the loader.
Loose material during loading allowed a rock to roll from the bucket and strike Cat 75, causing a rough load event. The impact broke the rear window. The operator later developed back pain and required clinic evaluation by ERT.
The haulage system was backing up when the back MBC operator notice there was waterline on the low structure and instructed the operators to pull in by. The injured employee had EE's foot outside of the operator's compartment when EE moved the MBC and crushed part of EE's foot between the MBC and the rib/bottom. This resulted in broken bones in EE's foot and a lost time injury.
Had an underground master equipment operator sustain a laceration to their ear while traveling under a drive through curtain. Curtain was torn at bottom and made contact with EE's ear while traveling under.
EE had hit a neutral line with their raised bed. EE states they jumped from the truck and hurt their left ankle but did not report the injury to any supervision. We found out 4 days later and were unable to conduct an investigation due to EE retaining an attorney. This is why this report has been delayed.
While a miner was docking the boat in rainy, windy conditions, the motor failed at idle, causing loss of control. Wet, slippery surfaces and an attempt to prevent collision led to a slip and fall, resulting in the miner's right forearm being caught and crushed between the boat and dredge. The miner was assisted and transported to the hospital at - 2:26 AM.
Employee was directed to retrieve a part from another on-site location. A loader blocked the roadway, requiring travel around it on uneven terrain in a side-by-side. While driving, rough ground caused the vehicle to jerk toward the loader. The employee's left hand, holding the safety handle, was caught in a pinch point during impact, causing an open fracture to the left wrist area
Employee was a passenger in a mantrip. While on the ME travelway @ XC 76-77 they ran over an 8" rock, causing the employee to strike their hard hat on the canopy of the mantrip causing pain in neck and top of head.
