IW exited the cab of the haul truck and hit their head on the truck's headache rack. This caused a laceration to the top of IW head which required staples to close.

Mine Ops Employee caught on/struck by chain going under locked Mine Shop gate.

Employee was walking through the plant to get to the parking lot at the end of EE's shift. EE walked past a flatbed truck with a large piece of expanded metal laying on it and scraped EE's arm along it, resulting in a three-inch laceration, requiring three stitches. No lost time.

Injured was loading supplies on CM (8W #2 51 blk) standing on the miner passing supplies to a miner on the offside. After miner was fully loaded they stepped down off miner and contacted a stored rib strap with left armpit and received a laceration that required 4 sutures to close. No problems with equipment or compliance issues, training or PPE.

The miner was performing routine maintenance as part of their job. As they were bending down to place an impact into the manlift, they hit their eye just above their safety glasses on a piece of sheet metal that was being used.

While cleaning pile in walkway between conveyor and elevator, the employee's left knee made contact with a machine guard on conveyor tail pulley. Upon contact, employee felt pain in left knee and was taken to the ER. Employee was given work restrictions and lost time.

IE was positioning themselves in the driver seat of a mantrip and their knee was driven into the operators box as they sat down. Injury was not the result of a training or compliance issue.

The employee was transitioning/moving from the tugboat back onto the dredge after an impeller had been staged on the tugboat to be taken away. While walking past the impeller, the employee's right index finger grazed against a sharp edge on the impeller, resulting in a laceration. The employee went to have it examined and received stitches to the wound.

Employee was bolting off the miner when reached up to grab a roof strap and struck right elbow on the bolting station. PPE was not a factor, mining systems was not a factor, compliance with rules and regulations was not a factor.

IE was on a crew changing rollers on the beltline. While crossing a designated belt cross-over, IE bumped head against a roof bolt, resulting in a headache. Injury was not the result of a training or compliance issue.

Walking down a flight of stairs, the miner scratched the back of EE's lower left leg on a stair. Scratch introduced staff infection into the blood stream.

Employee sustained laceration on forearm from bolt on hand rail.

The employee entered the feeder/jaw area with fall protection to remove a large boulder that could not be crushed. After accessing the area by lifting a safety gate, the employee began clearing smaller rocks. While sitting down taking a break to drink water, they accidentally contacted the gate, causing it to fall onto their left knee and resulted in a laceration.

Walked under a low section of a conveyor and banged the top of EE's head. EE had previously dropped EE's hard hat and was retrieving EE's hard hat. Requiring 9 staples in EE's head.

Miner was crouching down between support legs of conveyor to look under jaw and then stood up and made contact with crossbar that was part of the support legs. The contact led to a broken nose and cut lip. Stitches required.

Employee was walking to the tool slide in the #4 entry outby #1 unit with EE's head tilted to the side when EE's left ear came in contact with a bent piece of wire mesh resulting in a laceration that required sutures.

Employee was walking along the 7 North Track and the top of EE's hard hat struck the head of a roof bolt in the mine roof. Has pain in EE's neck. No rules or regulations were violated. Employee has training and experience.

Walking under a coal belt looking down due to ground been slippery from rain, EE hit EE's head on a conduit bracket and fell onto EE's back. Incident was reported to supervisor and miner said EE was feeling well. Next day shift EE reported that EE's neck was sore. EE was taken to urgent care for evaluation. The CT scan came out clean, and EE was diagnosed with a concussion.

EE was walking on grating that had been installed in slurry bridge. Crew was in the process of lifting the slurry tank agitator gearbox to do a test fit for the millwrights. Piece was overhead, EE was watching the piece and not watching where they were in relation to the open hole. EE stepped off the grating, there is a clip on the beam that EE struck shin on causing cut about 3".

At plant startup, EE walked under the tail pulley frame of the 400 feed conveyor and stood up too soon striking EE's back on the frame of said conveyor.

Miner was walking up a stairway using the handrails and cut their left ring finger on a piece of sheet metal that was protruding out from the back of shelving that is stored beside the stairs. Miner received four stiches.

EE was moving from the shaft of the screw to the floor of the screw near the top of the machine. When stepping down EE hit EE's knee on the corner of the pea gravel chute that run into the screw. Condition is still under evaluation.

Employee inspecting a hose stood up in a confined space and struck their hard hat on a frame causing their jaw to snap shut and chip a tooth with a minor gum laceration.

Ee was disconnecting a fire hose, walking while looking down, turned EE's head and ran into the outrigger of a crane with EE's face. Fractured EE's front teeth. Seen at Any Day Dental for treatment starting on 4/7 and went back in on 4/8 to complete. Released to full duty 4/9.

On 04/15/2026 management was notified by EE that they had struck right knee while exiting a Toyota Forklift. They further stated that this incident happened on 03/31/2026 at approximately 11:00am. EE has now been seen to have their knee drained due to swelling on 4/16/2026 and 4/20/2026. No lost time or restrictions at this time.

Fractured index finger descending stairs. Hand caught between rail and structure

The employee was assigned to inspect the clinker weigh feeder due to clinker chunks that could plug the system. They lowered themselves through the safety cage to reach the clinker weigh feeder, dropping their hard hat as they moved. They hit the pins with their head as they moved up, causing a laceration.

IE stepped on a sheet of metal and it flipped off of the step and landed on their foot causing a fracture.

Employee sat up, their hard hat fell off, and they bumped their head on the edge of their toolbox. They recieved 12 staples at the Emergency room

Employee struck EE's mouth on a support beam while walking along catwalk. The impact caused bleeding and loosened several upper front teeth, though no external facial injury occurred

Jammed neck by striking a beam with hard hat on. Did not request medical attention until 04/07/2026. 1st Dr appt was on 04/07/2026. Received restrictions upon 04/07/2026 appointment. Diagnosis is neck strain.

Injured stopped EE's motor outby the 9 North 27 block switch underneath a 6" aqualine (water discharge line). When EE stood up, EE struck EE's head on the airline causing pain in EE's lower neck (previously injured 4/28/25). No problems with equipment or compliance issues, training or PPE.

While exiting the cab of an articulated haul truck, employee stated EE's knee made contact with the vehicle's driver's side door panel. Employee was treated onsite by EMTs and released back to duty.

Employee slipped, fell on their back and hit their toe on something that was nearby and also hit their head on the ground. Employee said they felt fine but out of precautionary measures we sent them to the clinic for a check up.

reaching over the bolter they struck their right forearm on a T3 channel resulting in a laceration. They received 4 stitches and came back to work. no time missed.

Employee dropped EE's hard hat on the ground, picked it up and started walking and hit forehead on walkway support beam. Laceration to forehead.

Employee was returning to work from break and attempted to enter the operators seat for packaging from the wrong side. Their foot slipped off the foot rest and they cut their right leg above the knee.

Employee was in shop getting metal for plant, they walked around back of truck hitting right shin on hitch could not maintain balance and fell to ground causing a fracture to left femur.

Employee was walking past a trip of supply cars when their hand came into contact with a standard (metal pipe used to keep supplies on car). This caused them to cut their hand. Mining systems was not a factor, compliance with rules and regulations was not a factor, PPE was a factor.

Miner was completing work on a table in the lab, when EE dropped an item on the floor. As EE bent over to retrieve the item, EE struck the right side of EE's forehead on a corner of the work table.

While laying under a dump truck employee went to turn and reach for a cable and their foot got caught under the axel. The foot and knee didn't twist with the rest of their body, causing them to twist their knee. Employee did not report injury until 2/3/2026

Equipment operator was exiting the rear passenger seat of a pickup truck and stuck their knee against the door causing injury. Operator was place on restrictions.

Employee was stepping over a pipe when EE struck EE's right knee on the flange connection and cut through EE's pants, cutting EE's knee.

EE was walking back to Shuttle Car from the Continuous Miner, in the #3 Entry on 001 MMU, when EE struck a Roof Bolt strap hanging down from previous mining. EE complained of head and neck "jamming" together causing pain to neck and shoulder. EE seen family physician on 12-22-25, and was placed off work until additional testing/imaging is completed.

EE kicked frozen outrigger pad with right leg. EE initially felt no pain but 8-10 hours later, noticed some pain and swelling of the right knee. EE was accompanied by safety to an orthopedic clinic for exam the following day where they were prescribed steroids and given no lifting/pushing/pulling restrictions pending follow up.

On January 29th, 2026, Mine management personnel received info about an injury that happened on December 3rd, 2025. Employee received a laceration behind EE's right ear from a roof bolt that required 3 stitches. The employee did not notify mine management of the incident, went home and went to the clinic on EE's own without notifying mine management of the injury.

Employee banged EE's left shoulder on a trunk duck at the three-way in the plant. EE then tried to pick up a hose that was laying on the floor and EE heard a pop and felt a tingling sensation and pain going down EE's right arm.

The driver was completing a pre-trip inspection on the trailer. Driver was crouched down and stood up, thinking they had enough clearance, and hit their head on the trailer. Driver was wearing a hard hat, which compressed their neck.

Employee was working on 154 plant weather was sunny temp was 82 degrees. the plant was clean of debris. Employee was bent over went to stand struck head on I-beam
