Rock was stuck in grizzle bars. Loader Operator feed large rock to dislodge. Rock did not dislodge. EE got into the feeder with a sledgehammer to dislodge the jammed rock has EE swung the hammer EE missed the rock and struck the grizzle bars which caused the hammer to ricochet causing EE to lose EE's balance and falling approximately 18 to 20 ft.

2026 24 fatalities
Employee was cutting out the parent steel floor from a haul truck bed. While Standing underneath the section of plate being cut out the strap pulling on the plate broke and plate sprung back striking them. Resulting in loss of arm and multiple back injures.
Unbolted flange connection to swap out gasket. While aligning flange bolt holes to install bolts the pipe shifted and rolled off dunnage and crushed worker. Which resulted in a fatal head injury
situation still under investigation so far information gathered is startled worker tripped and broke spine, further information still being gathered. Driller died after being struck by falling rocks.
Employee was completing maintenance task and was fatally buried.
Fall from height from 5th floor to 4th floor through open hatch. Employee was Canadian National and does not have social security number.
At roughly 12:30 am on Saturday 7/11/26 employee was overcome by clean coal while shoveling spillage out from bottom of Clean Coal Silo. Employee is currently in hospital. Vitals are stable but final outcome still TBD. ***EE passed away 07/20/2026***
At the time of filing it is believed the miner was riding on the back of a railcar when the railcar derailed and pinned the miner between the railcar and a nearby structure. The miner was transported to a medical facility where EE died the following day.
Rear-end collision involving two haul trucks on a haul road.
Employee was discovered at shield 74. The employee was found caught between the shield resulting in a head crushing injury. Employee was transported to the surface and EMS performed CPR until time of death was called.
Contractor equipment transporter was in process of loading and securing front end loader to trailer for off-site transit. Contractor employee reached under the loader to grad a chain on trailer and was caught between the loader and trailer.
A contract mechanic/welder hired by another contractor was fatally injured when the ammonium nitrate auger EE was attempting to extract from a pipe exploded.
EE and another employee were pulling the engine and removing nose cone on the radiator. EE was on the bottom under the 992g wheel loader. The other employee was on top of the machine. EE was taking the radiator nose off, accidentally took bolts out of counter weights and it fell on EE. Resulting in a fatality.
Employee was setting a post at 24xcut in 7 South #6 track entry, to barricade off an area of concern. While tightening up a post a piece of rock dislodged from the upper portion of the rib and fell striking the employee. The employee suffered fatal injuries as a result of this event.
The employee was tasked with helping fill water tank. EE was found about 30 minutes later laying face down saying EE was hurt and needed help. At this point it appears that the weight of the trailer shifted off the cribbing under the landing gears and caused the trailer to overturn, either striking against or rolling over the miner.
An employee was working on a winch rope on a 650 scoop that was loaded with a shield, this shield was hit by another 650 scoop, causing the employee to be caught between the scoop EE was working on and the shield tip.
Employee was setting timbers for the next retreat mining lift inby spad 21417 when the slab cut entry that was previously cut fell to the roof bolts causing draw rock to strike the employee. Employee was coherent and alert while being transported outside. During transportation to medical facility they suffered a cardiac arrest. Incident remains under investigation.
Employee unhooked lanyard and fell through roof resulting in fatal injuries.
At approximately 05.12am on the 12th March 2026, a mechanic was struck by a drill boom in a surface workshop, causing fatal injuries.
The IE was moving the miner to the face in entry #3 when we found the IE pinned between the miner tail and the solid rib.
Unsure of what conditions contributed to the death
Nobody saw the accident and we have not been able to interview the EE
Third party delivery driver of raw materials was found deceased trapped between the frame and dump bed of EE's I-Bar owned and operated delivery truck. The driver was located on the West edge of the iron ore stockpile.
During preparation for dismantling of motor and gear box on slide gate. Employee didn't follow given instructions and removed all bolts from gearbox. That cause released of stored energy on slide gate and motor with gear box start spinning. Employee got caught by the equipment and started spinning with it. During spinning their head hit the handrail which caused a fatal injury.
