A roof fall occurred int he 4th s.w. sub panel. the #6 entry on the #2 section, id #027-0. The fall was located 130 feet outby the section tailpiece and measured 100' long, 20' in width and 8' thick.

The roof in the tailgate entry of the 7 east longwall section fell in at 19 and 50 survey tag. Cause of this incident was failure of prop setters. Fall did not obstruct ventilation and the tailgate blockage plan was followed. Initial fall was approx 17' wide by 18' high by approx 9' high.

THE FACE IN 27CN ON THE 1400' LEVEL WAS SHOT AT 6:30 AM. WHEN THE PRESHIFT CREW WENT UNDERGROUND TO CHECK THE FACES, THEY FOUND METHANE GAS DISCHARGING FROM SOME OF THE BLASTED HOLES. THE METHANE WAS ON FIRE, GENERATING SMALL FLAMES ON THE WEST HALF OF THE EXPOSED FACE.

WHILE ATTEMPTING TO REINSTALL A CABLE ON THE PULLEY FOR THE MAN CAGE GATE, A 4 X 4 X 6 PIECE OF WOOD WHICH WAS BEING USED TO PROP THE GATE UP FELL DOWN THE SHAFT AND DAMAGED THE BELL SIGNAL CABLE. THE SHAFT WAS INSPECTED, THE CABLE WAS REMOVED AND THE HOIST PUT BACK IN SERVICE.

THE ROOF HAD BEEN SUPPORTED WITH 4'TORQUE TENSION BOLTS, SUPPLEMENTED WITH 8'CABLE BOLTS. THE FALL OCCURRED ON A NON-PRODUCING SECTION.

A roof fall was discovered by the mine examiner in the No.3 unit, worked out area. The fall is 5 crosscuts south of MPL Np.7 and extends for approx two crosscuts or 100'. The fall is 19' wide and 4-5' high, MMU 003-0. This worked out area will be sealed in the near future.

UNIT WAS TRAVELING UNDER CREEK AND RAN INTO SOME BAD TOP. UNIT WAS PUTTING CABLE BOLTS IN XCUTS BUT HAD GOTTOEN TO #6 AND A ROCK FALL OCCURRED. IT WAS 40 X 19 X 6 AND WENT FROM #6 ENTRY TO #5 ENTRY THROUGH THE XCUT AND STOPPED 20' INTO THE XCUT.

A rock burst was discovered in the 4000-226 rasie and stope. It showed up on the rock burst monitoring system on Saturday 10-23-04 at approx. 11:30 am. No one was owrking at the time. Damage to the raise occurred at stope level (26 sets up form the sill) and 11 sets above (66 feet). Slight damage occurred int he east stope.

Discovered that a roof fall had blocked travel off the face into the tailgate entry. The fall measured approximately 19' wide, 15' long and 10' high. The air velocities on the face remained above plan minimums. Following implementation of the blockage plan, 3 passes were made and a travelway was reestablished.

No one was on the section when the fall occurred. It was discovered by the day shift mine foreman when he and his crew arrived on the section. Fall was in the #9 entry 1 break out by the feeder, fall was about 6' high and 20 by 25'. Section was abandoned.

INBY 55 142) WHEN A ROCK MEASURING ABOUT 5' THICK FELL ACROSS THE RIPPER DRUM OF THE CONINUOUS MINER MACHINE FOULING SUCH MACHINE IN THE PLACE. A RETRIEVER WAS USED TO PULL THE MACHINE OUT.

A fire occurred on the front end loader. The operator (victim) was dumping a bucket of refuse rock on a stockpile. A nearby truck driver noticed an oil leak on the loader and radioed the victim to shut down. The victim dumped the loaded bucket, backed up and drove the loader forward. The loader burst into flames. Fire was located between operator's cab and engine compartment.

Changing the shearer bits, when he rolled the drum he felt pain in his lower back. Diagnosed as back strain.

EE WAS DRIVING A HAUL TRUCK, WAS LOADED AND HAD BEEN HONKED OUT BUT DID NOT PULL OUT IMMEDIATELY. SHOVEL HAD RETURNED TO THE BANK, THEN OPERATOR REALIZED THERE NEEDED TO BE A VISUAL OF TEETH FOR DRIVER. AFTER HONKING THE HORN, EE BEGAN TO SWING BACK AROUND AND SAW THE TRUCK STILL THERE AND TRIED TO STOP THE SWING. HOWEVER, THE DIPPER HIT THE BACK OF THE TRUCK, JARRING THE DRIVER.

EMPLOYEE WAS CLIMBING DOWN OUT OF STORAGE TRAILER AND HE STEPPED ON A BLOCK OF WOOD AND LOST HIS FOOTING AND FELL ON HIS RIGHT SHOULDER. EE SHOULD HAVE USED A LADDER AND TIED IT OFF TO THE STORAGE TRAILER TO PREVENT MOVEMENT. THE ON-SITE SAFETY MGR. EXPLAINED TO EMPLOYEE THAT HE SHOULD USED A LADDER.

Employee was standing next to a load of 1-hole roof support boards being lowered by a scoop tractor when several boards slid off the pallet striking him on left leg.

Stepping down off trackhoe-missing last step-jumping to ground causing pulled muscles in back.

DID NOT START LOSING TIME UNTIL 10/13/2004. While moving I-beams, stepped in a hole and twisted his left knee.

EE was working on cont miner. He pulled on a wrench and fell backwards. His foot was hung under machine.

Employee was standing in her office and blacked out due to a personal illness. She fell to the floor and hit her head on a piece of furniture and cut the back of her head and received six stitches.

While cleaning gobb out of back legs on LW shield he slipped and fell and lost control of pressure hose. Hose hit him on his left eye area.

Cleaning components from transmission using air cleaning gun and dirt went under safety glasses and into right eye.

Employee was helping put guard on feeder. The other end of guard slipped out of co-worker's hand, shifting all the weight of guard on injured employee. The weight from the guard jammed right hand of injured employee against metal feeder frame.

OPERATOR ATTEMPTED TO MOUNT HIS MACHINE. WHEN HE WENT TO GO FROM THE 1ST TO THE 2ND STEP HIS FOOT SLIPPED OFF THE 1ST STEP. HE FELL BACKWARDS HITTING HIS RIGHT HEEL FIRST.

EMPLOYEE WAS WALKING WHEN HIS LT. FOOT SLIPPED & TWISTED HIS KNEE. HE WORKED UNTIL 10/11/04 WHEN HE WENT TO THE HOSPITAL & WAS DIAGNOSED WITH A SPRAINED KNEE, REFERRED TO A SPECIALIST ON 10/12/04. HE WAS TOLD TO REMAIN OFF OF WORK UNTIL RE-EVALUATED ON 10/19/04.

EE WAS WORKING ON 7A SECTION CHANGING BITS ON DRUM. WHILE MANUALLY ROTATING DRUM LEFT FOOT SLIPPED CAUSING EE TO FALL BACKWARDS, TWISTING HIS LOWER BACK. RCVD X-RAY ADN CT SCAN, DIAGNOSIS PULLED/STRAINED MUSCLE.

Employee was trying to get the tie down blocks out of the screener. He was prying with a long bar. The bar slipped and the employee fell approx. 6 ft to the ground.

Ee was using a cutting torch when the torch back-fired due to grease splotter.

He was blackjacking gears on crusher when the gears grabbed the grease paddle causing the paddle to kick back and smack the top of his right hand.

EMPLOYEE WAS ON 001 SECTION IN #5 RT. HELPING CHANGE THE OPERATOR SIDE OF THE RIPPER JACK. THE BOTTOM OF THE JACK HAD BEEN INSTALLED & THE HOSES ATTACHED. THE JACK WAS CRADLED IN A CHAIN & WHILE ATTEMPTING TO ALIGN THE TOP OF THE JACK IT FELL ONTO HIS RT. RING FINGER; PINNING IT BETWEEN THE JACK & THE PAN.

EE WAS THROWING A TRACK SWITCH AT THE TOP OF THE TRACK SLOPE. THE SWITCH THROW HANDLE SPRANG BACK AND TWISTED EE'S RIGHT WRIST.

WALKING IN ENTRY AND STEPPED ON A ROCK AND STRAINED HIS RIGHT KNEE. HE CONTINUED WORKING UNTIL 10/14/2004.

Injured was pushing on motor and felt pain in left knee. This was originally a NLT injury. Injured did not begin missing work until 11/11/04.

Struck by falling object

Employee was placing stone on a steel stand-up pallet. He placed a piece of stone on its edge on the pallet, let go of the stone and took a step backwards with his left foot at which time the stone fell towards him striking his right shin. Did not seek medical attention until 10-11-04.

THE EE STATED HE WAS PREPARING TO HANG A SOLID CURTAIN IN THE #2 ENTRY WHEN THE CONTINUOUS MINER BACKED THROUGH THE FLY PADS PINNING HIM AGAINST THE RIB WITH THE BOOM OF THE CONTINUOUS MINING MACHINE. THE EE CONTINUED TO WORK UNRESTRICTED UNTIL 10/11/04 AT WHICH TIME HE WAS PLACED ON RESTRICTED WORK BY A MEDICAL SPECIALIST.

HE WAS PULLING CABLE; PAIN STARTED IN BACK AND NECK.

EE WAS TIGHTENING LUGS ON C/H TIRE LOCATED IN ENTRY #3, 1 SE PANEL, X-CUT 3+50. A ROCK 3' X 19"" X 3"" FELL FROM BETWEEN BOLTS, STRIKING EE ON R. SIDE OF HIS HARD HAT. IT CONTINUED DOWN EE'S R. ARM, STRIKING THE BACK OF HIS HAND BEFORE GLANCING OFF HIS R. THIGH. EE RECD. FIRST AID & FINISHED SHIFT. LATER WENT TO ER DUE TO SWELLING & STIFFENING. BROKEN CARPUS BONE - CAST APPLIED.

EE HAD BEEN CLEANING A FLOOR DRAIN IN THE PREP PLANT. HE STEPPED IN THE OPENING, TRIPPED AND FELL FORWARD. HIS HARD HAT FELL OFF AND HIS HEAD HIT AN UPRIGHT BEAM.

EMPLOYEE STEPPED FROM A FLATCAR WHILE UNLOADING TIMBERS AND STEPPED ON A ROCK, TWISTING HIS KNEE. STARTED LOSING TIME ON 1/24/05. HAD SURGERY 1/26/05.

Mine mgt became aware of this accident on 11/5/04 Until this time EE told mine mgt that he was sick.It is company policy to report any accident before leaving the property & have an accident report filled out.He did not comply with company policy.On 11/5/04, he alleged to mine mgt that he had injured his groin while lifting a timber.

Employee had taken a pickup truck to pick up some conveyor chains on two pallets. He transported the chains to the prep plant. One of the chains had slid off the side of pallet. He pulled the chain back on pallet so it could be hoisted up into the prep plant and felt discomfort in his back.

While positioning the grate to attach the sling to the hoist, the grate tipped and struck employee on the foot. He suffered a hairline fracture of the right foot.

Third shift outby rolling up belt. Foreman told men not to climb on top of scoop to pull belt & left to go to another area. Employee climbed up on scoop anyway. Some of guys told him not to as well and when he climbed up to pull belt slipped & fell off scoop hitting ground.

EMPLOYEE STATED AS HE WAS LIFTING HIS TOOL BAG OUT OF HIS TOOL BOX HE FELT A PAIN IN HIS LEFT GROIN AREA. HE DID NOT BEGIN MISSING WORK UNTIL 10/9/04. (DIAGNOSED AS HERNIA - WAS SURGICALLY REPAIRED.)

Employee was exiting a motor as he steped out the motor moved causing him to fall. EE felt pain in lower back and shoulder.

EE was working on a ladder about three feet off the ground, tightening hose on the IR drill boom. After he came down from ladder he had a pain in groin area.
