EMPLOYEE WENT ON TO THE TOP OF WAREHOUSE #4 TO CHECK OUT THE WATER COOLER WITHOUT FALL PROTECTION. EMPLOYEE STEPPED ON SKYLIGHT AND FELL TO WAREHOUSE FLOOR 29 FEET BELOW. FALL RESULTED IN FATA LITY OF EMPLOYEE. EMPLOYEE FAILED TO COMPLY WITH COMPANY SAFETY REGULATIONS.
