BASED ON THE INFORMATION RECEIVED FROM THOSE PRESENT IT APPEARS THE IGNITION WAS CAUSED WHEN THE SPARKS FROM THE CUTTING MACHINE (SHEARER) IGNITED AN UNKNOWN METHANE BLEEDER WHEN CUTTING MATER IAL IN WASHED OUT AREA AT SHIELD #80. MSHA NOTIFIED, INVESTIGATION CONDUCTED.
A ROOF FALL WAS DISCOVERED AT 10 00 AM IN THE 1ST RIGHT SECTION AT THE #1XC !4 ENTRY. THE FALL MEASURED 18'X18'X8-9'.
AT 12:50 PM THE MAIN PORTAL ELEVATOR BRAKE WOULD NOT RELEASE. TROUBLESHOOTING REVEALED THAT THE BRAKE PISTON HAD BECOME MISALIGNED AND WOULD NOT ALLOW THE BRAKE TO RELEASE. THE BRAKE PISTON WA S REPAIRED AND THE ELEVATOR WAS PLACED BACK INTO SERVICE AT 3:28 PM.
THE OUTER DOORS OF THE ALIMAK ELEVATOR WOULD NOT OPEN. THIS WAS BELIEVED TO BE CAUSED BY COLD WEATHER. THE DOOR CLOSE SWITCH WAS LUBRICATED AND ADJUSTED, AND THE ELEVATOR WAS PLACED BACK INTO SERVICE AT 2:19 PM.
A BATTERY ON AN ANFO LOADER SHORTED OUT CAUSING EMPTY BAGS OF ANFO TO CATCH FIRE, CAUSING AN EVACUATION OF THE MINE.
A ROOF FALL WAS DISCOVERED AT 6:32 AM DURING THE PRE-SHIFT EXAM. THE FALL WAS INT HE #2 ENTRY, #2XC IN THE FIRST RIGHT SECTION. IT MEASURED 18'20'X8-9'.
AT 4:40 THE MAIN PORTAL ELEVATOR BRAKE WOULD NOT RELEASE, AND LOST POWER TO A BLOWN CONTROL FUSE. TROUBLESHOOTING REVEALED THAT THE BRAKE WOULD NOT RELEASE FULLY. THE BRAKE WAS REPAIRED AND TH E ELEVATOR WAS PLACED BACK INTO SERVICE AT 5:40 PM.

THE FALL MEASURED 60 ' IN LENGTH, 50' IN WIDTH, AND 12' THICK. THE FALL WAS APPROX. 3200' FROM THE ACTIVE SECTION AND DID NOT IMPEDE TRAVEL OR AFFECT VENTILATION. A FALL OF ROOF OCCURED IN THE 7TH S.W.SUBMAINS X-CUT 71 IN #5 ENTRY ON #5 UNIT. THE FALL WAS 130FT OUTBY THE FACE AND WAS LOCATED ACROSS FROM THE DUMPING POINT. THE FALL WAS 95'X 19'X 5.5'. NO EQUIPMENT, VENTILATION OR PERSONEL WERE AFFECTED.
HOIST WAS DRUM FROM 830-1145 AM 2 PHASER WERE RUNT UP IN JUNCTION BOX AT OUTSIDE TRANSFOMER BEHIND THE HOIST HOSE. 3 LEAD WERE REPLACED AND POWER WAS RESTARTED TO HOIST. EMERGENCY HOIST WAS DR OPPED TO BOTTOM AT AIT SHAFT IN CASE OF ACCIDENT APPROX 17 MEN, UNDERGROUND.
THE PRODUCTION HOIST WENT DOWN. A BAD BYTE ON THE OUTPUT CARD OF THE CONTROL WOULD NOT ALLOW THE BRAKES TO RELEASE. A GE REPAIRMAN CAME IN TO DIAGNOSE AND FIX THE COMPUTER PROBLEM.

THE FALL MEASURED 8 0' LONG X 18' WIDE X 6' HIGH. THERE WERE NO INJURIED OR EQUIPMENT INVOLVED AND VENTILATION WAS NOT EFFECTED. THE FALL WILL NOT BE CLEANED UP. MECHANIC STATED HE WENT TO TOP OFF THE RADIATOR COOLANT, HE TOPPED IT OFF & INSTRUCTED THE OPERATOR TO START THE ENGINE. ENGINE WOULDN'T START, SO HE CONNECTED JUMPER CABLES TO THE STARTER, ST ILL WOULDN'T START. WENT TO CAB AREA & SPOKE WITH OPERATOR. OPERATOR REPORTED THE SMELL OF SMOKE. MECHANIC FOUND FLAMES FROM FUEL PUMP & EMC AREA. HE EXTINGUISHED FLAMES WITH HAND HELD UNIT F

WHILE PUTTING PERMIT STAKES IN ARE 3E FOR FINALY PERMIT RELEASE, DRIVER SAW FLAMES IN HIS REAR VIEW MIRROR. DRIVER GOT AWAY FROM FIRE AND CONTACT FIRE BRIGADE PERSONNEL. THE GRASS WAS SET BY H EAT FROM THE CADILITIC CONVERTOR ON THE PICK UP TRUCK. THE PICK UP TRUCK WAS A 1999 F250 FORD TRUCK, AND THE FIRE BURNED APPROX. 40 ACRES.

NO INJURIES, ENTRAPMENT, EQUIPMENT OR VENTILATION INVOLVED. THE FALL WAS IN AREA THAT WAS BUTTED OFF. IT OCCURRED IN #3 ENTRY AT SPAD #433.
WHILE PERFORMING A SHAFT INSPECTIONS, THE CREW FOUND THE #3 HOIST ROPE SLAPPING THE GUIDE ROPES. AFTER FURTHER INVESTIGATION, THE GROOVE IN THE HOIST DRUM FOR #3 ROPE SHOWED EXCESSIVE WEAR. TH E OTHER 3 GROOVES WERE CUT TO ELIMINATE THE PROBLEM.

AN UNINTENTIONAL ROOF FALL OCCURRED AT THE GALATIA MINE MILLENNIUM PORTAL 4N HEADGATE RETURN, #1 ENTRY BETWEEN 635N AND 485N, CROSSCUTS 21 AND 22. THE FALL WAS APPROXIMATELY 150'LONG, APPROXIM ATELY 15'WIDE AND APPROXIMATELY 14' - 16'THICK. THE FALL DID NOT AFFECT VENTILATION. THE FALL WILL BE CLEANED UP.

A DUST EXPLOSION OCCURRED IN THE BIN OF THE OLD TRAIN LOADOUT AS COAL WAS BEING LOADED OUT OF THE #2 SILO. THE COAL WAS TO BE REMOVED AS IT WAS SUSPECTED OF BEING HOT.

DURING THE #1 HOIST PRE-SHIFT INSPECTION, THE MAIN HOIST BRAKE FAILED TO HOLD. THE MINE EVACUATION STARTED AT 10:35 P.M. AND WAS COMPLETE AT 10:46 P.M. THE HOIST BRAKE WAS ADJUSTED, THE HOIST TESTED AND MINERS RETURNED UNDERGROUND AT 11:05 P.M.
THE MAIN PORTAL ELEVATOR DOORS WOULD NOT CLOSE ON THE BOTTOM OF THE SHAFT. TROUBLESHOOTING REVEALED THAT TWO DOOR ROLLERS (PULLEYS) WERE WORN OUT. THE ROLLERS WERE REPLACED, AND THE BOTTOM LANDING SWITCH WAS ADJUSTED. THE ELEVATOR WAS PLACED BACK IN SERVICE AT 6:35 PM.
FELL JUST ABOVE BOLTS FROM SPAD 1103 TO 1429 (3 FULL INTERSECTIONS IN #5 ENTRY. BUTTED OFF #5 ENTRY INBY SPAD #1429 LEAVING A BARRIER BLOCK 60"" THICK. INSTALLED SUPPLEMENT ROOF SUPPORT IN #4 A ND #6 ENTRY BY ADDING ROPE BOLTS AND CRIBBING 4 RIGHT BREAK THROUGHS AND CRIBBING 6 LEFT BREAK THROUGHS IN AFFECT AREA.

THE CONTINUOUS MINER WAS IN THE SECOND LIFT IN THE RT WING OF BLOCK #94 WHEN THE PILLAR AREA FELL ON THE CUTTER HEAD, FOULING IT ON THE BOTTOM. THE MRS WERE SET PROPERLY AND ALL BREAKER POST W ERE SET. THE AREA WAS BOLTED WITH 6' TORQUE TENSION BOLTS. THE ROCK WAS APPROX 10'THICK, 16'WIDE, 30'LONG.
ROOF FALL WAS FOUND ON WEEKLY EXAM. FALL WAS ONE BRK INBY SPAD # 23271. FALL WAS 20' WIDE, 80' LONG AND 7' THICK. ROOF WAS BOLTED WITH 48"" GLUE BOLTS. FALL WILL NOT BE CLEANED UP. FALL WAS 500 0 FT. FROM WORKING SECTION. TOP WAS SLATE.
A FALL OF THE ROOF OCCURRED IN THE 2ND S.E. SUBMAINS, X-CUT 37 IN THE CROSSCUT BETWEEN #3 AND #4 ENTRY. THE FALL WAS 80' X 19' X 6'. NO EQUIPMENT, VENTILATION OR PERSONNEL WAS AFFECTED.

A METHANE IGNITION OCCURRED ON THE (002 MMU) 3 EAST SECTION AT APPROXIMATELY 10:10 PM IN THE #6 HEADING. THE FLAME WAS DISCOVERED BY THE MINER OPERATION. THE FLAME WAS EXTINGUISHED WITH A FIRE EXT. FLAME DURATION WAS ABOUT 15 SECONDS. (NO INJURIES)
SUPPLY MATERIALS HOIST WAS BEING USED TO DROP 5 LOADED SUPPLY CARS DOWN THE SLOPE INTO THE MINE. CAR WAS APPROXIMATELY HALF WAY DOWN WHEN A HYDRAULIC HOSE RUPTURED, SPRAYING THE BRAKE DRUM PAD S. THIS CAUSED CARS TO QUICKLY CONTINUE DOWN THE SLOPE CAUSING A WRECK WITH PARKED SUPPLY CAR ON BOTTOM.
BASED ON THE INFORMATION RECEIVED FROM THOSE PRESENT, IT APPEARS THE IGNITION WAS CAUSED WHEN THE SPARKS FROM THE CUTTING MACHINE (SHEARER) IGNITED AN UNKNOWN (METHANE BLEEDER), WHEN CUTTING MATERIAL AT SHEILD #169. MSHA NOTIFIED AND AN INVESTIGATION CONDUCTED.
AN UNINTENTIONAL ROOF FALL WAS DISCOVERED IN THE #5 ENTRY 1 BRK IN BY SPAD #320 SIZE OF FALL WASESTIMATED TO BE 6' H BY 25' L BY 25' W (INTERSECTION). AREA HAD BEEN SUPPORTED WITH 6' FULLY GROUTED RESIN BOLTS. NO INJURIES, VENTILATION WAS NOT INTERRUPTED. FALL WILL NOT BE CLEANED UP.

THE COAL MILL ON KILN NO. 2 WAS DAMAGED WHEN AN EXPLOSION TOOK PLACE IN THE DUCTWORK LEADING TO THE PRIMARY AIR FAN. STONE AND COAL HAD BUILT UP IN THE MILL'S REJECT TROUGH. THE STONE AND COAL HEATED TO EMBERS. A SPARK OR HOT PIECE OF STONE WAS CARRIED INTO THE DUCTWORK FOR THE PRIMARY AIR FAN AND TOUCHED OFF AN EXPLOSION. NO EMPLOYEES WERE IN THE AREA. THERE WAS NO FIRE.

the #2 operator had stopped int he intersection while the feeder was being cleaned and was beside the frontend of s/c. the #1 s/c came from the miner to dump his load of coal and did not see t he #2 s/c in the intersection and hit it. the #2 s/c operator was at the front end of the s/c when it hit.
EMPLOYEE WAS TRYING TO DROP THE INSIDE OF A CYLINDER INTO THE CASING. HE WAS HOLDING THE CYLINDER BY THE RIM TRYING TO EASE THE PISTON DOWN THE CASING. THE CYLINDER FELL DOWN THE CASING RATHER EASILY CATCHING HIS RING FINGER ON THE LIP. IT CUT THE RING FINGER OFF AT THE FIRST KNUCKLE.
INJURED WAS NOT COMPLETELY CLEAR OF PILE DRIVER AS IT WAS BEING LOWERED TO THE GROUND. IT IS BELIEVED THAT THE PILE DRIVER STRUCK AN I BEAM THAT THE INJURED WAS IN THE PROCESS OF DISCONNECTING A CLEVIS PIN. MOVING BEAM ROLLED AND CAUSED THE INJURED'S HAND TO STRIKE THE PILE DRIVER.
THE INJURED EMP'S RT FOOT WAS SMASHED BY A GATHERING ARM PICK ON A LOADING MACHINE. THE EMP HAD LACERATIONS AND FRACTURES TO RT FOOT AND 4 TOES REMOVED SURGICALLY AS A RESULT OF THE INJURY.
WHILE CLEARING THE DISCHARGE PIPE ABOVE #1 SCREW THE PIPE HE WAS USING SLIPPED AND FELL INTO #1 SCREW. HE INSTICTIVELY GRABBED FOR THE PIPE AND THE SCREW CAUGHT THE PIPE AND PINCHED HIS LEFT I NDEX FINGER BETWEEN PIPE AND FRAME.

EE ASSISTED WITH AN UNDERGROUND SURVEY AND AFTER THE WORK WAS FINISHED EE NOTICED MILE IN THE SHOWER WHAT SEEMED LICKE A HERNIA CAUSE IS UNKNOPWN BUT EE SUSPECTED THAT CARRYING THE SURVEYING G EAR IN A CROUCHED POSITION MAY HAVE CAUSED THE HERNIA.
MEMBER WAS BACKING A HAULAGE TRUCK UNDER THE ROCK BIN. HIS TRUCK STRUCK A HOLE RESULTING IN A BURNING SENSATION IN HIS LOWER BACK.

AS HE DROPPED THE ATRS THE CANOPY FLIPPED A ROOF BOLT STEEL AND THE STEEL ST RUCK THE EMPLOYEE ON THE LEFT SIDE OF THE HEAD. HE WILL BE INSTRUCTED TO ALWAYS STORE THE ROOF BOLT STEEL IN AN AREA CLEAR OF THE ATRS AND CANOPY. HE STARTED MISSING TIME ON 12/30/03.

HE WAS DIAGNOSED AS HAVING A FRACTURED RIB. HE HAS NOT RETURNED TO WORK, AND WILL BE OUT A MINIMUM OF 30 DAYS.

When Mr. Rosado was getting out of a truck he felt pain in his right knee when he landed. Inmediatelly he lost his balance and fell on the ground. He continued to work the complete shift.
EE was dragging several curtins towards the face when he stepped in a hole and twisted his knee. he went to the doctor and x rays was negate. he continued to work and his knee urtained to bother him. he went for an mri on 5-5-04. he got the results of the mri and on 5-7-04 he had surgery to repair a tore in his knee. he started missing on 5/7/04.

OPERATING A BOLTER IN A FLUSH FACE AREA, THE MACHINE WAS LOCATED OUT IN THE INTERSECTION WHICH WAS AN OVERCAST HOLE. HE WAS INSTALLING THE FIRST BOLT UNDER THE BROW, STANDING OUT IN THE OVERCA ST HOLE AREA. A SMALL ROCK FELL FROM THE ""HIGH ROOF"" AREA STRIKING HIS LEFT THUMB. THE INITIAL DIAGNOSIS WAS A HAIRLINE FRACTURE TO THE THUMB.
EE WAS HAULING TWO BUNDLES OF BELT RAILS INTO THE MINE. THE TOP BUNDLE 50 RAILS SLID TO THE SIDE. THE EE ATTEMPTED TO PUSH THE BUNDLE BACK ON TOP AND STRAINED THE MUSCLES IN HIS RIB CAGE, RESU LTING IN A DETACHED MUSCLE FROM HIS RIBS.
EMPLOYEE WAS USING A CAT PAD TO BEND ANOTHER PIECE OF METAL - IT SLIPPED AND SMASHED HIS FINGER - LEFT PINKY FINGER BROKEN.

EE was using a rope and a hole board tied to a roof strap to help untangle a roof bolter cable on a reel. while pulling the cable by hand he was standing underneath the board. the board slipped free and hit him on the shoulder. he worked until he had surgery on may 21, 2004

EE WAS LIFTING A BOTTOM ROLLER AND FELT PAIN IN HIS LOWER BACK. HE DID NOT START MISSING WORK UNTIL 12-22-2003. AN ACCIDENT REPORT WAS NOT FILED WITH COMPANY UNTIL 1-9-2004 ON THIS ACCIDENT.

EE WAS ASSISTING IN INSTALLING BOLTS ON THE LOCOMOTIVE BLOWER STAND SUPPORT WHEN THE 35 TON OVERHEAD CRANE ENERGIZED UNEXPECTEDLY, CAUSING SMALL BREAKS IN THE BLOWER STAND SUPPORT. EE WAS HIT ON THE BACK OF THE LEFT HAND WITH A PIECE THAT FLEW OFF.
EMPLOYEE EXPLAINED TO ME THAT HE WAS CARRYING A SET OF COME-A-LONGS AND AS HE WAS STEPPING THROUGH THE DOOR HE FELT A POP IN HIS RIGHT KNEE.
THE INJURED PERSON HAD FINISHED BOLTING THE FACE OF #2 ENTRY AND WAS PREPARING TO LIFT THE FOOT JACK WHEN HE WAS STRUCK BY A PIECE OF FALLING DRAW SLATE. THIS SLATE HIT HIM ON THE LEFT SHOULDE R. HE SUSTAINED A CRACKED LEFT SCAPULA. HE WAS LOCATED IN A ROOF BOLTED AREA WHEN ACCIDENT OCCURRED.
AN ELECTRICIAN AND INJURED EE WERE WORKING ON AN ELECTRICAL BOX. THE ELECTRICIAN POINTED HIS SCREWDRIVER AT WHERE HE WAS GOING TO WORK & THERE WAS AN ELECTRICAL FLASH.
DID NOT START LOSING TIME UNTIL 5/26/04. HAS BEEN GOING TO DOCTOR OFF AND ON SINCE ORIGINAL ACCIDENT.While walking belt line, slipped and fell and hit head on crib. Strained neck.

WHILE THE EE WAS WORKING AT THE REFEED STATION, BREAKING 50# BAGS OF PRODUCT INTO THE BIN FOR THE REPROCESSING, HE WAS MOVING A BAG FROM THE PALLET TO PUT ONTO THE GRATING FOR THE REFEED BIN BUT HE DID NOT LIFT THE BAG HIGH ENOUGH AND HE STRUCK THE BACK OF HIS RIGHT HAND ON THE EDGE OF THE REFEED STATION BIN. EE IS ON RESTRICTED DUTY AT THIS TIME.

While employee was walking for supplies, he was struck on the hardhat by a section of plastic pipe that was being hung. He missed work on 12/15/03 for related back pain. On 12/23/03 a physician diagnosed muscle strain and prescribed rest and therapy.
EMPLOYEE WAS CUTTING A SECTION OF BEAM OUT OF THE FLOOR STEEL FOR REPLACEMENT. WHEN THE CUT WAS COMPLETED, THE BEAM SHIFTED. HE JUMPED BACK, LOSING HIS BALANCE, FALLING APPROXIMATELY 4' TO THE FLOOR BELOW. HE WAS WEARING A HARNESS THAT WAS CONNECTED TO THE STEEL BUT AT FLOOR LEVEL. THE LANYARD WAS NOT CONNECTED HIGH ENOUGH TO PREVENT THE FALL.
RAINING WET MUDDY-GETTING OUT OF BACK OF PICK UP TRUCK-MUD ON SHOE AND CAUSE AND HIM TO SLIP AND FALL.

EE ATTEMPTED TO PULL IN SHIELD #173. WHEN IT DID NOT COME IN ALL THE WAY HE KNELT DOWN WITH LEFT KNEE ON PONTOON OF SHIELD #174 & RIGHT FOOT BETWEEN #174 & TAIL DRIVE. HE ADVANCED #173 & IT DR AGGED ALONG THE ROOF PULLING THE TAIL DRIVE BACK ABOUT 4"" PINNING HIS RIGHT FOOT BETWEEN #174 & TAIL DRIVE. HE WAS TAKEN TO E.R. WERE X-RAYS WERE NEGATIVE & DIAGNOSED WITH DEEP CONTUSIONS.
SUBJECT WAS GETTING OUT OF CAB OF FORKLIFT BY GRABBING ITS MAST AND PLACED HIS HAND (WHICH WAS GLOVED) IN A PINCH POINT RESULTING IN A SEVERE BREAK TO SEVERAL FINGERS ON THE RIGHT HAND.
THE EMPLOYEE WAS INSTALLING A ROOF BOLT INTO THE DRILLED HOLE WHEN A PIECE OF ROOF FELL STRIKING HIS LEFT ARM & RIGHT KNEE (ROCK APPROX 4"" THICK X 20"" WIDE X 6' LONG).
EE SAID HE WAS SETTING A JACK PIPE IN THE ROOF, TO SECURE TAIL PIECE, THE JACK PIPE FELL AND HIT HIM ON THE LEFT EYE (FOREHEAD).
EE WAS TIGHTENING BOLTS IN A ""DODGE"" COUPLING ON THE MINER HEAD WITH A 1/2"" RATCHET & A 3/4"" DRIVE WRENCH. EE STATED THAT HE FELT A STRAIN IN HIS LOWER BACK WHILE PULLING ON THE WRENCHES.
MECHANIC WAS CUTTING BOGIE PINS WITH AN ACETYLENE TORCH AND LEFT HAND WAS BY HANDLE OF TORCH. THE HANDLE OF THE TORCH WAS LOOSE ALLOWING OXYGEN TO BUILD UP IN THE GLOVE AND IGNITE.
EE STATES HE WAS UNLOADING TIMBERS FROM SCOOP AND PULLED MUSCLE IN HIS BACK.
STEM BLEW OUT OF VALVE BAND ON THE MINER CUTTING LEFT HAND AND FINGERS.
EE SAID HE HIT A BUMP WHILE HE WAS TRAMMING HIS DRILL AND SAID HIT BACK, STARTED HURTING.

A MECHANIC WAS USING A CHAIN HOIST TO PULL A FLOTATION CELL PUMP. HE HAD THE CHAIN HOLDING THE PUMP WHILE HE TOOK THE BOLTS OFF. THE CHAIN HOISTSLIPPED PINCHING HIS FINGER (RIGHT INDEX) CAUSING A LACERATION.
EE WAS DRIVING WEDGES ON A CAP LINE, WORKING ON A PLATFORM ABOUT 5' OFF THE GROUND. WHILE HE WAS DRIVING THE WEDGES WITH A HAMMER THE CABLE SLIPPED OFF ONE SIDE CAUSING HIM TO FALL.
HE WAS REPAIRING A PUMP. HE REACHED UNDER THE PUMP AND CUT HIS RIGHT MIDDLE FINGER ON A SHARP EDGE. HE RECEIVED TWO STITCHES.
EMPLOYEE WAS WORKING AS A BELT CLEANER AND SLIPPED AND FELL.

THE INJURED WAS OPERATING A RADIO REMOTE CONTROLLED CONTINUOUS MINER TO CLEAN UP A ROOF FALL. HE WAS POSITIONED OUTBY THE BOOM OF THE MINER AND WHILE CUTTING AND LOADING OUT THE ROOF ROCK FROM THE FALL A VOLLEYBALL SIZE ROCK WAS THROWN FROM THE CUTTING HEAD STRIKING HIM ON THE RIGHT SHOULDER CAUSING SWELLING AND A CONTUSION.
EMPLOYEE WAS OPERATING ROCK TRUCK AND SAYS HIS BACK STARTED HURTING WHEN HE WENT OVER A SMALL DEPRESSION IN PIT AREA.
EE CLIMBED LADDER TO REPAIR WATER LINE ON SAW. WHEN HE WAS DONE HE WAS CLIMBING DOWN, LOST HIS BALANCE AND FELL OFF LADDER.
EE WAS SWINGING HEAD OF BOLTER, HEAD CAUGHT ON ROCK, STOPPED THE HEAD FOR A SECOND THEN TURNED LOOSE CAUSING THE HEAD TO COME TO FAR OVER AND CAUGHT HIS LEFT KNEE AGAINST THE HEAD OF THE BOLT ER.
THE EE WAS USING A FORKLIFT & SELF-DUMPING HOPPER TO DUMP MATERIAL INTO ANOTHER HOPPER. THE EE WAS REACHING TO RE-LATCH THE SELF-DUMPING HOPPER AND FELT A STRAIN IN HIS LOWER BACK.
THE TIRE HOPPER HAD BEEN CAUGHT BY THE TIRE ELEVATOR CHAIN HOOK CAUSING IT TO ELEVATE. WHEN EE STRUCK HOOK TO FREE HOPPER, HOPPER FELL STRIKING EE'S RIGHT FOOT.
EE STOPPED NEAR THE NORTHEAST CORNER OF PLANT 2 MILLROOM IN ORDER TO FUEL THE PORTABLE AIR COMPRESSOR. WHEN HE STEPPED OUT OF FUEL TRUCK ONTO UNEVEN GROUND (LOADER TIRE TRACK) HE TURNED HIS AN KLE IN AN INWARD DIRECTION.

EMPLOYEE WAS SHOVELING COAL OFF THE TOP VESSEL DILUGE SUMP WHEN HE FELT PAIN IN HIS LOWER ABDOMEN. HE FINISHED THE SHIFT WITH VERY LITTLE PAIN, BY THE TIME HE GOT HOME THE AREA WAS SWOLLEN. HE WAS DIAGNOSED WITH A HERNIA WHICH REQUIRED SURGICAL REPAIR.
EE PICKED UP A POTHEAD W/HIS BACK TOWARDS REAR OF PICKUP TO PUT IT INSIDE. AS HE DID THIS, HE LIFTED & TURNED TO HIS LEFT ALL IN THE SAME MOTION & PLACED THE POTHEAD IN BACK OF P/U. HE FELT A PULL IN LOWER PART OF BACK, RIGHT SIDE, CAUSING SOME DISCOMFORT. HE CONTINUED TO WORK THE REST OF THE DAY, & WORK WEEK, NOTICING HIS BACK WAS A LITTLE STIFF & SORE ON 12/16/03, HE ARRIVED HOME
EE WAS CUTTING A PIECE OF METAL AT THE CONSTRUCTION SITE. A PIECE OF HOT SLAG FELL ONTO HIS FOOT AND BURNED IT. HE DID NOT SEEK MEDICAL ATTENTION UNTIL 21 DECEMBER 2003.

HE RECEIVED A STRAIN/SPASM TO HIS RIGHT MID UPPER BACK. TREATING DR. GAVE EE SOME MUSCLE RELAXERS AND PAIN MEDICATION AND REFERRED HIM TO A CHIROPRACTOR THE SAME DAY. THE CHIROPRACTOR MANIPULATED HIS BACK & RELEASED HIM TO FULL DUT
EMPLOYEE WAS HOOKING UP A BOX ROCK DUSTER TO A BATTERY POWERED SCOOP. HE WAS PLACING A PULL PIN IN THE HOLE. THE PIN WOULD NOT GO IN, SO HE TOLD THE OPERATOR TO PULL FORWARD, BUT HE WENT BACKW ARD AND THE VICTIM STILL HAD HIS HAND ON THE PULL PIN & IT WAS SMASHED AGAINST THE SCOOP BATTERIES.
EE WAS ABLE TO WORK WITHOUT RESTRICTIONS AFTER INJURY ON 10/25/02 UNTIL 12/9/03 WHEN HE BEGAN COMPLAINING OF BACK PAIN DURING SHIFT. AT APPROX 8:30 PM HE BENT OVER TO PICK UP CONTINUOUS MINER CABLE AND AGAIN FELT PAIN IN HIS LOWER BACK. HE CONTINUED TO WORK UNTIL AROUND 9:30 PM BEFORE HE LEFT WORK TO SEEK TREATMENT AT A LOCAL HOSPITAL. HE MISSED 4 WORKDAYS DUE TO THIS INJURY.
WAS WORKING ON SCOOP MAINTENANCE, WAS ON TOP OF SCOOP & SLIPPED ON METAL DECKING. HE INJURED HIS RT. SHOULDER. HE CONTINUED TO WORK UNTIL 1/13/04 WHEN HE WENT OFF WORK.
EE WAS DIGGIN IN A DITCH AND PICKED UP A ROCK TO THROW IT OUT OF DITCH. HE TURNED WRONG AND STRAINED LOWER BACK.
WATER PIPE BUSTED, HITTING HIM ON LEFT SIDE OF HEAD. UPON FURTHER INVESTIGATION, IT WAS SUSPECTED THAT EE SLIPPED WHILE STANDING ON A BUCKET, HE TRIED TO STOP THE FALL BY GRABBING A PVC PIPE, THE PIPE BROKE & HIT HIM ON THE LEFT SIDE OF HIS HEAD RESULTING IN A MILD CONCUSSION. HOWEVER, EE REPORTED THAT THE PIPE SHOOK WHEN HE OPENED THE VALVE & THE PIPE BROKE KNOCKING HIM TO GROUND.
EMPLOYEE PULLED HIMSELF UP ONTO HIS DOZER BY USING THE GRAB RAILS. HIS BACK STARTED HURTING.
EE WAS WASHING DOWN SECONDARY PLANT, WHILE PULLING ON HOSE-STRAINED/INJURED BACK.
WORKING AT THE SHOP WITH COWORKERS AND THE HEAD CAME OFF OF A SLEDGE HAMMER AND HIT HIM IN HIS HEAD.
WHILE DRIVER WAS STOPPED TO MAKE A TURN, HE MOVED HIS NECK TOO QUICKLY AND HURT HIS NECK.
EE STATES HE WAS IN THE WORKSHOP INSIDE THE COARSE MATERIAL WASHER. HE WAS ASIDE THE SHAFT TRYING TO REMOVE AN OLD BOLT USING A HAMMER AND PUNCH(CHISEL). HE STRUCK DOWNWARD WITH THE HAMMER, IT DEFLECTED AND HIT HIS HAND BETWEEN THUMB AND FOREFINGER.
Mechanic exited vehicle and walked towards tailgate; lost footing on snow covering ice and fell flat on his back striking head on frozen ground.

The ee stated he was washing pans when he bent over to pick up a side board he felt a pain in his lower back. the ee did not report the accident until the next day by telephone. this accident report is being filed to abate citation number 7221523 dated 5/25/04. this accident report is being submitted under protest.

EE WAS SHOVELING COAL ONTO CONVEYER BELT. SHE REPORTED THE ACCIDENT ON 12-12-03 WHEN SHE STARTED MISSING WORK.
EE WAS STOOPING OVER TO TRAM ROOF DRILL FORWARD, WHEN HE SAID HE FELT A PAIN IN HIS RIGHT KNEE. DOCTOR SAID HE HAD A TORN CARTILAGE. EE WORKED 12/9/03, 12/10/03, & 12/11/03. LOST TIME BEGAN 12 /12/03.
EE said he threw fuel in stove fire barrell, white smoke started coming out then it blew up on him.
AFTER ADJUSTING THE COOLING WATER VALVE ON A LUBRICATING UNTI 71LU14 FOR FINISH MILL #3 71BM03, THE EE STEPPED ON THE EDGE OF A RAISED FOOTING OF THE STRUCTURE SUPPORTING THE MILL EQUIPMENT AN D TWISTED HIS LEFT ANKLE. THE INCIDENT RESULTED IN A LEFT ANKLE SPRAIN.
EE WAS USING A BIG RATCHET WRENCH TO OPEN COAL MILL DOOR. HE WENT TO PULL UP ON WRENCH AND FITO PAIN IN HGIS L ARM BICEP AREA.
SUPERVISOR WAS TRYING TO PULL BELT OVER WHILE PULLING ON IT WITH A ROPE & SHEAVE. HE GOT HIS FINGERS IN THE SHEAVE WHEEL. CAUSE OF THIS INJURY WAS AWARENESS & AN UNSAFE ACT. COMPLIANCE WITH RU LES/REGS WAS A FACTOR; PERSONAL SAFETY EQUIPMENT WAS A FACTOR; MINER PROFICIENCY & ATTITUDE WAS A FACTOR; MINING SYSTEMS IS A FACTOR.
EE WAS HELPING PUT THE BUMPER BACK ON THE CONTINUOUS MINER. HE STATED LATER THAT HE FELT PAIN IN HIS LEFT SHOULDER. THE INCIDENT CAUSED A STRAIN TO THE SHOULDER AND THE EE STARTED MISSING TIME ON DECEMBER 9. HE WILL BE INSTRUCTED TO ALWAYS USE PROPER LIFTING PROCEDURES. THIS INCIDENT HAS BEEN UNDER INVESTIGATION PENDING MEDICAL REPORTS.

EMPLOYEE WAS USING A HAMMER WHILE REPAIRING A 350 TRACK HOE. NOTE: EMPLOYEE DID NOT REPORT THIS UNTIL DEC 22ND. HE WENT TO THE CO DR. EMPLOYEE WENT ON HIS OWN TO A CHIROPRACTOR WHO TOLD HIM TO TAKE 3 DAYS OFF. WE WILL CONTEST THIS.
WHILE CLIMBING DOWN FROM THE OPERATOR COMPARTMENT OF THE LOADER, THE EE SLIPPED FROM THE SECOND STEP (APPROX 35"" FROM THE GROUND) HE SLID UNDER THE LOADER STRIKING HIS RIGHT SHOULDER.
EE WAS RIDING THE MANTRIP FROM THE FACE AREA TO THE SURFACE. HE ALLEDGES THAT THE MANTRIP HIT A HOLE IN THE TRAVELWAY AND BOUNCED CAUSING INJURY TO HIS BACK. THE INVESTIGATION REVEALED THAT TH E MANTRIP WAS NOT TRAVELING AT AN EXCESSIVE RATE OF SPEED.
WHILE WALKING TO BACK OF ROOF BOLTER TO GET SUPPLIES, EMPLOYEE WAS STRUCK ON THE HEAD BY A ROCK APPROXIMATELY 4' SQUARE. IT SLID DOWN HIS BODY, STOVING HIS NECK, SHOULDER AND BACK.
EMPLOYEE WAS REMOVING USED WOOD PALLETS FROM THE FIRING FLOOR AREA WITH THE FORKLIFT. ONE OF THE PALLETS WAS CRUSHED AND DAMAGED. AS HE PROCEEDED TO DISMANTLE IT WITH A HAMMER, A PIECE OF NAIL BROKE LOOSE AND IMPACTED HIM IN THE RIGHT EYE. AT THAT MOMENT THE EMPLOYEE WAS NOT WEARING HIS SAFETY GLASSES. THE IMPACT LACERATED THE EYE AND HE REQUIRED SURGERY.
EE WAS BOLTING TOP IN #1 ENTRY AND PIECE OF ROCK SLID OFF OF CANOPY AND STRUCK HIM IN HIS RIGHT FOOT. TOOK HIM TO PINEVILL COMMUNITY HOSPITAL EMERGENCY ROOM. THEY ADMITTED HIM FOR OBSERVATION AND TO CHECK FOOT OUT. ROCK CUT A PLACE ON TOP OF HIS FOOT & POSSIBLY BROKE BONE IN TOP OF FOOT.
WHILE PERFORMING NORMAL WORK DUTIES (SHOVELING SPILLAGE) AND FELT A PAIN IN HIS SHOULDER. MEDICAL EVALUATION AND MRI SHOWED BONE SPURS & A DEGENERATIVE CONDITION IN HIS SHOULDER.

THE CHIP WENT BE TWEENT HE TOP EDGE OF THE ENTER GLASSES ONT HE EEYE THE DR GAVE THE EE DROPS FOR THE EYE AND OFF WORK FOR 1 DAY.
EE WAS WALKING ALONG TRACK AND STEPPED IN BETWEEN TRACK AND GUARD RAILLAND TWISTED RIGHT KNEE. THE REASON WHY THIS IS JUST NOW BEING REPOTED. THE EE WAITED TIL 1-21-04 TO HAVE THIS DONE.

EE WAS MOVING A SCRAP PILE OF METAL WHEN A PIECE OF METAL FELL ON HIS FOOT, CAUSING A CONTUSION. DOCTOR X-RAYED & SAID FOOT WAS ONLY BRUISED AND EE COULD RETURN TO WORK. EE STAYED HOME ANYWAY. THEN WENT TO ANOTHER DOCTOR A WEEK LATER AND WAS GIVEN TIME OFF.
WORKING ON BLASTING PATTERN, HIS FEET WERE UNEVEN AND HE EXTENDED HIS ARM TO CATCH HIMSELF FROM FALLING. HE FRACTURED HIS WRIST (LEFT) AND WAS REMOVED FROM WORK.

DID NOT APPEAR TO LOSE CONCIOUSNESS & WAS ALERT & AWAREOF HIS SURROUNDINGS AFTER THE CONVULSIONS STOPPED. EE HAS A HISTORY OF KIDNEY DISORDER & WE FEEL THAT MAY HAVE CONTRIBUTED TO THE DIZZINESS.

EE WAS UNLOADING TOOLS FROM ONE RIDE TO ANOTHER WHEN A SHUTTLE CAR PULLED IN THE X-CUT, STRIKING ONE RIDE & CAUSING IT TO MOVE FORWARD CATCHING HIS LEG BETWEEN THE TWO RIDES.
AN EMPLOYEE WAS UNLOADING 5"" X 7"" X 16' TIMBERS FROM A FLATCAR & STACKING THEM ON THE RIGHT RIB WHEN FELT SOMETHING PULL IN HIS RIGHT SHOULDER & BACK.
EMPLOYEE WAS HELPING MOVE AND LOAD A GENERATOR IN BACK OF A PICKUP SINCE THE POWER WENT OFF.
EMPLOYEE WAS USING A SLATE BAR TO CLEAN THE TRACKS ON THE KOMATSU PC 4000 SHOVEL. WHILE PULLING ON THE BAR HE PULLED A MUSCLE IN HIS RIGHT SHOULDER.
STEPPING AROUND THE FUEL TANK TO SHUT FUEL OFF, HE STEPPED ON THE FUEL HOSE AND HOSE ROLLED RIGHT FOOT UNDER IT.
THE EE STATED HE WAS HELPING MAKE A BELT SPLICE ON THE PONY BELT OF 2 EAST MAINS WHEN THE CHAIN BROKE THAT WAS HOLDING THE SHIEVE BLOCK PULLING THE BELT BACK TOGETHER AND THE ROPE STRUCK HIS R IGHT THUMB.
FROST WAS IN THE MATERIAL THAT HAD BEEN CRUSHED THE PREVIOUS DAY. MATERIAL WAS BEING RELOCATED FROM THE DISCHARGE PILE OF THE CRUSHER. THE TOP OF THE PILE WAS FROZEN. IN REMOVING THE LOWER AR EA OF THE DISCHARGE PILE, THE TOP OF THE PILE CAME DOWN AND CRUSHED OUR EE'S ANKLE AND FOOT. AT THE TIME OF INJURY, HE WAS MAINTAINING THE CRUSHER.
INJURED WAS PLASTERING STOPPINGS AND GOT ""BLOCK BOND"" INTO HIS GLOVE AND ONTO HIS SKIN. THIS LATER RESULTED IN A RASH OR BURN TO HIS RIGHT WRIST.
THE EE WAS INJURED WHEN HE WAS CONDUCTING ROUTINE INITIATION OF A MECHANIZED DEVELOPMENT BLAST. INJURIES RESULTED WHEN THE EE WAS STRUCK BY A SHOP DOOR, WHICH WAS DISLODGED BY CONCUSSION FROM THE BLAST. MINOR DAMAGE OCCURRED TO THE OPPOSING SHOP DOOR AS A RESULT OF CONCUSSION FROM THE BLAST.
EE WAS STRUCK BY A ROCK THAT FELL OUT BETWEEN ROOF BOLTS AND THE RIB. HE WAS STRUCK ON THE LEFT FOOT RESULTING IN SWELLING AND A CONTUSION. HE WAS IN THE FACE AREA ON #1 UNIT, RUNNING A CONT INUOUS MINER. ROCK WAS APPROXIMATELY 31"" X 31"" X 3 1/2"".

EE WAS WALKING INTO THE CONTINUOUS MINER SECTION. AS HE WAS CROSSING A WOODEN BRIDGE HE STEPPED ON A PROTRUDING NAIL WHICH PUNCTURED HIS LEFT FOOT.
EE WAS OPERATING A FRONT END LOADER AND SAID HE STRUCK A HOLE, SAID HE HIT HIS HEAD ON ROOF OF LOADER AND JARRED HIS BACK, SAID HE WAS WEARING SEAT BELT, SITE WAS INSPECTED BY SUPERVISOR. NO H OLE FOUND. DOCTOR IS STATING THAT THIS IS A QUESTIONABLE CLAIM AT THIS TIME.
EE WAS PULLING A 12' CABLE BOLT FROM A PILE IN THE WAREHOUSE YARD. THE BOLT SUDDENLY CAME FREE & IN AN EFFORT TO KEEP FROM FALLING HE TWISTED & STRAINED HIS RIGHT HIP. HE MISSED TWO WORK DAYS.
WHILE CHECKING PUMP HE TWISTED HIS KNEE. EMPLOYEE HAD PRIOR LEFT KNEE SURGERY. HE STARTED MISSING WORK 2/19/04.
While walking on a leachpad liner, employee slipped and his feet went out from under him. He landed on the back of his head and back. This incident occured on 12/02/03. He was taken off the job on 05/21/04 for physical therapy and rehabilitation with the possibility of corrective surgery.
EE HAD PLACED A TRACK JACK UNDER A BATTERY IN THE GREEN CHARGING AREA TO SLIGHTLY TILT THE BATTERY SO THE WATER AND CLEANER WOULD DRAIN OFF THE TOP OF THE BATTERY. THEY DRUG IT OVER TO THE PICK-UP TO LOAD IT IN THE TRUCK. THE PIN AND CAPS WERE PICKED UP BY HAND AT THE REAR OF THE TRUCK. WHILE STEPPING BACK, EE SLIPPED & THEY WERE DROPPED ON RIGHT LEG

INJURED PARTY WAS WALKING THE 1ST ANGLE BELT WHEN A PIECE OF ROCK STRUCK HIM ON THE LEFT SHOULDER AT THE COLLAR BONE AREA.
THE INJURED WAS STANDING ALONG SIDE OF A SHUTTLE CAR, HE THEN GAVE THE CAR OPERATOR THE O.K. TO TRAM THE SHUTTLE CAR. THE INBY END OF THE CA5R SLID INTO THE INJURED STRIKING HIM ON THE LEFT LE G AND LEFT HAND. RESULTED IN A BROKEN FEMUR, ANKLE AND HAND ALL ONT HE LEFT SIDE OF HIS BODY.

EE RACKING ROCK FROM SIDE OF BIN WHERE ROCK HAD CAKED DUE TO WET CONDITIONS WHEN A PIECE OF CAKED MATERIAL LESS THATN THE SIZE OF HARD HAT ROLLED LESS THAN 10 FEET STRIKING THE EE ONT HE LEFT LEG RESULTING IN BREAKAGE TO THE LEFT LEG BELOW THE KNEE.
EE WAS JARRED AROUND IN TRUCK CAB WHEN HYD EXCAVATOR INADVERTANTLY BUMPED INTO THE SIDE OF HIS TRUCK BED ( TRUCK WAS BEING LOADED) EE NOTIFIED OPERATOR ON DEC 31 THAT HIS NECK PAIN HAD INCREAS ED SINCE THE DEC 1 EVENT TO THE POINT THAT HE DIDN'T FEEL LIKE RETURNING TO WORK.

EMPLOYEE WAS JACKING UP THE UNIT TAIL PIECE WITH A TRACK JACK. HE THOUGHT THE JACK HAD LATCHED, BUT IT HADN'T. WHE HE LET GO OF THE HANDLE IT FLEW UP AND HIT HIM IN THE LEFT CHEEK.
EMPLOYEES WERE CHANGING ROTOR ON 130/150 UNIVERSAL CRUSHER AND WHEN THEY LIFTED UP THE ROTOR OUT OF CRUSHER EE WAS GOING TO ATTEMPT TO STABILIZEIT AND IT SWUNG BACK INTO HIM AND PUSHED HIM UP AGAINST THE DOOR AND MADE HIM HIT THE BACK OF HIS CALF MUSCLE ON THE INSPECTION DOOR. IT MADE A BRUISE ON HIS CALF AND I TOLD HIM WE SHOULD GET AN X-RAY.

Employee was climbing onto the dragline on top of the cab to help a co-worker change batteries. As employee was climbing on the dragline he felt a pop in his right knee. Employee is off work until next appointment 12-23-03. Employee received a right knee sprain.

A PLANT MECHANIC WAS REMOVING A 3/4"" CAP OFF OF AN AMMONIA LINE TO PURGE THE SYSTEM. HE SHUT OFF THE MAIN VALVE TO THE TANK BUT NEGLECTED TO BLEED THE PRESSURE OFF THE LINE BEFORE REMOVING THE CAP. THE CAP BLEW OFF ENGULFING HIM IN A VAPOR CLOUD OF AMMONIA. THIS RESULTED IN MINOR BURNS TO HIS RIGHT HAND AND CHEEK AND INHALATION INJURIES.

CAT TRUCK WAS TRAVELING TO B-6 PIT. THE TRUCK SLID OUT OF CONTROL ON A MUDDY, ICY CURVE. THE 35T WATER TRUCK WAS COMING DOWN THE HILL FROM THE SAME PIT. THE HAUL TRUCK SLID SIDEWAYS, THE BED OF THE HAUL TRUCK STRUCK THE CAB OF THE WATER TRUCK.

EE WAS WALKING FROM HIS PERSONNEL VEHICLE TO THE ROCK TRUCK WHEN HE SLIPPED ON THE ROA DWAY CAUSING HIM TO FALL. THE FALL RESULTED IN AN ANGULATED FRACTURE TO HIS LEFT ANKLE WHICH WILL REQUIRE SURGERY TO CORRECT.

WHEN HE REMOVED A COVER ON THE AIRSLIDE, THE MATERIAL INSIDE PURGED OUT ON TO HIM COVERING HIS FACE AND EYES WITH MATERIAL AND KNOCKED HIM DOWN ONTO THE STEEL PLATFORM WHERE HE WAS STANDING ON A 4 FT LADDER.
