ROOF FALL - FATAL - 30'X 30'INTERSECTION - SPAD #1003 - 1XCUT INBY. EE AND CO-WORKER WERE POSITIONED ON THE RIGHT SIDE OF THE CONTINUOUS MININGMACHINE WHEN THE ROOF BEGAN TO FALL WITH LITTLE OR NO WARNING.
