INJURED WAS OPERATING A BOLT MACHINE WHEN A PIECE OF LOOSE ROOF MEASURING APPROX. 5' LONG X 2 1/2' WIDE X 3" THICK. FELL FROM BETWEEN THE RIB AND OUTSIDE ROW OF BOLTS, STRIKING INJURED ON LEFT SHOULDER AND HIP CAUSING A BRUISE.
INJURED WAS PRYING ON A HUMP JACK WITH A SLATE BAR WHEN BAR SLIPPED CAUSING INJURED TO MASH HIS RIGHT HAND, THE SLATE BAR AND A TORQUE SHAFT THAT WAS LAYING ON THE BOTTO, CAUSING FRACTURE OF H IS RIGHT HAND.

EE IS AN ELECTRICIAN & HAD CUT A LARGE PIECE OF METAL FOR A REPAIR JOB & WHILE LIFTING THE CUT PIECE, IT SLIPPED OUT OF HIS HAND STRIKING HIM IN CHEST.
EMPLOYEE WAS CUTTING DOWN ROCK THAT WAS HANGING ON A ROOF BOLT, RIPPER HEAD OF CONTINUOUS MINER CAUGHT THE ROCK AND THREW IT BACK STRIKING EE ON RIGHT THIGH.
A SUPPLY TRUCK FOR BECKLEY WELDING HAD AN ACETYLENE BOTTLE CATCH ON FIRE & THE FIRE BURNED LONGER THAN 20 MIN. STATE & FED. WERE NOTIFIED. BECKLEY WELDING HAS BEEN CONTACTED & REQUESTED TO INS PECT BOTTLES & NOZZLES BEFORE DELIVERY TO OUR SITES.
EE WAS IN INTERSECTION SETTING TIMBERS AS MINER WAS BEING BACKED OUTBY INTERSECTION. THE INTERSECTION ROOF COLLASPED TRAPING HIS LOWER LEGS UNDER BACK.

PARTS TRAILER LOCATED ON SURFACE CAUGHT FIRE DURING IDLE SHIFT. TRAILER WAS DESTROYED. MINE IDLENO INJURIES. FIRE BELIEVED TO HAVE BEEN CAUSED BY A PROPANE HEATER.
FOUND AREA OF ROOF THAT HAD FALLEN 18' BY 18' BY 6' HIGH. THIS WAS IN AN OUT BY AREA IN THE INTAKE AIRWAY. THIS WAS A BAD AREA OF TOP WHEN IT WAS ADVANCED.
