Investigator narrative
While installing roof pins in front of LW shield, piece of rock fell, striking employee and knocking him down. Contusion to back of head, laceration to right hand and fracture to left ankle. The victim was flown by helicopter to the nearest trauma center, where he died on April 10, 2006.
