Due to high winds, Main Portal Facility lost power causing loss of elevator and slope. Power was re-established at 12:35 a.m. on 4/1/06 and elevator and slope were back in service.

On 3/30/06, an unintentional roof fall occurred on the 1st south off the 2nd west sub-panel at X-cut #39 in #6 entry on the #2 working section. This roof fall was located in an intersection and was supported with 10' point anchor roof bolts. The roof fall measured 25'long X 20'wide X 10'high and the roof fall will not be cleaned up.
25'long x 20'wide x 6'-14' fall in #4 entry near brk 78 on 3 east.
The fall is in the #8 & #7 entries of the return 2nd east entry. The fall was in #7 4 way intersection & extended across to #8 entry. The fall will not be cleaned up. In progress of setting timbers and cribbing approaches to fall.
A roof fall occured in the #5 entry on the 10 north belt 5 x cuts inby the head left of spad #36616. The fall measured 28'x20'x6.5'thick. Fall was bolted with 5' resin bolts. This area will be cleaned and bolted.
Sycamore Mine #2 had a roof fall on the mains section at cross-cut 4 to 5 in the last break. The fall measured 8' high x 15' long x 16' wide.
Batteries in brake car were down; it was discovered the reason being the charger was down also.
A rock fall occurred on 002 section main intake in 2 West Mains in the #6 entry at break 32. The rock fall measured approximately 40' wide x 20' long x 4' high. Rock fall was found while conducting weekly exam.
THE FALL WAS DISCOVERED ON THE PRE-SHIFT. THE FALL MEASURING 18'X 12'X 35' WAS LOCATED IN THE LAST OPEN CROSSCUT IN THE NO.6 ENTRY.
Roof Fall in an area of the mine that had been inactive since 5-31-05. The location is the intersection (old four way) of 2 east & 31 north (aprox. 300' x 50')It fell above the 8' Jenmar bolts . It shows shearing, calcite jointing and dampness. No personal injuries, no equipment damage, no issue for ventilation or escapeways. The area was baracaded.

A roof fall has occoured in the main east entries at spad 25 & 45 in No.5 entry. This roof fall is in the return. The dimensions of the fall are: Length 40' width 20' heigth 8'.
A cage tender was waiting in the cage for approximately 2 1/2 hours while electricians were troubleshooting the back-up generator during a planned power outage. The generator shut down at 9:15 PM and returned to service at 11:50 PM. The MSHA Helena field office was contacted immediately upon notification (9:35 PM), and again at 12:10 AM and 7:30 AM with follow up information.
The Hoist for the Man/Material Shaft for the Gateway Mine was unable to be operated due to a mechanical condition. The employees were called out of the mine and the mine idled until repairs were completed. The Hoist was inoperable from 10:30PM until 4:00AM 3/29/06. A solenoid switch was changed out and the hoist resumed operation.
MULTIPLE HILL SEAMS. ROCK WAS 25' X 16' X 5' THICK. FOUND ON PRE-SHIFT EXAMINATION. SECONDARY ESCAPEWAY WAS BLOCKED.
ROOF FALL AT 57 STOPPING ON TRACK ENTRY 2 BLOCKS OUTBY SPAD 45 + 3660. FALL WAS ABOUT 70'LONG X 16'WIDE & 20'HIGH.
An unplanned roof fall occured on MMU-003-0 section at the #7 intersection. Fall measured 30' in length, 16' in width and 4' - 9' in height. Inby spad #462 no accidents reported.
UNINTENTIONAL ROOF FALL OCCURRED IN RETURN AIR COURSE, 3RD NORTHWEST MAINS, #1 ENTRY, SPAD #1203. WET, WEATHERED ROOF.
A roof fall was discovered today and was approximately 150' long x 18' wide x 6' high. The fall will not be cleaned up.
A Fire Boss went down the elevator but was not able to open outside door without facing it. A trans'ducer was found to be deffective. The elevator was fully operational for someone to come out of the mine using this escape entry. At this time we are questioning the need for 7000-1.
Had a rock fall in intersection inby spad #2 breaks inby 1126.
Roof fall was found at 2:00 pm, 7000 ft. off section at 2-B Belt cross-cut between #3 & #4 entry outby spad #3270. Fall was 18 ft. wide, 45 ft. long, 2 to 4 ft. thick. This area was developed in April 2004.
An unintentional roof fall occurred at the New Era Mine at the #4 Entry on the 1 East Tailgate at the 3194E station mark. The fall was approximately 10' long, approximately 6' wide and of unknown thickness. The fall did not affect ventilation. The fall will not be cleaned up.
An area of dark shale with slicken sides fell pulling center bolts in last row of supports. Fall measured 15' long x 15' wide and from 3'to 5'high. Fall pinched miner behind hinge point.
A frictitional face ignition occured on the 7 B section MMU 081-0 in the #3 entrie.
A roof fall occured in the 1st north main in the #4 entry (supply road) at spad # 65+10 inby to spad #66+50. The fall was 120'long X 18 1/2' wide X 6'high.
UNINTENTIONAL ROOF FALL OCCURRED IN #3 ENTRY, SPAD #1150, 3RD NORTHWEST MAINS IN A NEUTRAL ENTRY. WET, WEATHERED ROOF.
An unintentional roof fall has occurred in the south East Main return, entry #7 x-cut 32+20. The cause of the roof fall was laminated and unconsolidated shale above the anchorage point. The area had 6' fully grouted rebar roof bolts installed for roof support.
Gas Fire on North side of Kiln 3 preheater tower. Gas was disabled immediately. The fire caused damage to cannons, lighting and air hoses. The connection fitting from the riser to the hard pipe failed, causing natural gas and oxygen to come into contact with ignition source and ignite. No injuries took place and MSHA was notified locally.
EE found an unitentional roof fall out by the feeder of #1 section. Spad# 7283. The fall does not impede travel and all sides of the fall have been dangered off and timbered. The fall measured approximately 60' long x 20' wide and was from 0' to 4' in thickness.
AN UNINTENTIONAL ROOF FALL OCCURED IN 2 N, XC5, ENTRIES 2 & 3. THIS WAS A CONTINUATION OF THE ROOF FALL REPORTED ON 3-20-06. THE PRICE FIELD OFFICE WAS NOTIFIED.
The third shift had got to the Section approximately 12:00AM and there was rock down in #3 Entry. Also, at the Feeder the Prep Foreman sent the scoop man for cribs and the area at Feeder was the first place they worked and #3 fell in by the time they got to it.
Had escape raise hoist's squirrel cage go through the limit switch on top and break off both limit switches brackets and shut the escape hoist down. We had to send a person up on a snow mobile to check it out. Hoist was down, until we could open the road up to the top of raise and get a boom truck up to the top raise and repair it.
The 1085 hoist was inoperable for approximately one hour as the result of a brake adjustment sensor failure on #1 side drum brakes. The sensor failed during normal muck hoisting operations causing the hoist to stop and apply the brakes. No injuries or damage resulted from this situation. On site personnel were notified.
On 3/20/06, an unintentional roof fall occurred at the #6 crosscut in #5 entry on the conveyor beltline. The area was initially supported with 10' point anchor roof bolts, the fall measured 25' x 18' x 12' high and the roof fall will be cleaned up.
A ingnition occured on the No 6 section at approx 5:05 am on monday morning 3/20/06. The miner #81 had cut approx 13 feet and had started cleaning up in the # 2 entry when the ingnition occured on the right rib. A methane test was made before mining started in the face 0.4%to0.6%. The lasted 4 to 5 minutes before extinguished.
3 breaks back behind face; 3 crosscut back. Open make #5 header. 30'long, 20' wide 41/2 "" thick. No affect on the air flow
Had and unintentional roof fall at break #131, parallel with #20 belt on track entry. Fall was 25' wide X 30' long X 10' thick.
An untintentional fall on the track entry at #131 break parallel with #20 belt fall was 25'x30'x10' high.
An original fall 60' x 20' x undetermined height occurred in #7 entry 30' outby spad 3244. The fall continued 150' outby the next day.
An unintentional roof fall occured at the New Era Mine at the 6 North Headgate @ 3070 N, Crosscut 45 between entry#2 & entry#3 (continuation). The fall was of unknown length, approx. 18' wide and of unknown thickness. The fall did not affect passage of men. The fall will not be cleaned up.
An unintentional roof fall occurred 1 break outby spad #2130. The fall measured 18' wide, 20' long and 6' thick.
A roof fall occured out by the tail piece during the night. The mine was idle at the time of the fall. The fall was found during the preshift examination prior to work on dayshift Sunday. The fall was located inby the intersection of 2+40 in #6 2nd west entry. The area was initially bolted with 6' fully grouted resin bolts. The cause was a hidden slip in the roof,
Found during the preshift examination prior to the start of 3rd shift.Mine was idle at the time of the fall. Fall located in #5 1st west entry at 22+20. Fall was 17'l, 18' w, 12' thick. The area was initially bolted with 6' resin bolts, trusses, had been added as secondary support. Cause was determined to be water & slip in the weakened roof.
A roof fall occurred one break outby the slope bottom near the belt shack.
AN UNINTENTIOINAL ROOF FALL DISCOVERED BY FIREBOSS IN PRIMARY ESCAPEWAY, INT. OF XC #5, #1 ENTRY, 1ST N. SUB-MAINS. 35' L x 20' W x 5'-6' H, DOME. TIMBERS, CRIBS, J-CHANNEL PLACED AROUND ROOF FALL. PRIMARY ESCAPEWAY RE-ROUTED, MARKED ON MAP & CREWS TRAINED ON CHANGE. THE FALL WAS IN AN AREA BOLTED W/5' BOLTS PRIOR TO CHANGE TO 6' BOLTS. FALL WAS REPORTED TO PRICE FIELD OFFICE.
The 2 Rt crew had just finished taking a cut in #9 Rm and move to #10, when the cut that had just been mined started to work. It fell in then the intersection just outby started to working, it also fell approximately 60' Long 7' height 18'wide.
The Miracle Run Elevator Doors failed to open when elevator reached the bottom landing. Shindler repairman determined that the bottom landing door opening arm was broken. The arm was repaired and the elevator returned to service.
Roof fall was discovered during weekly exam. Fall was 1 break inby spad #116, 2500' outby section. Size of fall was 60' X 15' X 8'. Fall did not disturb ventilation. Fall was dangered off from all approaches. No injuries were reported. Notified MSHA Asst. Dist. Manager at 10:15 AM about fall.
(1ST OF 2 ROOF FALLS THIS DATE.) An unintentional roof fall was discovered on C-Mains in #4 entry 1 block inby spad 13505. Height 8' to 9', Width 20', Length 50'. Support - 5' fully grouted resin with 10 cable bolts as supplemental support.
A roof fall outby on #3 belt line on the #1 entry, 1 break inby spad #369. The fall measured approximately 40' lenght, 20' width and 6' high.
(2ND OF 2 ROOF FALLS THIS DATE.) An unintentional roof fall has occured in B1A panel in #4 entry at spad 13175. It is in old works and does not affect passage or escapeways. Height - 7'; Width - 20'; Length - 40'. Support - 5' fully grouted resin bolts. Developed 12/3/04. No injuries or equipment damage.
Highwall collapsed on highwall miner as it prepared to start mining resulting in damage to front of machine.
A roof fall occurred in the 1 Main South, #6 entry, in the pillar line between crosscuts #6 and #7. The fall measured approx. 40' X 18' X 6'. The area was supported with 6' tension rebar bolts. An undetected slip was determined to be the cause of the fall.
Employee became stuck in elevator.
The elevator became stuck on the bottom landing and the doors would not open. It was repaired by replacement of electrical components and adjustments.
A roof fall occurred outby on #2 belt line on the #1 entry, spad #205. The fall measured approx. 60' length & 20' width.
AN UNINTENTIONAL ROOF FALL OCCURRED AT THE NEW ERA MINE AT THE 1725 EAST CROSSCUT BETWEEN #1 & #2 ENTRIES IN 2EAST HEADGATE, MMU 009/#11 CROSSCUT. THE FALL WAS APPROX. 30'LONG X 16'WIDE X 10'THICK. THE FALL DID NOT AFFECT PASSAGE OF MEN. THE FALL WILL NOT BE CLEANED UP.
A roof fall occurred in the 1st right intake in #3 entry at #5 crosscut sometime between 1:45 pm & 2:00 pm. The fall was reported to the field office around 2:10 pm. The fall measured approx. 19' wide on the outby end, 17'wide on the inby end & was approx. 22'long & approx. 7-9'high. 1st right was evacuated & a plan developed to clean & bolt fall area.
An unplanned roof fall occured in the intersection of the # 6 entry. 180 feet out by spad # 4154. At the #4154 end of the rail in track entry. Fall measures 18' wide 30' long and 12' to 14' thick.
A ROOF FALL WAS FOUND DURING THE WEEKLY AIR COURSE EXAMINATION AT 9+40 BETWEEN #6 & #7 WEST. THE FALL EXTENDED INTO THE INTERSECTION OF #7 WEST. THE FALL WAS 35'LONG X 16'WIDE X 15'HIGH. THE AREA WAS SUPPORTED WITH 6'FULLY GROUTED RESIN BOLTS. WATER WEAKNENING THE IMMEDIATE TOP WAS DETERMINED AS THE CAUSE. THE AREA WAS DANGERED OUT & TIMBERED.
At 10:30 A.M. on this date a rockfall was discovered in the intake at spad # 487. The fall measured 20' X 20' by 4-5' in thickness. The fall was dangered off with cribs and timbers set. Ventilation was not affected.
A roof fall occurred in the 1 Main East, #1 entry, intersection of crosscut #43. The fall measured approx. 18' X 18' X 7'. The area was supported with 6' tension rebar bolts. An undetected slip was determined to be the cause of the fall.
The safety switch on the elevator tripped while making trip to the surface causing the elevator to go into stop mode. One person was trapped on the elevator for approximately 50 minutes. The elevator was brought to the surface and removed from service immediately until repaired.
An employee was attempting to relight the preheat torch when the explosion occured, due to a build up of flammable gas. Initally we were told no 7000-1 was needed then field office called on 6/23/06 and requested one confirmed 6-27-06.
A roof fall was found in the #6 return break #9 intersection. The fall was found during a weekly examination. The fall is approx. 20' wide X 20' long X 3' thick.
An unplanned roof fall occurred on the Rt. return two crosscuts inby spad No. 2007 in the #7 entry where No. 3 north mains started. The fall measures 30' in length, 20' wide and 4' high.
THE OTIS ELEVATOR WAS OUT OF SERVICE AND WAS DISCOVERED TO HAVE HAD A BLOWN CONTROL FUSE DUE TO A LOOSE WIRE. OUT OF SERVICE FOR APPROX. 3 HOURS.
Had excessive amount of rain. The river went out of its banks and the pit area was flooded. There was no production on Monday 3-13 and Tuesday 3-14-06. The water started to recede and production started back up on Wednesday 3-15-06.
The faces in 28K North & South were shot at 6:30 AM. When the fire boss crew went undergroung to inspect the faces, they found gas discharging from several blast holes, and the gas was burning. After checking Co and methane levels. Supervison extinguished the flames. No reignation occured.
A ROOF FALL WAS DISCOVERED IN THE 2 SOUTH #2 MAINS, CROSSCUT 20, #3 ENTRY INTERSECTION. THE FALL MEASURES APPROX. 30'LONG X 18'WIDE X 7'HIGH. THE AREA IS SUPPORTED WITH 5'FULLY GROUTED RESIN BOLTS & 6 X 6 TIMBERS.
A roof fall measuring approximately 12'Wx14'Lx6'H was bolted with 6' bolts.
An unintentional roof fall occured in the Entry Intersection of the #6 Neutral Entry at spar # 4148. The fall measured 17' wide 7 to 9 foot high and 40' long. This fall was 900' out by faces in an untraveled Entry. Fall Breaked off and dangered off.
Large volume of water inflow from pilot hole above Powellton seam. Source of water unknown.
A roof fall occurred between 8:00am and 10:00am in an intersection of an outby area. 2 crosscuts inby station 2496 in #5 entry of 8 mains.
LARGE AMOUNT OF WATER COMING OUT OF GLORY HOLE (LOCATED OUTBY NORTH MAINS SECTION). CONTINUOUS FLOW. SOURCE UNKNOWN AT THIS TIME. STILL UNDER INVESTIGATION.
HOISTING INCIDENT : Cowling Portal Elevator - On Saturday, 03/11/2006 at 12:38 pm the Cowling Portal Elevator went down due to a door switch on the bottom being out of adjustment. The elevator was repaired in Saturday, 03/11/2006 at 01:06 pm, the door switch was readjusted.
A FALL OF GROUND OCCURED IN UE160 SOUTH DRIVE. A SECTION OF BACK CAME OUT APPROX. 20'WIDE X 30'LONG X 10-12'THICK. THE AREA WAS FLAGGED OFF & CLOSED THE HEADING TO FURTHER EXAMINE THE GROUND FALL.
An unplanned roof fall occurred on MMU 003-0 section in the face of #10 entry. The fall measured 12' x 5' thick, 6' wide. No accident reported. Fall was one break to the right of spad 443.
Skip opened up and spilled coal from the load cell to coal level. A rub rail had come loose.
ROOF FALL (7000-1-004-06) : LOCATION - Old Main West Left Return, #89 Crosscut, Entry #5. DIMENSIONS - H:6-7', W:23', L:22'. GEOLOGY - Soft, medium to dark gray shale with prominent roof jointing or slips in roof. ROOF SUPPORT : 6' Conventional bolts. The area was mined in November 1987. Prominent roof jointing in the roof were the apparent cause for the fall.
The #1 elevator was out of service for a period exceeding thirty (30) minutes due to malfunction of the COMP chain and damage to the Buffer switch. The COMP chain was adjusted and the Buffer switch was replaced and the elevator placed back into operation. No escapeways were affected buy the outage.
A rock fall occurred 1 break outby spad #326 in the #3 entry and extended through the crosscut over to the #2 entry in the 2 East Mains. The fall measured approximately 80' long x 20' wide x 1'-5' thick.
A roof fall was found in the intersection of No. 4 entry one break inby spad location 1850. The fall was approx. 20'x 20'x 4' high. There were no injuries and no equipment damage. Ventilation was not affected and travel was not impeded. It was above the anchorage zone.
A roof fall occured in the No.3 entry intersection and extending approx. 20 feet into the cross cut between no. 3 & 4 entries...Height 10 feet x length 38 feet x width 18 feet... working section 005-0 mmu... at Spad No. 9753
On 3/9/06 an unintentional roof fall occured at #18 crosscut, #7 return entry of the 4A submain. The area was initally supported with 10' point anchor roof bolts, the fall measured 40' x 20' x 10' high and the roof fall will not be cleaned up.
Unintentional roof fall was found at 11:00 AM during a weekly examination. Area fall was found was 1 x-cut inby spad 2706. Fall was 40' long by 20' wide and 6' to 8' high.
Brakes on the elevator slipped and allowed the elvevator to drift. The brake coil and shoes were replaced.
On 3/8/06 an unintentional roof fall occured on the MMU 002 working section. This roof fall occu- red in the #1 entry between #24 & #25 crosscuts, this area was supported with 6' fully grouted roof bolts. The roof fall measured 50'x 18' x 12' H. and will not be cleaned up.
The hoist engineer was making checks and failed to set the rollback back to the proper setting, causing the brakes to lock.
A rooffall measuring 20'Wx40'Lx8'H 25' out by spad no. 2368. Area was bolted with 6' bolts.
A roofall measuring 20'Wx25'Lx6'H occured in the No. 5 entry off No. 8 belt one break in by spad nO. 2345. Area was bolted with 6' bolts.
ROOF FALL (7000-1-003-06) : LOCATION - 1 West #4 Roadway, #7 Crosscut. DIMENSIONS - H:6-7', W:20', L:20'. GEOLOGY - Soft, medium to dark gray shale with prominent roof jointing or slips in roof. ROOF SUPPORT - 6' Instal bolts. The area was mined in November 1993. Prominent roof jointing in the roof were the apparent cause for the fall.
A roof fall occurred on the 005 section between #41 & #42 crosscuts in the #1 entry. The fall was approximately 14' x 12' x 6' high. The area had been supported with 5' high tech bolts and supplemented with 8' super bolts.
The shearer was cutting from the head to tail gate. The shearer was cutting to the bottom rock. At shield #109 the shearer operator saw flame on the headgate side of the shearer. The flames were extinguished immediately.
A city wide interruption by public utility company SWEPCO resulted in the mine being evacuated using the mine's secondary power source (diesel). The evacuation began at 11:25am & was completed at 12:25pm. The outage was projected to last for several hours so miners were sent home. SWEPCO restored power at 12:48 pm and mining operations were resumed.
A non-injury bottom fall occurred. The bottom or floor of the #5 entry sagged down. The local MSHA office was informed. A seal over the bottom ""floor"" was erected.
UNINTENTIONAL ROOF FALL 3 NORTH SUBMAIN #4 ENTRY X-CUT 9 TO 10. 25' LONG X 18' WIDE X 6-8' HIGH. APPROX. DATE OF MINING WAS 10/1/02. BOLTED WITH 6' TR BOLTS. PROPPED OFF - WILL NOT BE CLEANED UP.
HOISTING INCIDENT : Cowling Portal Elevator - On Tuesday, 03/07/2006 at 06:45 am the Cowling Portal Elevator went down due to a door lock switch on the bottom outside door being inoperative. The elevator was repaired on Tuesday, 03/07/2006 at 09:00 am, the door lock switch was replaced.
During a routine inspection of the escape shaft hoist, it failed to operate properly. Maintenance changed out an electrical timer contact and hoist was tested and operated properly. Repairs too approx. 35 minutes to complete.
A roof fall occurred on the 2L4W panel in entry #2 at crosscut 24. The cause of the fall was weak laminated shale. The fall was located in the return aircourse of #3 unit.
A FIRE OCCURRED ON THE 10TH FLOOR OF THE COAL PREP PLANT. THE DURATION OF THE FIRE EXCEEDED 30 MINS. CONES WERE BURNED OFF THE CYCLONES IN THE MORNING. AT APPROS 2:30PM FLAMES WERE OBSERVED. THE FIRE WAS EXTINGUISHED AN INVESTIGATION WAS PERFORMED AND THE PLANT WAS FOUND SAFE TO RESUME NORMAL OPERATIONS.
A rock fall was discovered during the weekly examination of the return area. The fall was 3500 feet from the working section. The fall was at SS 550 approaching the #3 seal. The area will be dangered off and a walkway to the #3 seal will be timbered off for safe access to the #3 seal.
A rock outburst occured in the 43-190 RS. Approx- amately 30' below 4000 level. No injuries as a result. The three miners were temporarily withdrawn from the area until an investigation could take place the next day. The supervisor of the MSHA office in Bosie was contacted at approx. 6:37 pm on 3/7/06.
An unintenional fall of roof was discovered 1 break inby spad #3911 in the #1 entry of 002 section along south mains. The fall was approx. 20' wide, 120' long & 6' - 8' high.
A possible unplanned ignition of explosives was reported due to smoke observed. Called in immediately per regulations. Futher investigation revealed smoke observed however no evidence of an explosion was found(noise,disturbed material). Our opinion: the shovel uncovered unignited ammonium nitrate that was burning in a hole that could not be observed until it was uncovered.
The roof fall occurred in the 1st south cast butt room #40 righside in second x cut from # 5 entry fall was approximately 30' long 20' wide and 4' to 5' high area was bolted with 4 & 3 bolts there was a gray slick & sided clay stone streak at top of roof fall.
THE OTIS ELEVATOR WAS OUT OF SERVICE DUE TO A BLOWN CONTROL FUSE W/1 PERSON ABOARD FOR 2 HR.
A bird strike at the power sub-station owned by Pacific Corp caused a power blackout that shut down the mine hoists for approx 4 hours. There was no damage to any equipment.
An ignition occurred at #59-#60 shield, F-12 Longwall Face. The ignition lasted approximately 15 to 20 seconds and was extinguished by water hoses from the shields. There was no damage to equipment and there were no injuries.
Fall on Main South @ 34+60 in return. Length 30', width 18.6, height 10. 5 foot grouted bolts. No property was damaged. Will not clean up. Developed 9/15/03. Just inby #3 within pillars.

Roof caved in old set of rooms adjacent to #1 Mains at X-Cut 31. Crew evacuated from mine. Additional caves and section heave occurred. Total extent of cave is unknown.
Unintentional roof fall 40'(L) x 30'(W) x 6'(H) in #3 Entry @ spad # 10613.
On 3/3/06, section C-9, an unintentional roof fall occurred on the 1st South off the 5th West Sub-Panel at X-cut #26 in #7 entry on the #2 working section. This roof fall was located in an intersection and was supported with 10'point anchor roof bolts. The fall measured 70'long x 20'wide x 10'high and the roof fall will not be cleaned up.

ROOF FALL (7000-1-002-06) : Roof Fall was found at the following LOCATION-Outby Main East Seals, #4 Seal. DIMENSIONS-H:6-8', W:19', L: Approximately 30'. GEOLOGY-Soft, medium to dark gray shale with prominent roof jointing or slips in roof. ROOF SUPPORT : 6' Conventional bolts.
THE 1085 HOIST WAS INOPERABLE FOR APPROX. 2 HOURS DUE TO BRAKE ADJUSTMENT PROBLEMS ON #2 SIDE. THIS SITUATION OCCURRED IMMEDIATELY FOLLOWING A ROUTINE HOIST ROPE REPLACEMENT. A MAINTENANCE DEPT. INSPECTION REVEALED BRAKE SENSOR ADJUSTMENTS WERE REQUIRED TO REMEDY THE SITUATION. NO INJURY OR EQUIPMENT DAMAGE OCCURRED AS A RESULT.
The fall area is at the end of a pre-existing fall and the intersection had supplementary roof support in it from previous conditons. Fall height - 5' plus Width - 19'; Length - 20' Bolts - 5' fully grouted resin and 10' cable bolts
Fall #1 Unit (x cut 3)entry out by unit water out of top before & after fall.
The intersection fell at 10 + 55 in #3 enty in the # 4 unit intake the fall was approx. 25' long 20' wide and 6' high. This area was mined 8/6/05. The unit is on retreat
The intersection at 8+00 fell approx. 20' x 25' & 6' in height. This area was supported with 6' fully grouted glue bolts. Developed 8/1/05.
A lift was bein mined from a pillar on the 2-left off 2 south panel when a piece of rock fell on the head of the mining machine and fouled the machine. A retriever was hooked to the machine and the machine was free in approximately 10 min. A rock fell inby the left row of roof bolts and no belts were damaged. This report is submitted in protest to satisfy a citation.
A roof fall occured on the 3 north roadway at #40 crosscut. The fall was approx. 20'x20'x6' thick and located in the #4 entry. 5' high tech bolts had been installed and supplemented with 8' super bolts.
50'L x 19'W x 6'H roof fall on #1 section in #3 entry near spad 2195.
A rock fall was foung in the #1 entry of SW mains, 1 break in by spad #2033 in the crosscut between #1 & #2 the fall was about 20' long 10' wide & ranging from 18' to travel way or air way & it will be cribed off. The mine is idle with 2 emloyees.
AN UNINTENTIONAL ROOF FALL WAS DISCOVERED IN THE #7 ENTRY ON #2 BELTLINE. IT MEASURED 20' X 30' X 4'. THIS FALL WAS TIMBERED AND DANGERED OFF AND WILL NOT BE CLEANED UP.
While installing roof pins in front of LW shield, piece of rock fell, striking employee and knocking him down. Contusion to back of head, laceration to right hand and fracture to left ankle. The victim was flown by helicopter to the nearest trauma center, where he died on April 10, 2006.
Injured worker was helping foreman with crusher rebuild. Foreman tapped adjusting rod with sledge hammer. Apron fell, catching IW's left index finger.
EE was on BB1 conveyor belt looking at blockage when a second EE turned on the belt causing first EE's body to be projected towards the chute. His left leg went under the belt when left leg wa s caught by the head pulley severing the left leg, freeing the employee.
EE WAS USING SCOOP TO MOVE A HEAD DRIVE, PUSHING AGAINST 2 HEADER BOARDS. THE 2 BOARDS FLIPPED OUT AND SMASHED HIS FINGER BETWEEN THE BOARDS AND THE FRAME OF THE HEAD DRIVE. (AMPUTATION: 1/2 MIDDLE FINGER ON LEFT HAND.)
Employee was using the overhead crane to move a 6""x7' long steel shaft. The shaft had a groove cut in each end for a keeper ring and also had two steel bushing on the shaft. When he lifted the shaft, a bushing on one end slid off of the shaft striking his finger cutting off 1/2'' of the tip of left middle finger.
Employee was in the process of helping another employee install a rock bolt when a rock fell out of the face and struck him on the right foot. The impact resulted in fractures of all five toes on his right foot.
Employee was attempting to remove a drinking water container weighing approximately 41.5lbs from the storage bin, at approximately waist to chest high. He pulled the container from bin, and in a fluid motion moved container up to and rested it on his shoulder for transporting. After he did this motion he felt a sharp pain in his lower abdomen.
Located on 8-A section during a longwall move EE's left thumb was amputated at the distal joint while attempting to block up a shield prior to transport. The shield tip slid off of the mast of the scoop bucket causing the shield to drop down approx. 3"". EE's thumb was caught between shield canopy and the crib he was holding (upright, vertical).
The employee had placed his right hand on the bed of a hydro-crane, while pulling a sling out from under a 4"" stainless steel pipe with his left hand. When he pulled the sling loose, a piece of pipe that was stacked on the lower one came loose, falling about 4"" onto the employee's right hand. The tip of the employee's right ring finger was smashed with minor bone loss.
The employee was lifting a sandpiper pump to move it and felt a sharp pain in his right groin area. This was diagnosed as an inguinal hernia (right).
Employee was driving an empty haul truck & hit a bump in the road, which bounced his hard hat off. He reached for the hard hat & his arm got pinched between the truck & the rib wall. The injuries to his arm were a severe avulsion of the forearm & fracture above the wrist. Seven days after the incident his hand had to be amputated above the wrist.
EE was struck on the hand by rock falling from the roof that fell in between shields no. 2 and no. 3.
9H section, while hanging ventilation tube with slider, he was supporting the tubes with his right arm when the other end fell and he had all the weight on his right arm.
Miner was futting a piece of pipe with a torch and the pipe broke loose burining his arm. Over the weekend the burn wound became infected resulting in fever and general malaise.
EE was using the shop hoist lifting a getman tractor. He was blocking cribs under the tractor when the chain slipped causing tractor to smash EE's fingers between tractor and crib blocks.
Employee operating rock truck, backed the 785 under the 992 loader, the loader operator put a load containing a large rock in the rock truck, jarring the operator in the rock truck.....as a result of the jarring employee alleges head, neck, ear pain.
EE was helping the other employees replace a shaker screen. This particular screen weighed app. 230 lbs. (48""long x 72"" wide) Upon lifting the screen, EE grabbed his lower back, but didn't say anything. Later, at app. 11:40 a.m., he went to the plant manager and told him that he thought he had pulled his back.
While riding in company vehicle from parking lot to work area, particles flew into employee's left eye.
EMPLOYEE TRAMMING SCOOP AROUND CORNER. TOP OF CANOPY HIT RIB, KNOCKING RIB LOOSE WHICH FELL IN ON EE IN THE DECK OF THE SCOOP. BRUISED UPPER BACK & RT. SHOULDER.
Employee was loading coal when the loader he was operating caught on fire. The employee jumped from the loader and when he landed, broke both legs. Employee jumped approximately 8 feet to the ground.
EE was tightening an anchor bolt on a counterweight guide beam. The crescent wrench he was using slipped off of the anchor bolt, causing EE to hit an adjacent anchor with his right fifth digit finger. This resulted in a bruise to the finger and EE was given 3 days off by his attending physician, therefore resulting in a lost time accident.
Employee was carrying a fan to a job site when he stepped into soft mud causing a strain to his right knee. Employee continued on regular duty with restrictions of no prolonged standing or stairs. Continued discomfort returned employee to doctor and surgery was scheduled for 5/4/06, when employee started missing work.
Employee strained back trying to get heart attack victim out of machine.
While building a pallet of stone, a stone slid off the pile and smashed employee's left thumb nail. Initially it was a bruised nail, after a follow up exam, a fracture was found to the left thumb. More follow up exams to follow.
As a mechanic was lowering a D8 dozer to the floor, he caught his hand in a pinch point between the boggie roller and the track frame as he attempted to guide the load. He sustained multiple lacerations of the right hand, but no broken bones. His wounds required stitches.
EE was walking to the office to get on duty when he stepped on ice, slipped and fell down on concrete steps hitting his lower left back against the concrete steps.
EE was hitting a chisel with a sledgehammer trying to remove a broken bit from the Miner Ripper Head. The chisel splintered and a piece of metal went into EE's right hand between the fore finger & the index finger.
Operator just finished a cut in the travelway and was attempting to back into the beltline. He was on the left side of system when he got caught between the front bridge and the corner of the ribline intersection of the X-cut and travelway.
Employee was unplugging the discharge pipe in the slope with a 2"" pipe. When the plug came loose, the pipe he was using to dislodge the plug came out and struck him on the knee.
EE hit rotation with bent steel in chuck. It came around and struck EE on his left wrist and left side of head.
Employee was operating a Mucker, trying to position an empty hoist bucket. The hoist bucket raised suddenly and struck the employee.
EMPLOYEE WAS LIFTING CRIB BLOCK FROM TOP OF FEEDER AND TURNED TO THROW IT WHEN HE FELT A SHARP PAIN IN THE CENTER OF HIS BACK.
EE was turning over crusher mantel with a chain and come-along when mantel went over center chain became slack then taut and struck EE left wrist.
WALKING AROUND AREA WHERE BULLDOZING, STEPPED ON BROKEN BLOCK & SLIPPED IN THE MUD CAUSING INJURY TO HIS ANKLE.
Employee was pulling belt with chain hoist, belt rail pulled out striking his face.
Took break from loading holes to go and relieve himself. Started to go over the hill from bench where work was being done, slipped on rock & went down over hill landing on a rock then rolling on down to catch bench.
While replacing rub rail on the continuous miner the victum lost his grip and dropped the rail on his foot.
EMPLOYEE WAS DRIVING A DUMP TRUCK & HE FELT PAIN IN HIS LOWER BACK. THE EE WAS WEARING A BACK BELT. HE DID HAVE HIS SEAT BELT ON.
Employee is a Roof Bolter and had just finished bolting a place. He backed the bolter up out of the way and him and the other bolter started hanging their cable up to the roof. The cable was laying in a small mud hole and when the injured picked up on the cable he recieved an electrical shock.
Injured was cleaning up rock when he found undetonated blasting cap, which detonated in his hand.
Individual was tramming scoop outby, hit a pothole, jarred his back.
Employee was working around Auger by highwall when a football sized rock fell from the highwall and struck the Auger, ricochetting off the Auger and striking the employee in the shoulder area.
Employee was at the plant installing a shaft. Felt something in his back ""twitch"". Employee did not mention the injury until 04/06/2006 to report the injury. Employee said his back has been hurting him since the injury. The injury has just recently become reportable due to appointment with a neurosurgeon resulting in loss of 1 day and Rx medication.
Employee was helping dismantle a D9 dozer when the left push arm fell from the dozer strking his right leg and foot.
EE was climbing out of scoop, when he let the scoop roll forward. He got his foot hung up and it twisted him around.
EE was operating the miner in the #1 Entry in the E-Tailgate Section, when a piece of rock fell out from between the straps, striking EE on the right hand causing a laceration requiring 11 sutures.
Injured employee was in the process of dumping a load of coal off of his tractor/trailer when the trailer turned over, which in turn turned the tractor over. Injured employee's neck was lacerated during turn over. What caused the laceration was undetermined.
EE was replacing a valve assembly on station 9. lost his footing landing on his right hip against the inside wall of the classifier. Upon follow up discussion with EE it was learned the accident occured when he was getting into the classifier tank.
While walking to job station, EE slipped on rock, twisting his right knee.
EE WAS GOING TO PUT SOME WIRE FOR THE POWER MOVE. THE EE STATED HE WALKED THROUGH THE AIR LOCK CURTAIN ON THE SUPPLY ROAD. WHEN HE STEPPED INTO A SMALL HOLE ON THE OTHER SIDE OF THE CURTAIN CAUSING HIM TO TRIP/STUMBLE. HE WAS TRYING TO CATCH HIMSELF & FELL TO THE MINE FLOOR.
EE was helping make a belt splice when he was struck on the right index finger with hammer.
EE went into the Granusol Mill. The operator was emptying out a baghouse. He had the side door open for visual aid. EE saw a clump of ore in- side the door of the baghouse. He stuck his hand inside to remove the clump. A airlock was on and turning. EE stuck his hand in the airlock.
While decending #34 conveyor belt walkway, employee slipped on debris/material builtup on walkway. Later, EE decided to have the knee checked due to swelling. EE visited and received an MRI on 4/17/06. As a result surgery was scheduled and performed on 4/20/06. EE is currently recouperating at home after a surgery to repair a tear.
Pulling on water hose, hose caught on block, hurting left shoulder. Had surgery on 6/14/06
EE was pulling a backhoe counter weight on a roll-back truck, when the chain broke causing the tow cable to strike the EE in the mouth.
Lifting guard rail post and strained right inner thigh.
AS EE WAS COMING DOWN THE STAIR FROM THE CABLE REEL ON THE CABLE TRUCK EE STEPPED WRONG AND TWISTED LEFT ANKLE. GIVEN ICE PACKS, NEXT DAY SEEN BY HIS DR AND TAKEN OFF WORK UNTIL RETURN FROM DAYS OFF 4/6. 4/13 NOTIFIED SUPERVISOR EE RET'D DR 4/5 DIAGNOSED POSSIBLE FRACTURE, TAKEN OFF WORK-CURRENTLY ONGOING
Individual was backing up a tow seater mantrip, rear tire hit a pothole in pavement and steering knob went around and hit the back of his hand.
Repairmen were changing out a crusher. The hand rail on the work platform had been removed so the the crane could lift out the crusher. EE stepped off the platform while the temporary rail was being prepared. He was hurt while trying to break his fall.
Injured was setting posts in muddy conditions when post fell and hand got caught between posts.
EMPLOYEE WAS DRIVING THE MACK TRUCK UNDER BINS THAT ARE SUSPENDED WITH I-BEAMS. HE GOT CLOSE TO THE BEAMS & THE LUG NUTS ON THE TIRE CAUGHT THE BEAM. THE STEERING WHEEL WAS JERKED OUT OF HIS HANDS INJURING HIS RT. THUMB.
Employee was assisting a fellow coworker that began to have a seizure by supporting him from behind. Victim of seizure began to fall; employee did his best to lessen the impact and injured his left wrist when both landed. Doctor issued employee brace and scheduled physical therapy sessions to lessen soreness.
EMPLOYEE WAS PUTTING IN A SPLICE AND THE KNIFE SLIPPED, CUTTING HIS RIGHT FOREARM. HE WENT TO THE HOSPITAL AND THEY CLEANED IT, PUT 2 STAPLES IN IT, AND ADVISED TO TAKE 1 DAY OFF.
HE WAS PUTTING UP PIN IN THIRD ROLL, A PIECE OF RIB POPPED OFF HITTING HIM IN THE BACK.
Employee was cleaning up floor pit with a loader. He hit a rock which stopped the loader - seat belt came lose and the employee hit his head on the windshield and his stomach hit the steering wheel causing some bruising.
Foundry driver (Truck3S) was crossing over back of his truck, tripped over his hoses and slipped off truck. Called ambulance, result was a dislocated shoulder.

While operating shuttle car, employee alleges the brakes locked up causing him to hit his head on the canopy.
Employee said a miner cable fell off the hook and he tried to catch it and it hyper-extended his right thumb.
EE was loading the bottom of a heading for shot from the ground. Other employees were in a basket in the air to the left of the employee. A large piece of shale/stone broke away above the EE and fell to the ground, landing on the employee knoc- king him to the ground and fracturing his ankle.

Conveyor slipped off of sawhorses and fell on left leg resulting in a fracture.

Water truck was traveling down E ramp when lost brakes operator tried to berm it on L side of the road to stop it. When Truck struck L berm ir swerved to the R and came in contact with berm on R side of road and turned over on driver side. EE had injured knee and some abrasions EE went to ER Dr gave him time off.EE was terminated
Walking through center walkway of ARO Twin Boom Walk Through Roof Bolter after retrieving bundle of roof plates from rear of machine. He stepped on roof bolts lying in the walkway, causing him to stumble, slip & fall. Coworker had placed 4 roof bolts on walkway. Coworker stated that EE had hydraulic oil on his shoes because they had repaired oil leak prior to starting bolting.
Employee stated that he bumped his head against mine top while operating scoop.
EMPLOYEE COMPLAINED OF PAIN RADIATING FROM HIS GROIN DOWN HIS LEG AFTER OPERATING A SCRAPER.
EE was removing the bolt holding the torque-arm (speed reducer) to install a guard bracket. The weight of the 3 hp motor & motor mount caused the whole drive to rotate & the motor mount hit employee. When pulling back, he hit his head on the end of the conveyor frame. Cut on mouth requiring 16 stitches, broken tooth & 2 chipped teeth. Cut behind right ear requiring 3 stitches.
On 3/25/06, EE told his supervisor that his back was aching. On 3/27/06, EE wanted to see a Dr. He stated that his back had been hurting for a couple of weeks and believed pain was caused from a previous back injury several years ago. The Dr. asked EE what he had done and EE said his normal job duties, and on Saturday he had removed a pulley from a motor shaft.
Employee was on his knees lifting and twisting large timbers as he was timbering the aircourse. As he lifted and turned he felt a pain in his low back and right leg. He continued working and the pain got worse. An MRI showed a bone fragment against a nerve. He started missing work on 7-24-06 and had surgery on 7-31-06.
employee stated he stepped off of loader onto a pile of dust resulting in neck pain...he also stated he injured his neck approximately (3) three years ago, and his personel doctor diagnosed a bulged disc at that time...final diagnosis and outcome unknown at this time...
AN EMPLOYEE THREW A USED 4 1/2"" GRINDING DISK OFF AN ELEVATED PLATFORM & THE DISK BOUNCED OFF A WELDING TABLE & STRUCK ANOTHER EMPLOYEE IN THE RIGHT EYE. EE WENT TO THE E.R. & WAS INFORMED HE HAD A SCRATCH ON HIS EYEBALL, APPROX. 3/4"" X 1/16"" & THAT HE HAD A PARALYZED PUPIL.
EE stepped off of D11 carry dozer backwards and landed on the rail of service truck and hurt his back.
THE EMPLOYEE WAS UNLOADING ROOF BOLT STRAPS & BOLTS FROM SCOOP BUCKET. DURING THIS HANDLING OF MATERIAL THE EMPLOYEE SMASHED HIS RIGHT LITTLE FINGER BETWEEN THE SIDE OF THE SCOOP BUCKET AND THE HEAD OF A ROOF BOLT. (FRACTURE & LACERATION - 12 STITCHES.)
EE was riding in the passenger seatof #1 Getman tractor. Operator of tractor hit a rock in roadway it caused tractor to bounce. EE bounced in his seat of tractor and struck his left elbow and forearm on the frame work of tractor. His elbow swelled up & was sore. Finished shift. Came in next day at 11:00 he stated arm hurt. Went to Dr. removed him from work 2 days.
EMPLOYEE WAS LIFTING A PIECE OF BELT STRUCTURE & FELT PAIN IN GROIN. HE WENT TO THE DR. ON 3/28/06 & RETURNED TO WORK ON 3/29/06. EE WORKED UNTIL 4/3/06 UNTIL SURGERY, HE HAD OUT PATIENT SURGERY.
Employee stated that he bumped his left leg below the knee on 3/27/06 on a piece of belt structure and it made a blue place. He said that he did not think it was anything but it got worse over time. When he came outside after working the third shift on 4/10/06 he showed his leg to the mine superintendent and told him what had happened. He told him to go to the doctor.
Injured was riding mantrip, when a piece of rock fell, striking injured in back of head.
DID NOT START LOSING TIME UNTIL 4/7/06. Slipped and bumped left knee on take-up frame.
Employee stepped in a hole and twisted his back. He kept working until 4/7/06 when his disability began.
EMPLOYEE WAS LIFTING ONE END OF H BEAM. HIS FOOTING SLIPPED WHICH PULLED HIM TO THE GROUND. LOW BACK BEGAN TO HURT.
Employee was climbing on the ladder to get into his rock truck when he felt a burn in his stomach.
Employee was lifting a barrel of grease onto the service truck and felt a ""pop"" and pain in his right shoulder.
Employee was attempting to hang a trolley on an I beam. The trolley slipped and the employee attempted to catch the trolley as it fell. The employee went to the doctor and has diagnosed as having a rupture that will require surgery.
EMPLOYEE STATED HE WAS DRIVING GOLF CART BETWEEN COAL HAULER & RIB. A COAL HAULER TIRE WAS LEANING AGAINST THE RIB WHEN EE DROVE THE CART BY RIB. THE CART STRUCK THE TIRE LEANING AGAINST RIB, AS IT WAS FALLING OVER EE REACHED OUT TO CATCH THE TIRE PULLING HIS LEFT ARM BACKWARDS.
EMPLOYEE WAS WORKING AT THE TRACK CHUTE, COUPLING MOTOR TO RAIL TRUCKS. EE PLACED HIS RIGHT HAND ON BUMPER CAR & THE END GATE OF THE RAIL CAR DROPPED DOWN ONTO HIS RIGHT HAND.
HE WAS GETTING READY TO INSATLL ROLF BOLT INTO FACE AREA WHEN A PIECE OF RIB FELL OUT HITTING THE CANOPY OF THE BOLTER, AND LANDED AGAINST THE BOLTER OPERATOR.
The adapter from the boom slid off bucket and hit into the EE's side. Bruised his ribs.
EE stated that while putting #184 portal bus on charge he was pulling battery plug out of the receptacle when it released suddenly causing his right hand to hit the pole dog assembly. This resulted in a non displaced fracture of the 2nd metacarpal of his right hand. He was wearing metacarpal gloves.
While working on equipment in shop, EE was removing a relief from hydrolic cylinder with a wrench and his back popped. It only felt like a sore muscle until 3/27/2006.
EE was replacing a belt roller when the belt shifted and caused his finger to be caught between the hanger and the structure.
THE MAINTENANCE WORKER WAS ON SOME STAIRS WHEN HE SLIPPED ON SOME HOT PIPES THAT THEY WERE WELDING TOGETHER. APPARENTLY HE FELL & THAT'S HOW HE GOT HIS INJURIES.
Employee was operating a dozer when he got a limb caught between the blade and the nose piece. The operator was trying to remove the limb when the limb slipped catching his hand between the limb and a guard on the back of the blade. The guard made a laceration on the employees hand.
While reaching for nails on the back of the bolter a piece of rock fell from the mine roof striking employee across the back. This accident did not become lost time until 03/31/2006.
The employee was helping to load explosives in a rock in the face conveyor when a piece of rock 2'thick 3' high and 10' long flushed off of the face and struck the employee.
Employee was hooking a cable onto the scoop bucket as the scoop started to tram the scoop canopy caught on a roof bolt causing it to move to the right pinning him against the rib. He was transported to the hospital and released to return to work. On 3/30/06 He notified Safety that he had returned to the doctor an was told to remain off work.
The EE was loading timbers onto the roof of a mantrip. The timber was dropped, trapping the tip of the EE's finger between the timber and the mantrip roof. X-rays showed a slight fracture.
The EE was struck on the left side of his face when a longwall shield hydralic line ruptured.
While scrapping the pan he bumped his finger.
Employee struck left knee while loading truck with 55 gallon drums.
EE was walking miner cable and hose into the cross cut to hang and pulled his calf muscle.
Employee cut a tree. When the tree fell it kicked up a stick that came back and hit him in the face shield. The face shield cracked and hit him in the eye.
EMPLOYEE CHARGED & DISCHARGED 5 FURNACES DURING THE DAY WEARING GOGGLES. HIS EYES FELT DRY DURING THE DAY. ON HIS DRIVE HOME THE SUNLIGHT WAS IN HIS EYES & HIS EYES BECAME SEVERELY IRRITATED & STUNG. HE WAS TREATED AT AN E.R. & THE FOLLOWING DAY AT OUR OCCUPATIONAL DR.
EE was walking across an elevated steel mesh walkway to a control house, when he slipped and fell on the walkway. It was snowing and sleeting at the time. The mid-rail and hand-rail prevented the EE from falling to the ground.
EMPLOYEE WAS USING TORCH TO CUT LINK OUT OF CONVEYOR ON FEEDER WHERE THE HOSE CONNECTS TO THE HANDLE. THE HOSE RUPTURED & BURNT HIS LEFT ARM. EE WENT TO THE HOSPITAL & WAS TREATED & RELEASED. EE WORKED HIS NEXT SHIFTS ON FRIDAY & SUNDAY & THEN WAS OFF DUE TO THE INJURY, HE PLANS TO RETURN TO WORK ON 4/3/06.
EE WAS OPERATING A 785 ROCK TRUCK, WHEN A DOZER BACKED INTO THE ROCK TRUCK. EE STATED THAT THIS JARRED THE TRUCK AND CAUSED HIS BACK TO HURT.
Contracted trucking company employee slipped off of the fuel tank step while climbing out of his truck.
Employee was operating 830 Rock Truck, the truck was being filled with overburden when a rock dropped in the bed and jarred the employee resulting back pain.
A piece of draw rock fell on bolter arm. EE reached with his hand to pull piece of rock off bolter arm and another piece of draw rock fell on up lever of bolter causing bolter arm to raise and pinch EE's hand against roof.
WALKING PAST COAL HILL #2 TAIL. STONE CAME OFF OF BELT & STRUCK RIGHT SIDE OF FACE.
Employee was operating a dozer, while backing up employee hit a rock causing the dozer to slide sideways, resulting in the employee injuring his back.
There was loose material on the floor and it had snowed that morning. The employee slipped on the material and fell on his back and sprained his back.
EMPLOYEE HAD HIS FOOT IN BETWEEN THE GUARDRAIL AND THE JAW CRUSHER WHEN A ROCK SHOT UP AND HIT HIS FOOT - FRACTURED HIS TOE.
EE was climbing out of the loader. He stepped on the platform, spun around to grab ahold of handle, and missed the next step. In his hurry he fell and strained his left knee.
EE stated he was desending on walkway from unit 92 refuse belt, stepping to ground level. EE missed last step. EE stated that the lights were to his back he misjuged the last step and sprained or twisted his left knee.
Member was conducting a a preshift of the pumps. Operation is in rehab of the slope. EE stated in his normal activity he experienced unbearable pain in his lower left back. He states he had no incident or accident. EE stopped work 4/4/06 for further medical test. He has been under doctor prescribed medication for the past year with back pain.
EE WAS STANDING ON BOLTER BOOM IN DOWN POSITION, INSERTING A TUBE OF GLUE INTO A DRILLED HOLE. EE SLIPPED OFF BOLTER BOOM STRIKING HIS LEFT ELBOW AGAINST THE SIDE OF THE BOLTER.
While unplugging a feeder the employee struck his elbow against the feeder sustaining an unknown injury to the right elbow.
While moving belt rails at the section tailpiece, EE felt pain in stomach area when he turned to throw rail against rib.
EE slipped and caught himself w/extended left arm, sustaining FX to left wrist scaphoid. Off work beginning 3/27/06.
EE was driving a bobcat and hit a bump, which shook him around causing him to pull a muscle in his neck.
While trouble shooting a power center, EE touched ice cube relay with screwdriver.
EE reports feeling sharp pain on neck when positioned on side, vacuuming on 31BC01 conveyor. On 3/24, employee reported small welt on back of neck, sent for med eval. On 3/27, physician placed on TTD. First lost workday 3/25. Probable insect bite or sting.
Miner was injured by pry bar that was used as a guide for pulling a rope by a front end loader installing new feed belt on screen conveyor. Miner received contusion to left shoulder.
Employee fell from scaffolding during dismantling process.
Employee hurt right shoulder while adjusting tail pulley on feeder belt. He reported injury to plant manager but did not want to see a doctor at this time. On April 4, 2006 he requested to see a doctor and later was referred to an orthopedic doctor and is waiting to be scheduled for minor corrective surgery.
Was working in crusher box when a sheet of steel hit his left knee.
EE was extracting a large block of sandstone from an excavation site using the bucket of his excavator when the bucket slipped from the back of the block causing the excavator to abruptly move backward and forward resulting in EE sustaining a back injury.
Employee was assisting co-workers in loading a belt take-up into a scoop bucket by retracting the dozer blade which was hooked to the take-up using a chain. Injured employee had his right hand on the scoop bucket frame and used his left hand to push on the chain. His right hand got caught between the dozer blade and the frame of the scoop bucket.
Applying belt dressing to head pulley of pug mill conveyor, employee inserted arm behind guard and sleeve was caught by conveyor. Employee suffered arm dislocation, fracture, and abrasions before he was able to hit conveyor stop button.
THERE WAS AN UNGUARDED OPENING NEAR WHERE THE BASE OF A LADDER WAS SET UP. THE INJURED EMPLOYEE CLIMBED THE LADDER BEFORE HIS CO-WORKER CAME BACK TO THE WORK LOCATION FROM LUNCH. THEY HAD BEEN HOLDING THE LADDER FOR EACH OTHER. AS HE CLIMBED THE LADDER, THE BOTTOM SLIPPED & FELL THROUGH THE HOLE & HE FELL THROUGH THE HOLE TO A PLATFORM 12' BELOW.
EE is a Shuttle Car Operator was helping hang miner cable at poewer center, hurt back while lifting cable, went to hospital was treated and released for back strain missed one shift & returned to work on 3/17/06.
EE was assisting in material handling in prep. for a well maint. job he lifted one end of an 8' section of 10' pvc piping so another EE could position a plank under it when he felt something in his back pop. Witnesses reported he was use- ing proper lifting techniques when the accident happened. EE did not seek medical treatment until 3/23/06.
Employee was bending a roof bolt and felt a pain in his lower right abdomen. Employee begin missing work on 3/30/2006
EE was struck by roof bolter as it was being trammed past him in the #6 face.
EE reported:Loading bolting supplies onto the roof bolter, he bent over picked up a bundle of roof bolt plates (approx 6-8lbs) turned to place on machine and experienced pains in low back. Con't to work for 3 hrs., pain cont'd & he came out of the mine. Was taken to local medical facility for treatment. (Possible irritation to previous ee's injury)
EE was greasing a muddy loader when he lost his footing and fell off loader.
EE and one co-worker were attempting to change the loader attachment from bucket to forks. Bucket got stuck, so EE started hitting it with a hammer. Bucket fell on both EE's feet.
While attempting to walk around the left S/C setting at the feeder employee stated that he struck his head on a piece of J-Channel that was hanging down 2"" from the roof. EE continued to work until 8:30PM. He transported himself to Rleigh General Hospital X-rays were negative. Attending Physcian gave EE a Doctors excuse to come back on 3/24/2006
EE was assisting a co-worker to change machine bucket to fork attachment. The co-worker noticed the bucket was stuck, so he started hitting the bucket with a hammer. The bucket fell on both EE's feet, fracturing EE's left foot.
High traffic area & employee was in blocked view of loader operator, standing on side of trunk. EE's right shoulder was hit by the corner of the front end loader.
EE had just completed changing an air lock key from an elevated platform. When the EE started to descend the ladder from the platform, the ladder twisted under his feet causing him to fall approximately 7 feet to the concrete floor. The EE sustained a lost time injury to his back, ribs, and knee.
Employee stepped on pillar to check belt, he slipped off pillar and fell to ground. He sustained knee and back strain injuries. Knee was not healing, referred for surgery on 5/30/06.

EE was instructed to remain off work until X-rays read. Physician was away from the office through 3/30/06. Diagnosed with siatica & EE instructed to remain off work and follow up visit on 4/10/06.
injured and co-worker had loaded a batam rock duster into the bucket of a scoop. injured was pulling the rock duster toward the back of the bucket. the rock duster moved suddenly catching his left thumb between the frame of the rock duster and the push ram in the scoop bucket resulting in a broken thumb. injured was wearing gloves.
The employee picked up a 50# bag of clay. He opened the bag and was pouring the clay on a forklift travelway (concrete drive). He stated that he was slightly bent over. He felt a sharp pain in his lower back. He was carried to the local emergency room. Injury - muscle spasm/lower back strain.

Employee was handling 4"" pipe, when the other end fell, the employee's end bounced and struck his right thigh, causing a bruise. On 03/28/2006 the company was informed that his Dr. had taken him off work.
The employee was using a pipewrench to tighten a 3/4"" diameter pipe. The pipe was overhead - the employee was pulling down - a ""pop"" was heard and pain was felt in the right shoulder.

Dozer pushing in hollow fill, spoil slid in behind dozer, placing a large rock in the dozers run. Operator did not see the rock. EE backed into rock, injured his neck and his right side was numb. Sent EE to hospital.
Employee was replacing a ""cat drive"" on the Long Airdox mobile bridge unit when he felt a pain in his back.
While lifting replacement roller, employee felt pain in left calf muscle.
Employee was using a sledge hammer to bend a piece of metal on the tail gate of his coal truck. He stated that something popped in his thumb causing pain.

Miner was feeding pit run into crusher when cone crusher clogged requiring maintenance. Miner cleared clog using bar. A 4-5 in. rock was ejected hitting miner in face on left side in the nose and cheek area causing nosebleed and broken nose. Miner has knowledge and experience of pit conditions and process required to efficiently crush pit run.
EMPLOYEE WAS CUTTING A PIECE OF STEEL WITH A TORCH. THE PIECE BROKE OFF & FELL, STRIKING EE'S FOOT. IT CAUSED A FRACTURE JUST ABOVE HIS RT. BIG TOE PAST THE STEEL TIP OF HIS BOOT.

While carrying a bag of rock dust, EE fell to mine surface injuring back.
Injured was carrying two cans of hydraulic oil from #3 entry to the #4 entry, when he felt pain in his lower back. Employee did not go to hospital and worked his next scheduled shift. Employee started missing work on 4/4/2006.
EE was walking into storage shed and right foot went through pallet and twisted knee. Took EE to hospital, sprained knee.
Mechanic was working on the engine on the 11 haul truck and had to come down the work stand to get a tool. As he stepped off the last step of the stand, he felt his left knee pop & get stiff. The mechanic cont'd to work but the paid did not go away, so he reported to his supervisor and was taken to the hospital to get checked out.
ROCK FELL OUT OF RIB AT FACE AREA WHILE EMPLOYEE WAS SHOVELING OUT LIFTER HOLES. ROCK STRUCK HIM IN THE HIP/LEG.

Employee was being loaded by an excavator when a large rock rolled in the bed striking the side of the truck bed. Employee was bounced around in the cab causing possible whiplash.
EE had been operating a skat track then got off to help stack a pallet. As he bent over to pick up a rock, he felt a pain in his lower middle back.
Employee was roof bolting; spinning 7-foot bolt when piece of shale fell hitting some bolts in the forward bolt rack. Some of the bolts were knocked out of rack striking employee on right knee.
INJURED & CO-WORKER WERE REMOVING A SPEED REDUCER FROM THE END OF A MAGNETITE AUGER. AS THEY ATTEMPTED TO REMOVE THE REDUCER USING A CHAIN HOIST, THE REDUCER BECAME STUCK. THE INJURED USED A SLATE BAR TO PRY LOOSE, IT SWUNG & HIT HIS RT. KNEE CAUSING DAMAGE TO A LIGAMENT.
While loading a burn pump into the bucket of the bobcat, employee's hand got caught between the burn pump & the bobcat bucket.
Employee was repairing a gear box in the miner. While using wrench with cheaterbar, bolt freed and he felt something pop in his shoulder. Hix first day away from work was 3/20/06
While building a brattice the shuttle car operator bumped the brattice knocking some block on the employee. This accident did not become lost time until 03/28/2006.
EE was splitting a piece of sandstone using a 4lb hammer and stone chisel when a fragment of metal broke off the chisel head and embedded in his left forearm. Physican Diagnosis Foregin body L fore- arm. Metal fragment removed. Laceration sutured.
EE pulled up 2 loops of miner cable and started cutting the face of #3 entry, which was 20'deep. Other EE's started pulling loops up so the cable could be hung across the crosscut. As EE was pulling cable, a rock under his feet rolled causing his leg to twist at knee. EE thought he was ok. Report filled out 3/30/06, started bothering EE. LTA on 6/7/06, needs surgery.
Employee slipped and struck left foot on ground while climbing down ladder of loader.
Hooking up chain binder to haul dozer, he pulled chain to hook up. Chain had twist in it. As he pulled, chain spun binder handle and hit him in the mouth, busting front tooth.

The injured employee is a belt repair man. He was standing on bottom belt putting a 100' piece of belt in on top with a set of channel lock pliers. He lost grip on belt with pliers and fell back- ward off belt striking a 4' water line with his back when landing.
EE WAS ASSISTING THE MECHANIC IN TURNING THE CUTTING EDGES ON LOADER. EE WAS KNOCKING OUT THE BOLTS WHEN THE END BIT BROKE FREE AND FELL ON HIS FOOT.
Injured employee was changing bits in tailgate shearer drum. A rock fell from the roof line at the face and struck employee on his index finger & forefinger of his left hand, resulting in a laceration to both fingers.
EE states he slipped on sloped, wet bottom while carrying 50lb bag of rock dust, landing on right hip. No accident report filed, sought treatment on 3/30/06. Advised mine management 3/30/06.

While striping down Sag Mill liners off the wall a liner came down and landed on another liner and slid into injured's foot, breaking his little toe.
TRAMMING 400 TO GET CLEAR OF SHOT. 2 LARGE ROCK TRUCKS COMING DOWN HILL WENT TOO HIGH TRAM SPEED TO GET CLEAR, RAN OVER LARGE IMBEDDED ROCK IN HAUL ROAD, CAUSING ERACTIC JAR .FELT PAIN IN LOWER BACK, WENT BACK TO SLOW SPEED WITHIN 100'.
Employee reported: The section FCT was down. In the process of attempting to locate the mechanical problem, he thinks he may have bumped his right knee. After completing the repair procedure, started to experience soreness & stiffness to the knee. He was examined by an orthopedic surgeon, the diagnosis was osteoarthritis & loose body particles in the knee.
He was getting plates off the bolter for the roof bolts when the other operator moved the machine and ran over his left foot.
DID NOT START LOSING TIME UNTIL 5/4/06. While lifting and pulling miner cable, strained right shoulder.
This accident occurred 3/16/07 and became a lost time on 4/20/07. Employee was carrying oil pump to different location in mobile equipment shop. He stepped in a small crack in the concrete floor, lost balance and fell. Pump landed on palm of left hand. Employee received medical attention and was released. Additional medical diagnosis on 3/4/07 indicated surgery required on thumb.
Roof bolting, piece of slate fell and struck auger. Rock on auger hit EE behind right ear and right side of his face.
INJURED EMPLOYEE WAS PICKING ROCKS OFF OF SCALPER BELT WHEN ROCK CAME OFF THE SCALPER SCREEN & HIT HIS LEFT HAND. THIS RESULTED IN A CUT ON HIS HAND (2 STITCHES) & TWO BROKEN BONES IN THE LEFT HAND ON THE LITTLE FINGER.
EE was attempting to climb out of a 582 scoop in #46 wall crosscut E-14 Section. He bumped against loose coal/rock rib causing it to fall and strike him on the upper back and neck.
EE was operating a coal hauler when he ran over a rock. This caused him to strike his head on the machine canopy. Later in the shift his back began to hurt.
Employee was working on the dust control sandvik crusher deck. He attempted to pull on belt to line up bolt. While tightening up the fitting with a wrench, he strained his lower back. He returned to work on 3/16/06 and has worked every day since until 6/16/06. He reported on 6/15/06 to the office, that he has been going to physical therapy since 6/7/06.

The employee was bending over getting dressed in the bath house and felt a sharp pain in his lower back.

Employee was assisting other employees load a section of 8"" x 4"" pipe into the back of a pick up when he felt his left shoulder pop. (Employee started missing work on 8/16/2006).
EE was removing a stone (appox 90lbs) away from a crusher chain feeder tail adjustment. While moving the stone, another became dislodged, rolling and striking his foot.
While driving scoop outside, the injuried person hit a hole in roadway taking steering wheel out of hand and hitting on top of hand.
ON 3/15/06, EMPLOYEE WAS STRUCK BY A TIRE FALLING OFF OF A FLATBED TRUCK. THE TIRE ROLLED OVER LEGS AND TIPPED OVER. EE FINISHED HIS SHIFT & TWO MORE SHIFTS WERE WORKED. EE WENT TO THE DR. & WAS TOLD TO TAKE 3 DAYS OFF. EE QUIT ON 3/22/06 & DID NOT WORK THAT DAY.
Employee was walking and was struck by a piece of laminated sandstone with some slate mix. It fell out between a row of roof bolts. The rock was 31"" long, 33"" wide and 2"" to 5"" thick. It struck him on his back.
Inj ee and another employee was adjusting the blades of packer machine #2 at the packing House area. Suddenly the employee just by him moved the blades manually pinching inj ee's middle right finger between the blade and the blade's housing. The equipment was electrically locked -out
EE was fire bossing #2 head when he slipped and twisted left knee.

Employee was repairing a 1 1/2"" impact wrench using a vise. When he loosed the vise the impact was starting to fall. His finger pinched between the tool and the edge of the workbench. He received a fractured finger tip and stitches.
EE injured his left thigh when he came in contact with the head of a Joy 1415CM. EE was tramming the miner, and got too close. The head struck him on the leg.
Unit #1 - Employee was putting down cable crossers to take the bolter into #2 entry. When employee was lifting miner cable to move it against the corner of the rib, he experienced pain in lower back and a sharp pain going down left leg.
KNEE PAD STRAP BROKE OFF, PAD CAME OFF & EMPLOYEE HIT HIS KNEE ON A ROCK ON THE BOTTOM OF MINE.

EE started that he was cleaning the truck. He accidentally sprayed Armor All into his right eye.
Injured twisted lower back while shoveling on belt line.
While walking down the longwall set-up face, individual struck head against a roof bolt hanger. This jammed his neck. On 3/27/06 individual visited a doctor, the evaluation found his neck to be in spasm and causing a mis-alignment. Individual was kept off work and received physical therapy.
The miner was injured while racking out a 4,160 V switchgear for a raw mill. The miner went to open the switchgear for the wrong mill. He opened the switchgear still under full load. The mechanical interlock failed to hold. The miner is seriously burned.
EE had installed a new motor in R-35 Euclid. They were putting the hood down on the truck mounting bolts when EE fell off the R-35 Euclid.
EE was pulling a rock off picking table into a 5' by 3' shaft, which is just below the belt line. The rock hit his penis causing cuts and bruises.
EE was lifting forks to put back on to Lo-Trac when he felt pain in Right shoulder. EE did not start missing work due to this accident until 3/22/06.
EE was pulling a torgue shaft out of Conv. motor on Joy miner when pulled outer cover hot oil had built up behind the cap & expelled on right hand
EMPLOYEE WAS SITTING ON MINE FLOOR SERVICING A MANTRIP & WHEN HE STARTED TO GET UP HE FELT PAIN IN HIS RIGHT KNEE.
EE was operating a continuous miner when section of roof fell. The coal section broke into smaller pieces when it came in contact with the shuttle car he was loading. This caused a piece of coal approx. 3'x 4'x 6' to strike the injured EE from the side near his hip and upper leg.
He was helping lap belt with three other employees and felt something pop in left shoulder.
Pulling miner cable slack to hang cable.
Employee was walking back to the towers along the power cable when a coworker swung the basket to pull the slack in the cable to the back of the shovel. When the basket swung, the cable lifted up and struck the employee knocking him to the ground. This resulted in a fracture to the left wrist.
EE was replacing roller on #1 belt line and twisted his knee.
Employee stated he was performing early startup activities and as he was walking around a dozer blade to check oil levels, he apparently stepped on uneven ground, causing his knee to 'pop.' Shortly thereafter, he knee began to hurt and swell.
The employee was pulling a cable and lost his balance. The employee fell to the ground on top of the plug. Employee started losing work days on 3/17/06.
Employee stepped out of the tool truck onto the ground and stepped into a rut, turning his left ankle.
EMPLOYEE WAS HELPING PUT A WEAR PLATE ON BELT #3 AT THE PLANT WHEN 2 OR 3 GOLF BALL SIZED ROCKS FELL FROM ABOVE & HIT HIM ON THE BACK & ARM.
Rock dusting the face area, walking backwards throwing rock dust and stepping in a hole, twisting his right ankle.
EE was assisting a continuous miner operator when a section of roof fell. The coal section broke into smaller pieces whe it came in contact with the shuttle car he was standing near. This caused a piece of coal approx. 3'x 3'x 6' to strike the injured EE from the side near his knee pushing his leg in a horizontal motion.
EE was changing the chain on the D9R dozer. When he hit the hammer to the shoe, a fly passed through the security spectacles and impacted his left eye.
EE was exiting the boom truck, his foot got caught between the truck and fuel tank. EE fell straining groin muscle. He went to the doctor 3/13/2006, received treatment and returned to work. After a period of time he complained that the injury still existed, returned to the doctor 4/3/2006 recommended three weeks recovery.
EE was assisting drill operator and while adding drill steel got his left hand pinched between steel and coupling.
EE alleges that he tripped and fell while walking across the quarry. Nobody witnessed the alleged fall. EE has tried unsuccessfully (to date) to have a prior injury to the same shoulder covered under workers compensation.
EE stated that he was walking along side the crusher power unit when he got approx 10 ft. from the last open cut xct. the rib and part of the top rolled out and struck him knocking him to the mine floor. Second rock fell & wedged between the mine floor and power control box. This kept the weight of rock from falling completley on him.
Employee was lifting a bag of b-bond when he felt a pain in his right groin in the same area where he had had a hernia repaired in April of 2005. Doctor reports have not been sent from the doctor seeing him after this incident on 3/13/06 as to the extent of the injury.
EE was building an intake stopping on 005 section. He was standing on ladder finishing wedging the stopping when a piece of draw rock fell and hit on his head and down his back causing him to fall off ladder to the ground. He was about three steps on ladder when he fell landing on his back.
EE WAS CUTTIN GALVANIZED PANEL USING A CUTTING TORCH & SUFFERED A CORNEAL ABRASION WHEN A PIECE OF HOT METAL ENTERED LEFT EYE.
Individual slipped on ice as he was getting out of van. Was seen by ER doctor on 03/12/06.Received MRI on 03/15/06.Final diagnosis was received on 03/29/06.Diagnosed with left shoulder sprain. Prescribed therapy for 2 weeks.
Employee attempted to keep jackleg from falling forward and experienced pain in his lower back. The physician did not release him for work pending additional evaluation.
Unit #2 - Employee was bolting #8 entry. The height was 5' 9"". Experienced pain in lower back going down left side and left leg when moving a bundle of roof plates. Pain got worse as employee tried to resume bolting.
While EE was grinding steel, a piece came up and under his full face shield and glasses and lodged in his right eye.
EMPLOYEE WAS RIDING IN THE FRONT AREA OF A BATTERY POWERED RAIL RUNNER MANTRIP WHEN IT JUMPED RAIL CAUSING THE SUDDEN IMPACT OF THE FRAME OF THE MANTRIP AGAINST THE TOP OF THE RAIL. THE SUDDEN JOLT FROM THE IMPACT CAUSED A BACK STRAIN AS HE WAS SITTING IN A CROUCHED POSITION WHEN THE ACCIDENT OCCURRED.
Moving power cables to prepare powertrain for transport. Stumbled over rock while dragging cable, fell against another EE who was helping him. The other EE fell back on him twisting his right ankle.
Employee was pulling a shield and when he lowered the shield, the baselift blew out the threads of the cylinder, causing metal shards to strike employee in the arm.
Employee was installing V-belts in the scalper screen, using a digging bar for tension. The bar gave way and he fell to the ground injuring his knee.
HOOKING UP 20 TON DIESEL TO SUPPLY CAR & GOT THUMB CAUGHT BETWEEN COUPLING & TONGUE.
EE was performing maintenance on a horizontal roller conveyor using a hammer. The hammer slipped and hit his left index finger while he was holding a punch. No modified work available and his accident, will be a lost time.
Lime trailer was plugged up. Opened clog with a rod. All hydrated lime dust broke loose at one time. Got it in eyes.
Removing and installing segment on D11 Dozer. EE did not miss any work with this injury at first. His back was still bothering him so he went to Dr. on 4/20/06. & Dr. took him off work. EE is currently off work due to injury on 3/10/06.
EXCAVATOR OPERATOR WAS BRUSHING IN FRONT OF A SURFACE MINE. A DEAD TREE LIMB FELL OUT OF THE TOP OF A TREE AND STRUCK HIM IN THE LEFT WRIST.
Rock fell from overhead belt, bouncing off the floor of backhoe the employee was operating (canopy is in place and in good working order). Rock bruised right ankle.
IW was opening a valve when he got dust in both eyes.
AS HE WAS WALKING THROUGH THE #36 CROSSCUT ON THE LONGWALL BELT, HE SLIPPED IN A TIRE TRACK & FELT PAIN IN HIS RT. KNEE. HE CONTINUED TO WORK UNTIL 3/27/06, AT THIS TIME HE WAS DIAGNOSED WITH A TORN LIGAMENT.
Employee's left finger was lacerated & fractured after it was caught between the belt press roll and the press frame, requiring hospitalization for potential grease infection since employee has diabetes and a pin emplacement.
The employee was helping another employee installing a belt on the Press. As they started to work on it, the equipment operator activated the conveyor by mistake causing another employee to fall in the conveyor. The employee tried to help his co-worker and he was injured with the conveyor causing him a fracture on his pelvis, back pain and injuries on his legs.
WHILE CHANGING V-BELT ON #1 SCREEN DECK, EMPLOYEE'S RIGHT FOREARM WAS CAUGHT BETWEEN GUARD AND THE BELT, CAUSING A LACERATION.
EMPLOYEE WAS TRAVELING THROUGH THE PRIMARY ESCAPWAY ON INTAKE, A PIECE OF ROCK FELL BETWEEN ROOF BOLTS & HIT HIS RIGHT WRIST.
DID NOT START LOSING TIME UNTIL 3/14/06. While holding cable, reel started and caught finger between reel and cable. Fractured right little finger.
Employee was digging into a rock pile with a front end loader, when he tilted the bucket upward, the back end of the loader came off the ground causing the employee to pitch forward. He struck his stomach on the round ball on the steering wheel causing an abdominal contusion.
EMPLOYEE BACKED THE ROCK TRUCK HE WAS OPERATING OFF THE NORTH DUMP, TRUCK TURNED UPSIDE DOWN & SLID APPROX. 75-100'TO THE BOTTOM. EE WAS WEARING HIS SEATBELT & REMAINED IN THE CAB. ANOTHER EE WAS MAINTAINING THE DUMP SITE & OBSERVED THE INCIDENT.
Miner was in compliance with rules and regulations. The equipment involved was the mill roll and he was using personal protection equipment and other safety devices. Miner was well trained and was using and following precations to prevent.
Employee was lifting and handing off an anti-spin mechanism out of a cone crusher, when he experienced sharp pain in lower back, buttocks, and legs. Doctors report reveals bulging disc in lower back. Unable to work next scheduled shift on 3-17-06
EMPLOYEE WAS PUSHING ROLLER IN PLACE, THE ROLLER FELL STRIKING HIM ON THE WRIST.
EE WAS WORKING AT NO.24 BRATTICE WHEN EE CAP LAM STARTED GOING OUT. EE STARTED WALKING UP TO NO.34 BRATTICE TO CALL FOR A NEW CAP LAMP WHEN EE TRIPPED HURTING LEFT WRIST, ELBOW AND KNEE.
EMPLOYEE STATED THAT HIS LIGHT WENT DEAD & HE STEPPED IN A HOLE IN THE MINE FLOOR, TWISTING HIS KNEE.
Employee slipped on film of mud on floor east of TC-2317
RUNNING SHUTTLE CAR, HIT A HOLE IN THE ROAD AND HIT HIS HEAD ON THE CANOPY.
While walking across the parking lot the employee stepped on something and twisted his ankle causing him to fall and hit his right knee on the asphalt. Did not become a lost time until 3/17/06.
EE was running s/c, stopped to move a rock out of roadway and reinjured his left forearm.
HANGING ROPE ON DOLLY, GOT FINGER CAUGHT IN ROPE, CAUSING ROPE TO PINCH & CUT HIS FINGER.
THIS INCIDENT BECAME LOST TIME DUE TO EMPLOYEE HAVING SURGERY ON HIS FINGER.
Operating roof bolter, drill steel got stuck in hole. Operator reached out to hold steel and was going to hit the reverse rotation on drill. Instead he hit forward causing steel to come out and strike the back of the operator's hand causing a break in the left wrist bone.
EMPLOYEE WAS JARRED WHILE RIDING IN MANTRIP. EMPLOYEE DID NOT BEGIN MISSING WORK UNTIL 5/8/06.
EE was driving a ramcar when he hit a hole in the travelway, jarring his back.
EE stepped on a rock and twisted ankle. Began missing 3/13/06. Diagnosis slight tear in achillies tendon.
Employee was removing tools from a drawer when the weight shifted and the drawer started to fall. He caught the drawer and felt pain in his back. He was taken off of work on 3/30/06.
Employee was picking up a pump when he pulled something in his lower back. Employee was also experiencing chest pain.
EE was operating the continuous miner, making a clean-up run on the right side of entry (curtain side). He was stooped down and a piece of rock slid off rib, striking him on lower left side of back. Went to Dr. on 3-8-06. Went back to Dr. on 3-13-06.
Employee was walking when his ankle rolled over causing him to fall. He sprained his right thumb and had abrasion to his right palm. Doctor put splint on thumb. Doctor's excuse 2 days no work. We could not get employee to come back to work on light duty, he stated he was quitting.
Employee on truck reached to pick up box, went to turn around and his left knee popped and he couldn't move leg.
Working on undercarriage, trying to get top roller off using a hammer, a piece of metal came off the roller and was embedded in the right little finger.
WHILE WALKING TO THE CAT 825 COMPACTOR, EMPLOYEE STEPPED IN AN IMPRESSION IN THE GROUND MADE BY THE COMPACTORS SHEEPS FOOT DRUM & EE TWISTED HIS LEFT KNEE.
EE was getting out of the backhoe He stepped on the backhoe track which had wet dirt on it and he lost his footing and fell. His hip/back hit the track causing contusion.
Employee threw block onto back of trailer and his back went out.

Injured states that he was turning rotor by hand to inspect for wear, when his hand got caught between the rotor & frame. (He works 4/10 hr. days.) Off 3/8 & 3/9.
While setting curtain timbers, a timber rolled off back of scoop, fell and hit his foot. Fractured and cut his right 4th toe.
EMPLOYEE WAS EXITING SCOOP WHEN HE STRUCK HIS HEAD ON A CABLE BOLT, CAUSING PAIN IN HIS NECK.
As he was operating a roof bolting machine, he bumped hard into ATRS portion of the machine with his shoulder. He continued to work, but started missing work on August 14,2006 to have rotator cuff surgery.
Injured was struck by a steel plate opening and shutting a flood gate when bagging material . Material was stuck he opened gate and fell on hand 4 x 6"" plate.
Employee was hand-loading cobbles into loader bucket to clean around base of stairs. He was facing away from the loader and tossing cobbles backwards. Cobbles were next to a metal platform. He struck left elbow on metal frame causing large contusion just above elbow.
Employee complains of abdominal pain during work activity. He started missing 3/9/06. Hernia surgery is scheduled for next week.

EMPLOYEE REACHED OUT OF STOCK TRUCK TO UNLOCK TAILGATE AND PULLED A MUSCLE IN HIS LEFT SHOULDER. THE EMPLOYEE FELT PAIN BUT DID NOT SEEK MEDICAL ATTENTION UNTIL 3/10/06. ON 3/10/06 THE EMPLOYEE WAS ABSENT FROM WORK AND WAS PAID FOR TIME AWAY FROM WORK.
ROCK FELL FROM THE ROOF, STRIKING EMPLOYEE ON THE RIGHT HAND, PUNCTURING HIS HAND ON THE TRAM LEVER. MEDICAL TREATMENT INJURY.
Employee was climbing the logwasher hopper when he hit his knee on a piece of angle iron. He injured his knee cap.
EE was installing belt structure for the longwall belt on headgate #10. He was lifting a piece of 60"" top structure when he felt a pain in his right shoulder. He completed the shift and didn't report to work his next shift.
The haul truck went off the side of the road traveling over a berm and onto a lower section of a ditch terrain. The operator was jarred around inside the operator's cab causing multiple injuries. Initial assessement of injuries reveals laceration to chin, lips, pain to right leg and to lower back.
Injured was installing a degas packer in a degas hole. The packer was pressurized but continued to leak water. Injured looked into the hole and the end of the packer blew out, striking the injured in the face. (Fractures and lacerations to lower and upper jaw.)
The miner was removing a pin from a dozer blade with a hammer and punch. He misjudged the contact point of the punch, therefore, striking the left pinky finger. Impact caused fracture.
Employee was helping clean trapped material from behind the driven sheave on the primary crusher. He was manually turning the sheave while another employee knocked the material out. He caught his left index finger between the sheave and the crusher frame, fracturing the finger.
Employee was pulling and lifting a steel cable, used to pull the gob truck as it was stuck in mud. He felt a pain in left shoulder and neck.
EMPLOYEE HAD JUST HUNG THE MINER CABLE IN THE #3 ENTRY WHEN HE TURNED & WAS WALKING TO HANG CABLE AT NEW LOCATION WHEN HE WALKED INTO TOP.
While maintaining a pineapple (hoisting apparatus) in Plant II, he felt a pain in his lower back.
After picking up a pry bar, EE reported severe pain in right lower back.
EE was installing a new chain sprocket on the digger drive. After the completion of the project, EE felt pain in his right hip. Plant manager was not notified of injury until 3/15/2006. EE was seen by Dr. & recommended to have MRI and be seen by surgeon. EE underwent surgery on 3/21/2005 for herniated disc.
DRIVER OF DUMP TRUCK SAID HE HIT A DIP IN THE PIT, BOTTOMED SEAT OUT JARRING HIM AND HURTING HIS BACK.
While cutting a bolt with a torch, the employee's pants caught on fire causing 3rd degree burns on his leg.
EE was using a cutting torch. EE was wearing safety glasses and a piece of hot metal flew into his eye.
While operating roof bolter, a piece of rock fell between last row of bolts and TRS striking EE on neck and left shoulder.
Employee sustained an Electrical Flash Burn while working in a 480VAC MCC. Flash Burn area was to the Head and Neck area.
Individual was trying to reform a piece of the frame of a SC21 shuttle car, prying on it a hammer flew out and hit him in the hand.
THE EMPLOYEE WAS HANGING CABLES FOR THE MOBILE ROOF SUPPORTS. A PIECE OF DRAW ROCK DISLODGED FROM BETWEEN ROOF BOLTS AND STRUCK HIM ON HIS RIGHT LEG JUST ABOVE HIS KNEE. THIS RESULTED IN A LACERATION WHICH REQUIRED SUTURES.
Employee experienced pain in her knee. She originally stated that it was not work related. The Doctor recommended surgery and today it was reported to us that it could be work related. Employee claims she may have suffered damage to her knee when she tripped on a rock and landed on her knee.
Turning on feeder, fell against rib on wrist. Had surgery on 5-24-2006
EMPLOYEE WAS LOADING A SCRAPER IN A BARROW PIT, HE CALLED THE D8 REQUESTING A PUSH. HE STOPPED DUE TO THE HARD SPOTS & THE DOZER HIT HIM, CAUSING HIS HARD HAT TO KISS THE STEERING WHEEL. HE WAS TAKEN IN TO THE E.R. & GIVEN TWO DAYS OFF WORK.
Injured was helping another operator to raise bosco beams to roof. As the TRS was raising, the TRS pad with beam on it tipped causing the beam to slide off and hit his left forearm.
Two welders were working on lib back to back. Reflection of other welder flash burned eyes.
#4 Entry @ 45 X-cut - Employee was a passenger in mantrip driven by co-worker, sitting in middle seat on right side of mantrip. Operator backed up and hit the rib on the right side causing employee to go back into frame of seat. Injury to neck area and upper back. Released to RTW 03/09/06. Lost 3 Work Days.
CONDITIONS CONSISTED OF WATER BEING SPRAYED ONTO STEPS BY WATER TRUCK, CREATING SLIGHT AMOUNT OF MUD ON STEP. EMPLOYEE WAS WALKING INTO BREAK ROOM TO GET ICE FOR HIS PERSONAL COOLER. AS EE WALKED UP THE STEPS, HIS LEFT FOOT SLIPPED IN SOME MUD.
HE WAS CARRYING RAIL JACK ACROSS RUN, SLIPPED AND FELL CATCHING FINGER IN BETWEEN LIFT PART OF JACK SMASHING HIS FINGER. * STARTED MISSING ON 03/06/06**
ee was injured when he lifted a rock out of crusher on 3/4/06.It was reported to company 5/3/06 when ee went to have a MRI on his back.
