A roof fall in the 1 South in #4 entry in the pillar-line between cross-cut #40 and cross-cut #41 measuring 50ft. x 19ft. x 6ft. This area was mined in April 2004 and supported with 6ft. tension re-bar bolts. This fall was caused by an undetected slip in the mine roof.
A ROOF FALL OCCURRED IN THE 4TH SOUTH PANEL AT #3 CROSSCUT BETWEEN #2 & #3 ENTRY. THE FALL MEASURED 25 FEET LONG, 18 FEET WIDE & 4 FEET THICK.
EE supervisor left the mine/road bldg site @ 2:30pm 8-31-04 after an hour of fire watch and servicing cut 231 dlc. the next moving on his way to machine he found the cat 231dlc burned out. ee contacted other ee and another ee contacted 911 & an fire inspector on the details of the fire. they seemed it mechanical accident.
A roof fall occurred on the 3rd section main west belt entry #4 at crosscut 156. Dimensions of the roof fall are 28' length, 18' width, 15' high. The intersection was supported with 10' point anchor bolts. The fall will be cleaned up.
A roof fall in the 1 South in the #4 entry in 1/2 the intersection of cross-cut #22 extending through 1/2 the pillar-line towards cross-cut #23 measuring 35ft. x 19ft. x 6ft.-9ft. This area was mined in April 2004 and supported with 6ft. tension rebar bolts and 9ft. mega bolts. This fall was caused by an undetected slip.
A ROOF FALL OCCURRED IN THE 1ST NORTH, #10 ENTRY, #50 CROSSCUT. THE FALL WAS 25' LONG, 25' WIDE AND 8' HIGH. THE FALL DIDN'T AFFECT TRAVEL, VENTILATION, DAMAGE EQUIPMENT, OR CAUSE ANY INJURY. THE AREA WAS BOLTED WITH 6' & 8' POINT ANCHOR BOLTS.
Mine electrician was traveling underground when he found the fall. Two others went to check fall. It stopped falling at 10:40 a.m.
Roof fall occurred in #5 entry of south mains at 20 crosscut. This was the belt line. Ventilation was not impaired. No people were injured or equipment damaged.
A roof fall occured between entries 4 and 5 of the 2nd North East at survey station 00. The fall measured 40' long by 18' wide by 9' thick. This area had been supported with 9' double locks and 6' fully grouted resin bolts. All approaches will be timbered.

An unintentional fall of roof occurred while conducting retreat mining on 013 section. The fall occurred one break outby spad #9199.
A NON INJURY OCCURED WITH AN APPROX. 400'SECTION OF HIGHWALL, SLOUGHED OFF INTO & UP AGAINST THE COAL HAULER. THE EMPLOYEE WAS NOT INJURED. COAL MINING ACTIVITIES IN THE NORTH PIT. PUMP 3 WAS SUSPENDED & EQUIPMENT & PERSONEL WERE REMOVED FROM THE PIT. EXTERNAL DAMAGE OCCURED TO THE COAL HAULER. MSHA AZTEC OFFICE WAS NOTIFIED THE NEXT MORNING.
A fire started in the right rear compartment of the engine area. the source is oil and heat but which hose and what type of oil is unclear at this time. the fire spread from rear of truck to the deck and cab.

A roof fall occurred near spad 5248 it measured 8 to 10 feet thick, 18 feet wide and 120 feet long.

A roof fall occurred at break #15 on the 4-c belt line. The fall measured 40' long by 18' wide and approx 8' thick. The area was developed several months ago and did have cribs installed on the belt line. The fall will be rehabilitated.

A ROOF FALL WAS FOUND BY THE FIREBOSS DURING THE PRE-SHIFT EXAMINATION OF THE EP AREA IN THE SOUTH MAINS INTAKE ESCAPEWAY. FALL WAS APPROXIMATELY 30 FT IN LENGTH BY 20 FT IN WIDTH BY 5 FT IN HEIGHT.
During pre-shift the pre-shift examiner heard a rock-fall. The fall was 2 breaks inby spad #2180 in the #5 entry. The fall measured 60'L X 20'W X app. 6'H.
A roof fall in the 6 North Panel in #3 entry in the intersection of cross-cut #37 and extending part of the way through the pillar-line towards cross-cut #38 measuring 60ft. x 18ft. x 5ft. This area was mined in August 2004 and supported with 5ft. fully grouted roof bolts. This fall was caused by a hidden slip in the mine roof.

A roof fall occurred on track entry about 300 ft inby #12 switch of Somerset portal. No ventilation was affected and no equipment was damaged.
A mud slip occurred in the #5 entry at the last open intersection spad #6 to 50. this lsip caused an unintentional roof fall app. 20' x 20' x 6'. the intersection had been previously bolted.

A roof fall occurred at the 3rd main west return aircourse entry #6 at crosscut 119. Dimensions of the roof fall are 40' length, 18' width, 8' - 12' high. The intersection was supported with 10' point achor roof bolts. The fall will not be cleaned up.
On August 27, 2004 an unplanned roof fall was discovered at the Justice No. 1 Mine. It was discovered during the weekly exam of the bleeder system. The fall size is unknown. The fall is in the # 1 entry near spad #427. Area was developed in June 2001 & was supported w/6' torque tension bolts; prop setters on a staggered 10' pattern.
A roof fall was discovered int he 2 main west #5 mains at 90 crosscut between #2 and #3 entries. the fall measures approx. 50 ft long, 18 ft wide, and 6 to 8 ft high. the area is supported with 5 ft fully grouted resin bolts.
THE FALL OCCURRED DUE TO A SLIP. MEASURING 8' X 18' X 50'. THE AREA HAD ALREADY BEEN CRIBBED AND DANGERED OFF BEFORE FALL OCCURRED.

A processor went bad in the elevator used to transport men from the surface underground. The processor was replaced and the elevator inspected.

AN ENGINE COMPARTMENT FIRE RESULTED FROM A MECHANICAL FAILURE ON 526-518 KRESS COAL HAULER DRIVELINE.
A roof occured in #1 hdg of 1 rt. set. 73-75 bk. The area was bolted with 4' resin bolts. Fall was approx. 5 to 6 high 16ft wide. the immediate top was gray sholae. this area is 8 blicks outby section and was develped approx. 8-5-04 this is 9 return entry, we have 2 return hygs in this area. fall will not be cleaned up.

Miner was walking by load center at m/b 12 and saw flames coming out of lid approx 1 1/2'. two contract drillers came by and attempted to extinguish flame with 20 lb. fire extinguisher while miner contracted dispatch. when they were unsucessful, stenck was released & mine was evacuated. there were no injured. initial investigation caused were old load center running

AMPHIDYNE FAILED AND WAS REPLACED. PHONE CALL WAS ATTEMPTED AT TIME OF INCIDENT BUT WAS UNSUCCESSFUL. HOIST RETURNED TO SERVICE 8:45 A.M.
While conducting the daily inspection, the elevator failed to operate properly. The BRO contactor failed and was replaced.

A roof fall occurred measuring 20' by 20' by 8-10' high beginning at spad #1515 in the #5 heading. No equipment was involved, no ventilation was disturbed, no injury occurred and the fall will not be cleaned up.
Mine lost power 8/23-9:35 AM. Compromises two ways out. Employees stopped work, assembled in lunch room. Problem with KCPL-no info on duration of outage. Evacuation ordered 9:40 using backup power supply, called MSHA. Evacuation recorded 9:40. 9:47-employees out of mine and accounted for. Phoned MSHA later in day-employees sent home, will be back 8/24-6:00 AM shift.
The skip stuck open due to mud build up. This make the hoist inoperable.
While scaling loose rock with the scaler, rock came down on the transfer of #2 & 3 belts. The area was approx. 8 ft. thick x 25 ft. in diameter.

A roof fall was found in the No. 8 entry INTAKE, the fall measures 35' long, 18' wide, 6' thick. The fall occurred 2000' outby the 002 section and was due to laminated rock and coal streaks in the strata. The area was developed Mar 04 with 5' torque tension roof bolts. The fall did not impeade travel/ventilation and NO injuries occurred. The area was resupported and barricaded.
IN THE #4 BELT ENTRY, ON THE #2 BELT, 18' OUTBY SPAD #1337, A FALL OCCURRED ABOUT 5:15 P.M. MEASURING APPROXIMATELY 9' W X 60' L X 7' H.
A FAILED FUSE AT THE MAIN SWEPCO SUBSTATION RESULTED IN A POWER INTERRUPTION THAT AFFECTED BOTH HOISTS. AUXILIARY POWER WAS DOWN AT THE TIME WHICH REQUIRED MINERS TO REMAIN UNDERGROUND UNTIL SWEPCO POWER WAS RESTORED AT 2:03PM. THE HOISTS WERE TESTED & MINERS RETURNED TO WORK.
Pre-shift examiner discovered a roof fall which had fallen sometime over the week-end. The roof fell in the face area of no.2 entry, across the intersection to the No.3 entry and was 5' thick.

The Hart Street elevator malfunctioned and would not operate properly. A door switch malfunctioned and would not let the elevator function. The door switch was replaced and the elevator was returned to normal operations around 12:30 A.M.

AN UNINTENTIONAL ROOF FALL OCCURRED AT THE GALATIA NORTH PORTAL ON THE #3 BELT/CROSSCUT 94/7950'. THE FALL WAS APPROX. 100' LONG, 18' WIDE AND 8' THICK. THE FALL DID NOT AFFECT VENTILATION OR PASSAGE OF PEOPLE. THE FALL WILL BE CLEANED UP. FALL #21U.
ROCK FALL IN #1 ENTRY ON #2 SECTION APPROX. 1100' FROM FACE (SPAD #682). FALL WAS 2.5'L X 17'W X 15'H AND WAS BREAKERED OFF.
A ROOF FALL OCCURRED AT THE ABOVE MENTIONED MINE. THE SIZE OF THIS FALL WAS 30' X 20' X 4'. THE LOCATION WAS IN THE NEUTRAL AREA, SPAD 5878, 900' FROM THE PRODUCING SECTION. ROOF SUPPORT WAS 42"" RESIN BOLTS. AREA WILL NOT SEE REHAB.
A fall of supported roof occurred on the 001 section, 15th crosscut, no. 6 panel, in entries no. 1,2,3,5,6,& 7. The fall was appx. 10-18' thick, rib to rib. No injuries occurred and no equipment was damaged.
ON FRIDAY MORNING AT 9:15 AM A ROCK BURST OCCURRED ON THE 5500 LEVEL IN THE 31 STOPE. THE MAIN EPICENTER WAS APPROXIMATELY 250 FEET EAST (STRAIGHT AHEAD) AND 64 FEET BELOW THE 31 EAST STOPE. SLIGHT DAMAGE ACCURRED IN THE STOPE, ONE SHIFT OF SCALING AND BOLTING WAS REQUIRED TO PUT THE STOPE BACK INTO NORMAL OPERATION. THE MSHA FIELD OFFICE IN BOISE WAS NOTIFIED.
A rock fall occurred 3 breaks inby 017-C section headdrive located in the first right of the first right off the Fields Fork Mains. Fall measured 4 ft thick, 40 ft long and 20-25 ft wide.
A roof fall occurred at no. 6 crosscut of ""0"" entry at a 4-way intersection. This intersection had been re-bolted with 8' cable bolts; however the fall occurred in a horseback above the anchorage point of two of the cable bolts. The remainder of the area is intact. The area that fell was approximately 12' x 12'. Contributing cause was the ongoing squeeze.
Roof fall in 1st left#5 entry x-cut #11 in a bad fault in intake. Surrounding areas had already been cribbed off & isolated. Area bolted with 6' double lok bolts. Fall was 25'x20'x8' thick. Area mined in april 2004.
A roof fall was discovered on the 3 North Panel at #45 crosscut, between #2 and #3 entries. The fall was adjacent to spad #17447. The fall was approximately 6' high, 20' wide and 45' long. The area had been originally bolted with 5' bolts. The area was cribbed off.
125+51 #1 RETURN ENTRY - 5 NORTH - ROOF FALL. MINED IN FEB. 2004. 10-12' HIGH X 16-17' WIDE X 100' LONG. INITIAL FALL WAS NOT ABOVE ANCHORAGE, BUT MORE MATERIAL FELL OUT LATER.
An unintentional roof fall was discovered at about 5:30 AM in the intersection of the #3 entry and into the right crosscut at survey station no. 1311. The fall was approximately 4 ft high x 19 ft wide x 60 ft long.
An unintentional roof fall occurred at c-mains, 1 block inby spad 5666 (37 block) on 4 track. it is 6' high x 18' wide x 60' long. it is in an abandoned area in common air.
A roof fall occurred in entry #1 of the 2nd N.E. mains at survey station 12+60. The fall measured 140' long x 19' wide x 9"" thick. This area had been bolted with 6' fully grouted and 9' double lock bolts. All approaches will be timbered.
Based on the information how received from those present it appears that the ign. tron was caused when the spark from the cutting machine shearer ignited an unknown methane bleeder while cutting hard rock land coal at shield #104 with headgate drum. msha was notified timely.
Unintentional Roof Fall in the return (old works) app. 7200' from face in Pit II, #1 & #2 Returns. Dimensions: 300' Long X 20' Wide X 5' High. No spad # available. Fall discovered during weekly exam.
A rock fall occurred at 1:30 p.m. in the #2 heading 10' inby spad #1366. there were no injuries. the travelways and airways were not affected. the fall measured approx. 5 ft high, 18 ft wide, and 65 ft long.
ELEVATOR WAS DOWN FOR MORE THAT 30 MINUTES BECAUSE OF MALFUNCTIONS AND BROKEN DOOR SWITCHES. THERE WAS NOT DAMAGE TO THE ELEVATOR.
After person replaced some blown fuses on hoist drive system,operator, ee ran safety's with a fully loaded skip going down ward and counter balanced empty one going upward. when he passed overspeed ee which is closed to bottom. loader skip did not stop in time before getting bottom. damage appears to be broken bolts hd dumping bucket on bottom skip.

Roof Fall in return of Hatfield mains entry #4 & #5 x-cut 20. This was secondary escapeway which was rerouted and was found on a weekly examination. Fall was 25' wide x 130' long x 10' high. Fall was in slip and tied to an existing fall and area was bolted with 6' resin bolts and 8' resin bolts.
A ROOF FALL OCCURRED IN THE NO.1 RETURN ENTRY APPROXIMATELY 140' OUT OF THE FACE ON THE 002-0 ""R"" SECTION. THE FALL MEASURED APPROXIMATELY 100' LONG X 18' WIDE X 8' THICK. THE AREA WAS KNOWN TO HAVE SOME ADVERSE CONDITIONS AND THE AREA HAD BEEN NARROWED TO 16' WITH CRIBS ON ONE SIDE AND 10' CABLE BOLTS INSTALLED IN THE INTERSECTION.
A roof fall approx 22' l by 16' w by 8' h occurred in the no.5 entry in by spad no. 1988. This location is approx 8000' from the closest working section.

A ROCK FALL WAS DISCOVERED IN THE #5 ENTRY ONE CROSSCUT INBY SPAD #1550. THERE WERE NO INJURIES AND THE TRAVELWAYS AND AIRWAYS WERE NOT AFFECTED. THE FALL MEASURED APPROXIMATELY 1 FT TO 7 FT HIGH, 18 FT WIDE AND 50 FT LONG. IT APPEARED TO BE A SLICK SIDED ROCK.
THE SCOOP WAS BEING USED TO PULL A PIECE OF BELT TOGETHER AT #148 BREAK ON WEST MAINS. SCOOP BACKED INTO A PILE OF ROCK CAUSING THE BATTERIES TO RAISE UP & TWIST. THIS PINCHED 1 OF THE POWER LEADS AGAINST SCOOP FRAME & GROUNDED THE CABLE. THIS CAUGHT THE CABLE INSULATION ON FIRE. FLAMES PRESENT APPROX. 5 MINS. BREAKER WAS KNOCKED & ROCK DUST USED TO EXTINGUISH FLAMES.
CONT MINER WAS CUTTING COAL ON #1 UNIT #2 ENTRY. HE HAD MADE A 20' CUT ON LEFT & RIGHT SIDES. CUT A 12' ON RIGHT AND HAD THE LEFT OUT UP TO ABOUT 6 FT OF THE SQUARE OFF. OPERATOR, RAM CAR DRIVER AND BOLTER SAW AN ORANGE GLOW UNDER THE HEAD IN THE PAN. THIS LASTED 1-3 SECONDS & DISAPATED. FIRST CUT OF A SET OF ROOMS INTAKE SIDE WHICH HAD BEEN DRIVEN IN ON ENTRY ADVANCEMENT.
East Mains 6 entry, between 7 and 8 block, MMU 008 50 ft long by 17 ft wide by 10 ft high. Support: 6 ft 3/4 inch resin bolts, roof mats, wire screen. Fall dangered off, additional support installed around fall.
THE MAIN BELT ENTRY WAS IN APPROX 35' FROM EDGE OF HIGH WALL AND WAS SUPPORTED TO WITHIN 3' OF FACE. THEN MINER WAS TAKEN IN TO TAKE AN ADDITIONAL 6' OUT OF LIFT. ON CLEAN UP ROOF CAME ON BRING 3 PERMENT C CHANNELS WITH IT. ROOF THAT CAME WAS MUDDY AND BROKEN UP.
A ROCK FALL OCCURRED MEASURING 20'X20'X12' HIGH IN AN INTERSECTION AT SPAD #2582. NO EQUIPMENT WAS INVOLVED, VENTILATION WAS DISTURBED (VENTILATION WAS RE-DIRECTED AROUND THE FALL AREA) NO INJURY OCCURED AND FALL WILL NOT BE CLEANED UP.
Hoistman was trying to exhange chairs in the hoist cab while running. Accidentally hit brakes forward applying brakes on hoist with full power. Main breaker blew fuse. Hoist down 10:40 p.m. to 12:49 a.m.
A roof fall occurred in the intersection of the #4 heading at #147 break on the west mains. No injuries resulted from this occurence.
The intersection of #2 entry at #172 block (1 block inby spad #1136) fell. 18 x 18 x 6' high. the area was bolted with 5' & 6' super twist bolts. It also had 2-4 10' cable bolts in it.

A ROOF FALL WAS DISCOVERED IN THE RETURN (NO. 1 ENTRY) OF THE 001 SECTION. IT BEGAN 80' INBY SPAD NO. 1024 AND EXTENDED 70' UP TO THE NO. 1 ENTRY. THE FALL WAS 20' WIDE AND APPROXIMATELY 7' HIGH.
A fall of the roof occured in the 3rd s.e. subpanel, x-cut 27 in #2 entry on #3 unit the fall was 68ft x 25ft x 12ft. no ventilation, personal or equipment was affected.

WET SHALE AREA WITH SLICK BACKS. SLIP AREA PULLED LOOSE ALLOWING ABOUT 20-50' X 19'W X 6-7'THICK. THIS AREA HAS BEEN CRIBBED OFF NO AIRWAYS OR TRAVEL WAS AFFECTED.
A unintentional fall occurred on 3rd east tiway off main north due to adverse roof conditions.
AN UNINTENTIONAL ROOF FALL OCCURRED AT SPAD #43+ 65, ON THE 3RD SW SUBMAINS #UNIT (MMU-003-0) #4 ENTRY. THE APPROACHES TO THE FALL HAVE BEEN TIMBERED & THE AREA DANGERED OFF. IT MEASURED 35' X 50'X 15'. IT WILL BE CLEANED UP.
A roof fall occurred measuring 20' wide x 30' long x 10' high beginning at #2 entry at spad 635. No equipment was involved, ventilation was disturbed (crushed brattish has been rebuilt). No injury occurred and the fall will not be cleaned up.
A small ground fall occurred between 1:30-2:00 pm in the 13 north return escapeway just inby 5 block at about 3:30 pm it was observed that the remainder of bad ground had fallen. the approx. dimensions of the fallen ground was 60' x 25-30' wide x 10-12' thickness. no persons or equipment was involved.
While disconnecting battery charger for Ram Car #2166 from charger, one of the leads arced and caught ram car on fire. Crew turned the electrical power off and left the area because of smoke.
A ROOF FALL OCCURRED IN THE 13TH EAST, #5 ENTRY, #121 CROSSCUT. THE FALL WAS 20' LONG, 20' WIDE, AND 8' HIGH. THE FALL DIDN'T AFFECT TRAVEL, VENTILATION, OR CAUSE ANY INJURY. IT DID DAMAGE A SUPPLY TRAILER. THE AREA WAS BOLTED WITH 6' AND 8' POINT ANCHOR BOLTS.
Unknown conditions. Was found by a foreman (Jeff Morgan), as he was traveling in the mine. No one was working in the area when the fall occurred.
AN UNPLANNED ROOF FALL OCCURRED ON THE #2 BELT LINE, #4 ENTRY NEAR SPAD 212, NO ACTIVE WORKING FACE. DEVELOPED FEB. 1997. AREA WILL BE REHABILITATED. FALL MEASURED 50' LONG X 20' WIDE X 7' HIGH. NO ACCIDENTS REPORTED.
Kiln was slowed down and the bullgear lubricant overheated probably due to the change in heat profile which can occur when kiln is slowed. Fire was inside of the bullgear guard and took around two hours to extinguish. Damage is the loss of some lubricant and loss of production.
A roof fall was discovered during a weekly examination of the 2B bleeder entry. It occurred at #7 intersection of the OBTG section #1 entry. No people were injured; no equipment damaged; ventilation was not impaired.
The Cedar Creek elevator was out of service from approx. 9:30 pm until 5:15 am due to problems with the leveling switch & 48 volt relay.
Rockfall was found on the date and time in item 6. the fall was 30' long 20' wide 4-5' thick, located at spad 228 in #5 entry on first right.
Fuel flow failure in pre-cal tower resulted in an unexpected surge of hot raw material (dust) through kiln and clinker cooler. The material was conveyed to conveyor 62BC11 and ignited the belt conveyor before sensor could register the temperature spike. The conveyor fire was discovered at 1:11 p.m. and extinguished at 2:11 p.m.

A roof fall occurred at No. 7 entry at No. 7 crosscut. This was a continuation of the previously reported fall at No. 6 crosscut. Area was bolted with 5' fully grouted bolts. The ongoing squeeze broke off against the solid. The fall was 18' wide by 50' long.

THE HOIST BROKE DOWN APPROX 40 MIN, 4:00 PM TO 4:40 PM. THE SOUPLING BETWEEN MOTOR & REDUCER CAME APART. A NEW COUPLER WAS INSTALLED. WORK AT FACE WAS STOPPED DURING BREAK DOWN OF HOIST.

A roof fall occurred in the 2nd main west in the #6 entry at 71 XC. The fall measured 60' long, 17 1/2' wide and 5-8' thick.
The slope hoist at our Weatland Portal failed to operate properly. The investigation revealed that the slack rope switch located near the hoist drum had been tripped causing the malfunction. The switch was reset and the hoist was returned to operation.
Wet area with slikk backs and formations in roof with rider sham, allowed roof to separated above roof bolt machine and fell, fall has been cribbed off and travel way remarked, no plans to rehab this area.

A roof fall in the 1 South #2 entry in the pillarline between cross-cut #38 and cross-cut #39 measuring 50 ft. x 19 ft. x 6 ft. The area was mined in April 2004 and supported with 6 ft. tension rebar bolts. This fall was caused by a hidden slip in the mine roof.
Rock fall (70' length by 18' wide by 10-20' high) on 2 section approx 2400' from active face, fall located at spad #486 between #6 and #7 crosscut.
During the weekly examination, the examiner found a roof fall (about 30' by 15' by 12' thick) in the cross cut between entry 1 and 2 of the 9E headgate. The area had been last examined on 8/4/04

A roof fall occurred on the 3W roadway at cross-cut 5 in entry #5. The cause of the fall was weak laminted roof shale. The fall blocked a supply road to the Wheatland portal. The fall will be cleaned up and the roof and area will be re-supported.

AN UNINTENTIONAL ROOF FALL OCCURRED ON 3 RIGHT SECTION, 19 CROSSCUT #3-#4 ENTRY. THE FALL MEASURED 80' X 20' X 6+ FEET. THE AREA WAS MINED OUT 1500"" OUTBY AND SEALS HAD BEEN PLACED BETWEEN 19 AND 20 CROSSCUT.
A roof fall was discovered in the 2 main west #5 return aircourse at 88 crosscut in the connecting crosscut between #2 and #3 entries. The fall measures approx 50' long, 16' wide and 5' high. The area was supported with 5' fully grouted resin bolts.
The #2 portal elevator (Anderson Run) would not operate. The brakes on the elevator would not release. Parts (coil) had to be replaced. No one was trapped on elevator. Elevator was repaired on 08/04/04 by 1:00 AM. Elevator was down for 6 hrs and 30 minutes.
Job mechanic had removed the old filtering system from the diesel fueling station so as to install a new larger system. Some spillage resulted. As he proceeded to weld on the bumper of the fueling station, the diesel spillage caught on fire and spread along the station. He could not control the fire in time by extinguishers, thus destroying the fueling station.
#4 HOIST DOWN DUE TO SHEARED KEYWAY IN SHAFT FOR ROTOTROL. SHAFT WAS REMOVED AND NEW KEYWAY WAS CUT. #4 HOIST WAS RETURNED TO SERVICE AT 10:00 AM ON 8/4/04. UNDERGROUND MINERS HAD TWO OTHER SHAFTS FOR ESCAPEWAY OUT OF MINE.

A roof fall occurred on the 1L4W panel, cross-cut 26, entry #3. The cause of the fall was weak laminated roof shale. The fall temporarily blocked the roadway to the #4 section. The fall was cleaned up and the roof supported.

An unintentional roof fall occurred at the millennium portal at the 5n longwall headgate entry #1, 4486n at crosscut #52. the fall was approx. 40' long, approx. 18' wide and approx. 10-12' thick. the fall will be cleaned up.
EE was making electrical repairs to the load side loop on a deenergized pole mounted solid blade disconnect switch. He had finished the repair work and was starting to come down the pole. Some how he received an electrical shock and fell down the pole.
The employee had backed under a feed chute to get a load. He dismounted the truck to activate the feed belt. He mounted the truck and moved it to spread the load evenly as it filled. He dismounted the truck to shut off the feeder and the truck rolled forward, pinning him between the truck and a cement pillar.
(3) MINERS WERE WORKING TO INSTALL & MOVE A NEW CONVEYOR BELT. PILAR WAS FEEDING THE BELT OFF THE ROLL TOWARD THE HEAD PULLEY. VICTIM WAS PULLING THE BELTING TOWARDS THE TAIL PULLEY WITH A 3/4"" ROPE ATTACHED TO THE LACING HOLES W/3 STRANDS OF WIRE. THE PULL ROPE DETACHED FROM THE WIRE & HE FELL BACKWARDS ONTO A CONCRETE SLAB. HE WAS 12'ABOVE THE GROUND, FALL PROTECTION NOT WORN.
Causes Unknown - Still under investigation. The victim was helping switch rail cars from a siding when he was struck & run over by the moving cars.
5 North face area, right side, cross 5 to 6 at survey station 38 and 82. Coal and rock fell out on right side of miner knocking man's head and neck into side of the continuous miner.
EE had just finished installing timbers and began to observe the top for the miner operator who had begun to cut the end cut in the #2 entry. A section of roof measuring 18' wide 0-60"" thick fell on ee causing fatal injuries.
During the Sag Mill 3 reline employee was removing one of the belly liners. He set it on the liner handler cart to remove it from the mill and was using his left hand to turn the cart when the liner shifted and smashed his left index finger amputating the the left index pad and finger nail.
While shoveling mud he pulled a muscle in lower stomach or groin area.
Injured was eating lunch with remainder of crew in shade of railcar, when locomotive hooked to cars & started moving. Injured was knocked down and in falling his hand was run over by wheel of railcar.
EMPLOYEE WAS OPENING CHUTE TO CHANGE FLOW OF ROCK, SLIPPED AND FELL. WHEN HE FELL, THE CHUTE DOOR SLAMMED SHUT ON HIS LEFT HAND, CUTTING OFF TIP OF MIDDLE FINGER.
EMPLOYEE WAS USING HAND WRENCH TO REMOVE BOLTS FROM RIDE STRUT ON HAUL TRUCK. COULD NOT BREAK LOOSE & HAD ANOTHER EMPLOYEE USE HYDRAULIC CRANE BOOM TO PUSH ON WRENCH, THE WRENCH CAME OFF THE BOLT HEAD & STRUCK EE IN BOTH HANDS & CUT OFF PART OF (1) FINGER & CUT/SMASHED (2)OTHER FINGERS.
An ee bent over to pick up a post when he felt a sharp pain in his lower back.

INJURED WAS DISMOUNTING FROM NW#2 BELT. HE HAD PLACED HIS LT. FOOT ON A TOP CONVEYOR RAIL WHILE HOLDING ONTO A STRUCTURE SUPPORT CHAIN. AS HE BEGAN TO STEP DOWN WITH HIS RT. FOOT, HIS LT. FOOT SLIPPED OFF THE RAIL & IN BETWEEN BELT & STRUCTURE, CAUSING HIM TO FALL. THE AREA WAS WET.
Ee was getting out of scoop. when he twisted around to get out his leg got hung and he twisted his left knee.
EE STATES HE HIT A CHUCK HOLE AND THIS HURT HIS BACK A LITTLE (8/31/04). THE PAIN LEFT AND EE DID NOT REPORT IT UNTIL THE AFTERNOON OF 9/2/04 (EE DID NOT SPEAK TO SUPERVISOR UNTIL 9/3/04). EE CALLED 9/3/04 AND STATED THAT HE HAD LOWER BACK PAIN & WAS GOING TO A DOCTOR. DOCTOR EXAMINED EE AND PRESCRIBED 2 MEDS. DOCTOR GAVE HIM WORK RELEASE FOR 9/7/04.
While using a sledge hammer employee missed the washer screw paddle. Sledge hit his right index finger smashing onto framework of wash plant. Fracturing right index finger.
Climbing down off Cat 992, operator of Case uniloader (skidsteer) struck operator in hand while on ladder w/blade extension. Bruises & abrasions only.
While cleaning shop floor with steam cleaner, stepped back onto a jack lying on the floor. While trying to regain balance steam cleaner shot hot water onto right foot penetrating the boot with the hot water. The result was 2nd degree burn to the right foot.
THE INJURED WAS USING A LADDER TO HANG BELT CHAINS. THE LADDER KICKED OUT FROM UNDER HIM CAUSING HIM TO FALL BACKWARDS. HIS HEAD STRUCK THE MINE FLOOR CAUSING PAIN IN HIS NECK.
Employee was installing the second roof bolt in a row of bolts when a piece of rock fell from the mine roof and hit the ATRS then rolled under the ATRS striking him in the left leg on the upper thigh.
EMPLOYEE WAS SPLITTING SANDSTONE WITH A CHISEL & AN 8LB. HAMMER. IN THE SPLITTING PROCESS, HE MISSED THE CHISEL, STRIKING THE MIDDLE FINGER OF HIS LT. HAND. INJURY OCCURED ON 8/31/04 BUT NOT REPORTED UNTIL 9/2/04. EE WAS TREATED FOR INFECTION & CONTUSION OF THE INJURED FINGER, (2) LOST WORKDAYS.
Employee was installing bolts when the bolt head became wedged in the wrench. Employee corrected the problem and in raising the boom by mistake, instead of the rotation lever, the bolt swung around with the plate cutting employee's left forearm.
EE was trying to position the bulldozer blade vertically with a loader. when the loader went back, the blade fell down and he was thrown out of the loader bucket and fell from an altitude of 10 ft.
EMPLOYEE WAS STEPPING DOWN FROM OVERDENSITY PUMP, FELT PAIN IN RIGHT KNEE/HIP.
Because of heavy rain, the crushing crew had to shovel before they could start up. Ee stepped off the concrete pad and his foot slipped in the mud. He tried to catch himself and when he brought his other foot around, he twisted his ankle and caused a fracture in the ankle area.
Large stone crushed finger while working on production line cutting & trimming stone.
EMPLOYEE WAS WALKING ACROSS SECTION WHEN HE STEPPED IN A HOLE CAUSING A SPRAIN/STRAIN TO HIS LEFT KNEE. THIS ACCIDENT ORIGINALLY OCCURRED IN AUGUST BUT DID NOT BECOME REPORTABLE UNTIL 11/24.
Ee was using a Komatsu WA 30 (small bucket loader) to stack & move ""Screen Cloths"". The Screen Cloths are metal & stack began to tilt & fall. Ee was backing up on foot & lost his footing & stumbled backwards. He fell & in doing so injured his lower back & left knee. He also sustained some minor scrapes & cuts.

While hauling dirt to dump in a Cat 777D off highway truck, he ran over a rock on the road and bounced in his seat. he hit his elbow on the arm rest, causing a contusion to the elbow.

EMPLOYEE WAS OPERATING A SHUTTLE CAR & CLAIMS THAT HE RAN OVER SOMETHING IN ROADWAY & JAMMED HIS HEAD INTO SHUTTLE CAR CANOPY. IT WAS OBSERVED THAT NOTHING WAS IN THE ROADWAY. INJURED WAS TREATED & TRANSPORTED BY AMBULANCE TO HOSPITAL.
Ee was putting cat head back in on 7 right #3 belt drive. As he was twisting the cat head to line it up he felt a pain in his left ring finger, which resulted in torn ligaments & tendons to his left ring finger which required surgery to repair. The ee started missing work on 8-30-04. Ee was wearing gloves.
Employee was trouble shooting a cut off switch for the R-5A refuse belt. As he was coming down from the structure, he slipped on loose refuse and fell. He struck a beam 24 inches below and injured his right side.

AFTER COMPLETING THE TASK PAIN & STIFFNESS WAS FELT TO HIS LOW BACK. THE CONDITION WORSENED OVER TIME, ON 9/9/04 HE RECEIVED MEDICAL TREATMENT & BEGAN MISSING SCHEDULED WORK DAYS.
EMPLOYEE WAS CLEANING BELT AND BACKED INTO LOOSE ROOF BOLT AND A PIECE OF ROCK FELL AND BRUISED HIS NECK AND LEG.
TWO EMPLOYEES WERE MOVING A SET OF LOCKERS DOWN STEPS. AS THEY GOT NEAR THE BOTTOM THE EMPLOYEE AT THE UPPER END RELEASED HIS GRIP MOMENTARILY TO MOVE A FIRE EXTINGUISHER. THIS ACTION CAUSED THE LOCKERS TO SLIDE, UNEXPECTEDLY PINCHING THE INJURED PERSON'S RIGHT WRIST. HE SUSTAINED A FRACTURE OF THE RIGHT RADIUS WHICH IS CAUSING HIM TO LOSE TIME FROM WORK.
The employee was policing the section when he stepped into hole in the bottom, twisting his right ankle.

After continuing to miss work with no communication as to why, he was sent letter of termination. When he received letter of termination he called and said that he had been injured.

The employee pushed a stone on to the conveyer rollers. A chain guard was knocked off and landed on his foot.
Employee was running a front end loader and felt a pain in his back. This accident was not reported as work related until 9/23/04. The case was litigated and was accepted as work related on 12/28/04.

Employee bent over to pick up pump and his knee popped. EE did not tell anyone until 9-9-04 at 8:00 am. I took him to the doctor at 1:30 pm and ee is on light duty.
TWO MEN WERE MOVING 60"" BELT SADDLES (20 FT. & 25 FT.) TO SPLICING STATION FOR AFTERNOON SHIFT. AFTER MOVING THE SADDLES, EE FELT PAIN IN HIS LOWER BACK. HIS CONDITION HAS WORSENED AND HE BEGAN TO MISS WORK ON 9/20/04.
GLOVE WAS CAUGHT ON A BOLT AS THE MACHINE WAS ROTATING THE BOLT. EE'S RIGHT MIDDLE FINGER WAS CAUGHT AND FRACTURED.
While employee was using manual hoist to lift a metal tube, the tube struck him on his lower right leg causing a small laceration. Six sutures were required to close the wound.
Draw rock fell from top striking employee on right foot. (Started missing work on 8/27/04).
Employee was manually operating wet wash sand plant and caught his hand between a belt and a pulley resulting in his left middle fingertip being cut and requiring 5 stitches.
While changing out a dump valve on longwall pump, the employee twisted his left wrist and re-injured it. Employee continued to have problems with his wrist. He had surgery on 11/30/04.
Employee was making repairs to the flats at the end of the dredge using the tender. He stepped from the tender, foot was caught causing employee to fall against Ibeam. bruised left side shoulder/elbow, etc.
Employee was trying to open the doors on rail car #37. The door was difficult to open and employee gave extra push and popped his right shoulder.
DID NOT START LOSING TIME UNTIL 8/30/04. Strained back while riding bus. Bus hit something on track and bounced the employee hurting upper back.
Employee was lining hopper with plate for wear purposes. He lifted plate overhead and pulled a ligament in his arm.

EE WAS USING A METAL PRY BAR TO LIFT AND POSITION A SLAB OF SANDSTONE FOR SPLITTING WHEN THE PRY BAR SLIPPED OUT FROM UNDER THE STONE. THE STONE FELL ON HIS LEFT FOOT. HE WAS WEARING FULL METATARSAL SAFETY SHOES. DOCTOR REPORTED NO BROKEN BONES BUT A CONTUSION OF THE LEFT FOOT.
EE was driving truck, there was a curve at the bottom of the hill; he was going too fast and the truck landed on its side.
Loosing bolt with wrench; wrench slipped causing head to hit spray bar.
Employee was in rock truck the loader loaded the truck with a big rock the big rock jared the truck.
PUTTING BREAKER IN ON SHUTTLE CAR, CATHEAD FLASHED AND BURNT HAND.
CLIMBING OFF OF LOADER, HE SLIPPED ON BOTTOM STEP AND HIT ELBOW ON STEP ABOVE.

HE GRABBED THE COVER AS IT WAS SLIDING OFF AND TWISTED HIS BACK. SHORTLY AFTER THE INCIDENT HE BEGAN TO HAVE SPASMS IN HIS BACK AND WAS TRANSPORTED OUTSIDE.
EMPLOYEE INFORMED HIS SUPERVISOR AT QUIT TIME THAT HE HAD PAIN IN HIS LOWER BACK & LEFT LEG. THE PAIN STARTED SOMETIME AT THE BEGINNING OF THE SHIFT & AS THE NIGHT WENT ON IT GOT WORSE. HE STARTED MISSING WORK BECAUSE OF THIS ON 9/2/04 & IS STILL OFF WORK.
Ee was climbing out of dust pod and his knee pad slipped and he struck lower part of left knee on a stud bolt. EE DID NOT START LOSING TIME UNTIL 9/2/04 DUE TO INFECTED HEMATOMA.
Ee was trying to stop the shuttle car and he caught his right foot between the shuttle car frame and rib.
EMPLOYEE WAS BACKING MINER AROUND CORNER AND BENT OVER TO THROW CABLE OUT OF THE WAY. RIB ROLLED ON HIM. COMPLAINED ABOUT SPOT IN BACK OF HIS NECK HURTING, TOOK AMBULANCE TO HOSPITAL. #1 ENTRY MAINS SECTION.

End of door of the excavador opened and it pinched the fingers of the left hand.
Employee was assisting in off loading a CLA fork (approximately 150 pounds) from supply truck when his left hand was caught between fork and truck bed. The tips of his middle and ring fingers were fractured.
EE WAS POSITIONING BAGS OF PRODUCT ON A SEMIAUTOMATIC PALLETIZER VIA USE OF AN AIR TABLE. WHILE PUSHING ONE OF THESE BAGS, HE FELT A PAIN IN HIS LOWER BACK.
Employee was shoveling out rock granules in the silos when he felt a sharp pain in his back. He felt stiffness the rest of the day and reported it to his supervisor near the end of the shift. He did not request to seek medical attention at that time.
Employee stated that while backing the roof bolter out of the No. 1 entry, he slipped on a rock and fell, landing on his right knee.

Ee stepped across into a battery scoop bucket, his right foot slipped and he hyper extended his right knee. he sought medical attention and the mri showed a torn acl and torn meniscus cartialge. he is going to have surgery to repair his right knee.
Employee was cleaning out rock chute when he hit his right wrist on the chute bruising it.
Employee twisted his left foot while getting into the pickup truck at the terminal tanks, fracturing the 5th metatarsal on his left foot. Release date is unknown at time of filing.

Employee was shoveling product off a catwalk area, stepped backwards and fell through an opening in the expanded metal flooring. Fell 11 feet to the ground below, spraining his right shoulder
EMPLOYEE WAS ADDING AN EXTENSION TO THE WATER TRUCK SPRAY BAR. HE HAD REMOVED THE SPRAY BAR & CLAMPED IT INTO A VISE & WAS TIGHTENING ON A NEW SECTION. THE PIPE SLIPPED OUT OF THE VISE & STRUCK EE IN THE HEAD. EE RECEIVED A CUT & SLIGHT CONCUSSION. LOST TIME.
2 left - 5 south - crosscut, man was adjusting rib strap and plate when bolter lost control of breast drill. Drill hit him and as he fell backward, he tried to catch himself and strained his right shoulder.
Employee was cutting curb when a small piece of granite flew behind his glasses into his eye.
EE TWISTED RIGHT KNEE WHILE WORKING ON UNEVEN GROUND. THIS WAS NO REPORTED WITHIN 10 DAYS AS EE DID NOT THINK HE WOULD BE GOING TO THE DR. KNEE STARTED BOTHERING HIM;EE WENT TO DR 8/30/04. EE DID NOT MISS ANY WORK & CAN PERFORM ESSESENTIALLY ALLHIS DUTIES & WORKING AT REGULAR JOB. 11/12/04 EE HAD ARTHROSCOPIC SURGERY OF FOR 34 DAYS.
Employee had right hand on mast. Carosel closed pinching thumb on right hand. Employee lost skin on side and tip of thumb requiring skin grafting. Thumb also had fracture.
While leaving the pit on the haul road he forgot to lower his truck box. He was driving over the posted speed limit with the box in the air. His box hit the over land conveyer which caused the truck hydraulic box to land on the cab of the vehicle.
While installing a new movable jaw die, the lifting eye was to tall hitting the cat walk. He tried holding the die in place with his foot while the welder cut the lifting eye. The weight of the die came back crushing his foot between the stationary wall and the jaw die.
The ee was walking toward his work area, and passing near the secondary crusher, he felt the impact of an artifact on the back of his head. This produced a wound in his left/ back side of his head in spite of wearing a hard hat.
Employee stating he was throwing fan cable out of the roadway and felt pain in his lower back.
Employee was pulling piping off of a truck and dropped a piece on his right foot, breaking his big toe.

WHILE CARRYING A LADDER, THE EMPLOYEE SLIPPED ON LOOSE MATERIAL, TWISTING HIS LEFT KNEE. EMPLOYEE BEGAN RECEIVING TREATMENT 3-28-05.
HURT BACK WHEN LIFTING A CABLE TO ATTACH TO A BUCKET.
EMPLOYEE WAS CLEANING PLUGGED SCREW UNDER 3 BIN (ORE BIN). WHEN EMPLOYEE STOOD UP AND TURNED AROUND HE RAN INTO THE BOTTOM PLATE OF THE BIN CAUSING HIM TO HIT AND CUT HIS RIGHT EYE LID.
Employee was advancing the power on the #2 section. As he was dropping the continuous mining cable, small fine rock and rockdust fell from the mine roof covering his head and the top part of his body. As he was brushing the material from his head and top part of his body some of the loose material entered both of his eyes. He sustained a scratch to the cornea of his right eye.
EMPLOYEE STATED HE WAS COMING THROUGH THE BACK DOOR OF BATH HOUSE AND THAT HE TRIPPED OVER A RUBBER MAT THAT WAS CURLED UP ON THE END. THIS CAUSED HIM TO STRIKE HIS RIGHT KNEE ON THE CHANGING BENCH SETTING IN THE AREA.
THE EMPLOYEE WAS SERVICING EQUIPMENT, STARTED IN THE MORNING AND HAD DRIVEN A SERVICE TRUCK DOWN INTO THE PIT. HE APPARENTLY HIT A ROCK OR A HOLE, SHAKING THE TRUCK AND FLIPPING A BARREL OF ANTI FREEZE OVER. HE ATTEMPTED TO PICK IT UP BY HIMSELF AND STRAINED HIS BACK.
EMPLOYEE WAS CLEANING HOT DUST WITH BOBCAT WHEN BOBCAT STALLED. EMPLOYEE EXITED EQUIPMENT AND STEPPED INTO HOT KILN DUST AND RECEIVED SECOND DEGREE BURNS OF THE FOOT.
While haul truck was being loaded, the shovel dipper struck truck bed.
Working on 9A track overcast returnside. Ee was using slate bar to pry overhead on stringer beam to install stairs tie down. As prying on solid concrete - block under heel of bar broke - causing ee to fall against steps - striking nose and rt. eye - fracture & cut.
Fell off of excavator, landed on ground on right hip and right shoulder.
EMPLOYEE WAS HAULING SUPPLIES WITH A RAIL LOCOMOTIVE TO THE #2 SECTION. CAR DERAILED WHILE HE WAS PUTTING THE SUPPLY CAR BACK ON THE TRACK. THE LIFTING JACK FAILED, CAR FELL, STRIKING HIM ON THE LOWER LEFT ARM.
INJURED EMPLOYEE WAS TRYING TO GET A PIECE OF ROCK OFF MINER WITH A SLATE BAR WHILE TRYING TO GET THE ROCK OFF MINER HE FELT OR HEARD SOMETHING TEAR IN HIS LOWER BACK.
EMPLOYEE WAS OPERATING A BRIDGE CONVEYOR WHEN HE GOT HIS ELBOW AGAINST THE TOP JAMMING HIS LEFT WRIST DOWN ON THE CONTROLS, BREAKING HIS LEFT ARM.
Victim was loading roof bolter, victim lifted a box of resin to put on bolter and experienced back pain and trouble breathing.
While bolting, his drill steels became stuck inside the chuck and together. When attempting to pull them apart the employee separated his right shoulder.
EMPLOYEE STATED WHILE PUTTING GUARD UP AT DISCHARGE IN PROCESS SUSTAINED LACERATION TO RIGHT POINTER FINGER.
EE WAS WALKING TOWARD HIS EQUIPMENT AT START OF SHIFT WHEN HE STEPPED INTO A HOLE AND TWISTED HIS LOWER RIGHT LEG.
IE was carrying materials down flight of steps. His shoe heel caught in steps. IE twisted right knee.
EE said he was walking in #7 entry (in return) when he stepped in a hole and twisted his left knee.

Injured was building a permanent stopping when a piece of draw rock fell from mine roof striking injured on right shoulder. Rock was 2' in diameter, and 2"" thick.
Employee was unloading asphalt truck. Believed truck to be empty. Took hose off without shutting valve on truck. Truck was not empty which resulted in asphalt flowing from valve onto employee.
Employee stepped out of LHD and stepped on a berm in the fuel storage area causing his right ankle to turn. Orginal diagnosis was twisted ankle. Further evaluation showed torn ligament. First shift missed to repair ankle was 10/5/04.

While opening a 5 gallon paint can with a retractable knife EE accidentally cut his left index finger with the knife causing him an open wound
Employee slipped while trying to get out of the cab of a fork truck.
Employee was working on installing plastic into the trailer of a coal train truck. He was attempting to move the plastic when he tripped over a lifting cable. He fell into the sheet of plastic on the left side of his chest, resulting in two broken ribs. EE had assistance and was utilizing a tagline.
EE, WHILE SPREADING HIGH CALCIUM LIME, APPARENTLY GOT SOME ON THE KNEE AREA OF CLOTHING NOT REALIZING THAT. LATER ON DURING THE SHIFT HE GOT HIS KNEES WET WHICH CAUSED A CHEMICAL REACTION.
EE BEGAN TO FEEL PAIN ON 8/20/04, BUT CONTINUED TO WORK. HIS PAIN INTENSIFIED ON 8/23/04 AND HE WENT TO THE ER. DIAGNOSIS: BILATERAL INGUINAL HERNIA (SURGICALLY REPAIRED ON 9/1/04). NOTE: EE LIFTS UP TO 100 LBS. (STONES).
Employee lifted a tool box out of a locked box and strained his lower back.
Employee was changing Z-screen in Deister and was using a crowbar, the end of the crowbar was under the corner of the Z-screen and as the employee pushed down on the crowbar it slipped out of the corner and smashed the finger of the opposite hand. The left index finger required two stitches.
The employee was a passenger in a rail-man bus that was following another man-bus. The front bus lost traction due to wet rails, slid backward and the two man-busses collided. The employee sustained a sore right leg. The examination showed a possible torn ligament. The employee did not return to work his next scheduled shift.
EE WAS SITTING ON TOP OF RAILRUNNER WHILE MOVING IT AND CAME TO A CURVE IN TRACK. THE RAILRUNNER JUMPED TRACK CAUSING EE TO INJURE LEFT SHOULDER. (STARTED MISSING WORK 10/25/04.
The employee was removing his drill steel from under a box of resin and a 3 ft bolt wrench was on top of the resin box. When he pulled the steel from under the box, the wrench fell and struck his right small finger. The tip of the finger was out of place and the physician pulled back in place.

The ee returned to work his next scheduled shift, but became dizzy during his shift. The examination was negative, but the physician determined that two days away from work was necessary with rest and observation.

Employee was climbing down the ladder from the top of a railcar when a rung of the railcar ladder popped loose and the employee fell 8' to the ground landing on his feet. he was diagnosed with deep bruises in each foot.
EMPLOYEE WAS HOLDING THE CATHEAD OF A MINE CABLE WHILE THE CABLE WAS BEING PULLED BY A 4-WHEELER. THE ROPE SLIPPED CAUSING HIS THUMB TO BE CAUGHT BETWEEN THE CATHEAD & THE 4-WHEELER - RESULTING IN A FRACTURED RIGHT THUMB.
While landing a section of conveyor, set down on top of foot.
EE WAS USING HIS FOOT TO HOLD DOWN A CRIB TIE THAT WAS STUCK UNDER A SWING ARM THAT WAS BEING DELIVERED TO THE MINE. ANOTHER PERSON STRUCK THE CRIB TIE WITH A SLEDGE TO DISLODGE IT. WHEN THE TIE RELEASED, THE EE LOST HIS BALANCE AND FELL DOWN. HE STRUCK HIS LEFT ELBOW AS HE LANDED. THE DISTANCE OF THE FALL WAS ABOUT 3 FEET.
Employee was working on discharge line in plant on cyclone pump. The next section of pipe was in place when employee hit the pipe with hammer and the come-a-long rolled around catching his left little finger.
Employee was preparing to climb up on his assigned loader when his foot slipped off of the bottom rung of the ladder and hit the ground, turning his ankle the wrong direction.
LADDER WAS PLACED ON LOOSE MATERIAL & BECAME UNSTABLE CAUSING A FALL. REQUIRED SAFETY EQUIPMENT WAS WORN AT TIME OF ACCIDENT & CAME OFF DURRING FALL ALLOWING THE UNBOLTED GRILL OF THE TRACTOR THAT THE EE TRIED TO GRAB FOR TO COME OFF & IT STRUCK & CUT HIS HEAD.
Ee attempted to remove full oil drum from discharge end of mill #11. The edge of the drum was placed on the lip of the bobcat and when ee tried to lift the drum more it fell over and landed on his left foot.
Ee drove Cat 627F scraper down hill into pit, as left tire dropped into pit, the operator was slammed against left side of cab, bruising his side and kidney area. He went to doctor the following afternoon due to some blood in his urine. He was instructed to take 5 days off to ensure that he would be ok.
Employee aggravated previous back strain when he attempted to lift a pump by himself. Didn't receive MRI results until 8/30.

Waiting to be loaded the ee decided to get off his truck and while walking outside and around the pit area he tripped hurting his right elbow, hand and back.
EE was plastering a stopping and got some plaster in his eye. He said that it came through the side of his safety glasses.
EE injured his left knee while crawling through the mine. He was wearing knee pads at the time, left knee became swollen by the end of the shift.
While operating a continuous miner on a pillar section, employee was injured when a rib rock measuring 5'x 2'x 6"" fell & struck him on his left leg. He was preparing to take the 1st lift to the left in the #7 Heading.
The boat loading leader was carrying an impact idler through the T-2 transfer door when his helper dropped his end. The leader could not hold his end of the idler and it fell on his left foot, fracturing his big toe.
Loading belt rails on mini track and rail dropped in mud splashing in his left eye and cutting his eye.
EMPLOYEE WAS LOWERING THE DRILL STATION CANOPY AND HE LOWERED IT TOO FAR AND CAUGHT HIS LEFT HAND, LACERATING HIS HAND.
The employee was performing a pre-shift examination for the oncoming Graveyard shift. He was walking through the Last Open Crosscut between the #2 and #3 entries. A bounce occured which blew coal out and struck him in the face and shoulder.
Employee greasing crusher, stepped on rebar and rebar broke causing employee to fall approximately 1 1/2 feet to ground level. Employee injured left leg.
WHILE SPLICING A PHONE CABLE, HIS KNIFE SLIPPED AND CUT HIS LEFT HAND.
Employee was loading 6 foot timbers into a scoop bucket and felt pain in his lower back.
Employee stated he stepped down off of a rail and turned his ankle while laying track.
Ee was cleaning up around conveyor belt using a shovel & 1 1/2"" water hose. When he returned to his ladder he felt a pain in his left shoulder (strain/sprain). This was initially under investigation by ins. co. for possible connection to prior injury - insurance company accepted claim as a new claim 10-14-04.
Employee states he was climbing back to the platform with an air hose and wand when the air line hose hit him in the back and he lost his footing and fell approximately 4 feet on his neck and back.
EE was moving a section of grating in place on a platform. He decided to do it on his own and not wait for help. EE was tied off to a handrail with one lanyard & was using the other to drag the grating. He pulled the grating too far & it fell 15 ft, causing him to be pulled down against the existing grating he was standing on. EE was released to return to work the following day.
EE WAS GOING INTO THE PLANT DRIVEWAY TO HAUL AWAY MATERIAL FROM THE PLANT. HE RAN INTO SIDE OF PLANT
EE WAS WORKING ON 8A SECTION. HAD COMPLETED MINING CYCLE AND WAS IN PROCESS OF ROCK DUSTING LAST 40 FEET OF MINING IN TURN #1 TO #2 CROSSCUT. WHILE PULING ROCK DUST HOSE BACK, A PIECE OF COAL RIB FELL AND STRUCK THE EE (ESTIMATED TO BE 1""-8"" THICK X 3' X 4'). RESULTED IN CONTUSION TO RIGHT LEG AND HIP.
WAITING FOR PINNER TO MOVE UP TO NEXT ROW, HE HAD LEFT FOOT LAID OUT BEHIND HIM WHEN PINNER PULLED UP AND RAN OVER FOOT.

A piece of rock was hung up in the 12-A headdrive, while removing rock employee strained/sprained right shoulder.
Ee was splitting stone with a chisel and 6 lbs hammer. as he swung his hammer toward his chisel, the stone unexpectantly separated causing him to pinch his right index finger between the stone and his hammer handle.
Pushing line onto as it was feeding out of equipment. rock struck him on ankle.
When checking his ram car batties, he lifted the battery lid (lid was stiff). Lifting the lid, pain was experienced right shoulder. Soreness continued & on 8-27-04. he was treated by Dr. He continued to experience pain & discomfort. He was later diagnosised to have a torn tenden. Surgery was performed on 4-18-05 missing scheduled work days on that date. first day missed: 4-18-05.
Unloading timbers from a flat car when one of the timbers rolled down and mashed his finger tip.
#6 shield was plowing into the mine bottom. another ee complressed the base lift jack s0 ee could put a crib underneath the base.t he other ee released the button for the base lift jack which caused the shield tip to come down. the shield canopy struck injured ee on top of head.
INJURED EMPLOYEE WAS WELDING OVERHEAD WHEN A FOREIGN OBJECT BECAME LODGED IN HIS EYE. HE WAS WEARING SAFETY GLASSES.
EMPLOYEE WAS PULLING SOME CABLES WHEN HE FELT A TWINGE AND THEN SIGNIFICANT PAIN IN HIS LOWER BACK. HE HAS A HISTORY OF BACK TROUBLE, INCLUDING A LOST-TIME ACCIDENT IN 1999. X-RAYS REVEALED DEGENERATIVE JOINT DISEASE, LUMBAR DISK DISEASE AND CHRONIC CHANGES IN L5-S1. HE STARTED MISSING TIME ON 8/18/04 TO GO FOR PHYSICAL THERAPY. DR.'S NOTES RECEIVED 9/2/04.
Employee was changing screen cloth. Screen cloth was being tightened down with tension pin and wedge when the wedge shot upward and hit employee on the lip cutting the upper lip and loosening three teeth.
Employee, who was using the water truck to water the road in the pit area, was washing spillage from underneath the 7'Crusher pedestal when he needed more hose to complete the task. He yanked on the hose causing his feet to slip in the mud, falling onto the frame of the crusher pedestal.
Was changing out a lip shroud on 59 shovel, weights about 90 pounds. Felt pain in his right groin.
Employee was installing a 1/2"" steel plate to mount a rotary feeder. He used excessive force to pull the plate into position, and partially tore his right bicep tendon.
Per the employee what happened: Lifting 50 lb pumice bags while bagging and putting them on a skid. And still kept working sweeping the dust on the floors.
Injured was tramming scoop down an approximate 7% grade in roadway, and claims to have lost control of scoop and ran scoop bucket into rib, causing an abrupt stop, which in turn caused alleged injuries.
60 feet right of spad 663 #5 entry, draw rock fell between rib and bolts; rock was approximately 1 foot in diameter, and 3/4 inch thick; this rock struck injured in back.
EMPLOYEE REPORTED HE WAS DRILLING A HOLE NEAR THE END OF A PUMP SHAFT FOR A COTTER KEY WITH A HAND HELD DRILL, WHEN THE 9/64 INCH DRILL BIT BROKE. EMPLOYEE RECEIVED A PUNCTURE WOUND AT THE BASE OF THE MIDDLE FINGER ON HIS RIGHT HAND. EMPLOYEE RECEIVED SUTURES AND RESTRICTION FOR USE OF RIGHT HAND, WHEN HE WAS TAKEN TO THE DOCTOR.
EE came off night shift and complained of stomach/abdominal pain.
Injured stated that he pulled off bump & ran over something & bounced head against top.
Employee stepped off excavator and ankle rolled on uneven ground causing a sprain.

EE STATED THAT HE TRIPPED OVER THE SHUTTLE CAR CABLE & INJURED HIS LEFT LEG
PACK HOUSE OPERATOR OPENED DOOR TO PACK HOUSE ELEVATOR AND FELL IN ELEVATOR WHICH WAS NOT ON THAT FLOOR. ELEVATOR HAS A 2ND GATE THAT NEEDS TO BE CLOSED BEFORE ELEVATOR WILL MOVE. WHEN ELEVATOR IS IN CORRECT POSITION IT IS LIT & THE ENTRANCE IS ALSO LIT. EE BACKED INTO ELEVATOR W/OUT LOOKING.
INJURED WAS CARRYING WATER LINE ON SHOULDER WHEN HE SLIPPED AND FELL, THE WEIGHT OF THE PIPE CAUSED PULLED LIGAMENTS.
EE GOT THE DOZER STUCK OVER THE OUT SLOPE, IT WAS REAL STEEP. WHEN HE GOT OUT OF THE CAB HE SLIPPED AND FELL. HE SLID DOWN THE TRACK AND STRUCK THE BLADE OF THE DOZER.
Employee said he was walking and turned ankle. He said he had done this previous to employment, he said walkway was good & no loose rocks or obstacles.
WHILE WORKING A DUMPING POINT ON SECTION, EE TOOK A SLATE BAR TO FLIP A ROCK OFF OF THE CHAIN CONVEYOR FEEDING TAILPIECE. THE BAR FLIPPED UP AND HIT HIM IN THE NECK.
EE WAS DESCENDING THE STAIRS OF THE 4BELT/2BELT TRANSFER STATION & AS HE NEARED THE GROUND HE LET LOOSE OF THE HANDRAIL. AS HE STEPPED ON THE 2ND STEP FROM THE BOTTOM A WELD FAILED & ONE SIDE OF THE STEP DROPPED 4"" CAUSING HIM TO LOSE HIS BALANCE & FALL APPROX 12"" TO THE GROUND
EE WAS ROOF BOLTING WHEN A ROCK BROKE AND SMASHED HIS FOOT RESULTING IN LOST WORK DAYS.
Trying to get water out of air line by opening a ball valve. When he opened the valve the pipe struck him in the chest causing him to fall. He received a severe contusion on his left shin resulting in a hematoma.

EE WAS CHANGING BATTERIES ON A SCOOP AND WAS PULLING THE BATTERY BELL FROM THE BATTERY. HE SLIPPED AND HIT HIS HEAD ON THE FIRE SUPPRESSANT HANGING FROM TOP. A KNOT AROSE BEHIND HIS RIGHT EAR.
PAIN IN LOWER BACK AND LEFT LEG. EMPLOYEE BELIEVES PAIN CAUSED BY RIDING ON TRACTOR WITH FOAM FILLED TIRES BOUNCING AND JARRING FROM HOLES IN ROADWAY.
Employee was walking, stepped in a hole with right foot, felt pain in right side of body.
EE was hanging miner cable in #3 entry on #2 unit at x-cut 22. He strained some muscles in his neck. Other ee was helping him hang the cable.
Employee bent over to pick up a piece of nylon rope and experienced pain in his lower back. He was diagnosed with a lumbar strain.
EMPLOYEE WAS PULLING WATER HOSE IN PELLETIZER FROM LOWER FLOOR TO OPERATING FLOOR. LATER, HIS BACK BEGAN TO HURT. WENT TO CLINIC DIAGNOSED WITH DEGENERATIVE DISC DISEASE. REPORT BEING FILED BECAUSE OF CITATION #6159919. WE BELIEVE THAT THIS IS NOT REPORTABLE.
Removing fuel nozzle from fuel tank at end of fueling operation. ee slipped as the nozzle was being pulled out. he fell backward on to the side of a bucket of another loading pantead nearby.
EE WAS LOCATED UNDER SUPPORTED ROOF LOCATED NEAR THE SECOND ROW OF SUPPORTS, OUTBY FROM AN UNSUPPORTED CUT WHERE THE MINING MACHINE WAS WORKING. HE WAS IN A KNEELING POSITION, OPERATING THE MINING MACHINE WHEN AN AREA OF MINE ROOF FELL OUT FROM BETWEEN PERMANENTLY INSTALLED ROOF BOLTS STRIKING HIM. THE ROCK CAUSED A FRACTURE TO HIS LEFT INDEX FINGER & A DEEP BRUISE TO HIS THIGH.
MINER OPERATOR WAS OPERATING MINER; SAID IT HIT A PIECE OF ROCK AND BOUNCED HIM AND HURT HIS BACK.

Employee was driving off road truck. She said while being loaded a rock fell in the back of the truck and bounced her up. When she came down she hurt her back and is having numbness from hip to knee on the left side. She went to a doctor on the 17th. She is still out of work.

The ee was using a torque wrench to tighten a lug bolt on mantrip wheel. The ee stated that he pulled something in groin area.
Employee was walking to his work area, when a stone fell from roof striking him on the head and neck, fracturing 2 two vertibrae.

THE EMPLOYEE STATED HE WAS WATCHING A SCOOP PULL THE BRETBY CHAIN IN THE PAN LINE USING A 3/4"" NYLON ROPE WHEN THE ROPE BROKE AND STRUCK HIM ON THE LEGS.
EMPLOYEE WAS INSTALLING A ROOF BOLT AND WHEN HE REACHED HIS ARM AROUND TO THE CONTROLS HE STRUCK HIS ELBOW ON THE CANOPY JACK STAND HOUSING.
EMPLOYEE PULLED LOWER BACK MUSCLE WHILE LIFTING AN INSPECTION DOOR ON THE FLASH DRYER.
WHILE BEING LOADED UNDER THE SHOVEL A LARGE CHUNK DROPPED JARRING HAUL TRK. 20 MINS LATER EE REPORTED THAT HIS BACK MUSCLES WERE GETTING TIGHT. HE SAID HE HAD BEEN IN SEVERAL AUTO ACCIDENTS HIS BACK WAS BROKEN 2-3 YRS AGO & REINJURED INA HEAD ON 5/04. DR DIAGNOSED MUSCLE STRAIN & RX'D PHYSICAL THERAPY. NO VISIBLE DIFFICULTY CLIMBING DOWN LADDER OR WALKING
EE STATED THAT WHILE HELPING REMOVE THE ATRA JACK FROM THE MINER, HE FELT PAIN IN HIS GROIN. (EE DID NOT START MISSING WORK UNTIL 8/21/04.)
WHILE SERVICING A ROOF BOLTER, EMPLOYEE WAS HANDING A CAN OF HYDRAULIC OIL TO ANOTHER EMPLOYEE. A PIECE OF ROCK FELL FROM BETWEEN ROOF BOLTS AND STRUCK EMPLOYEE ON TOP OF HEAD.
WHILE WELDING ON SHOVEL BUCKET RECEIVED FLASH BURNS TO BOTH EYE AREAS.
Employee was walking beside the roof bolter and raised his hand on the resin tray when a rock fell from the top and struck his fingers.
EE was talking down the feeder cable when he tripped over a rock and fell on his left hand.
EE stated that he was loading a spool of 1 1/2"" hose on a trip when he felt a pain in his lower back. his first lost work day did not occur until 12/6/04 due to surgery.
Employee was working on a platform with a square 32 inch hole cut out on the floor. Employee was not wearing fall protection and walked too close to it and fell 27 feet.
Employee was tramming an electric roof bolter. He released the tram controls and the bolter rocked. When it rocked back, it mashed his left foot between the frame and the floor. Fractured 3 toes on left foot.
INJURED WAS WORKING ON TOP OF THE EXPLOSION DOORS OVER THE CYCLONES OF THE THERMAL DRYER. HE STEPPED BACKWARDS OR HIS FEET SLIPPED CAUSING HIM TO FALL SIX TO SEVEN FEET STRIKING HIS RIGHT ANKLE AND LEG ON A METAL BEAM.
PERSON WORKED ALL DAY ON 34 BUTT BELT DRIVE ALIGNING POWER PACK. EE DID NOT REPORT ANY TYPE OF INJURY THAT DAY. THE PAIN BECAME ACUTE ON SATURDAY EVENING & SUNDAY. ON MORNING OF 8/16/04, EE CALLED IN TO WORK & REPORTED INJURY TO SUPERVISOR. EE WENT TO THE DR. ON 8/16/04.
Employee was operating a dozer and had just finished pushing on the left side of the dump, and felt pain in his neck, and blacked out momentarily. He was taken to the E.R, and Dr stated that it was probably a muscle strain that resulted in nerve impingement. EE was given one day off, and prescription medication.
Employee bent over to look under the crusher and felt pain in his back
EE was working on the feedshelf of the preheater and turned to quick. when he turned it pulled his back on the left side.
Ee was operating diesel personnel carrier traveling outby to the ""A""""B"" power center located outby the working section. As he reached location of ""A""""B"" power box he stepped out of personnel carrier & proceeded to power center. As he was walking he stepped on a loose piece of coal causing him to twist his left ankle & left knee.
EE STATED THAT HE WAS ROOF BOLTING WHEN THE WRENCH CAME DOWN ON HIS LITTLE FINGER. EE REFUSED MED TREATMENT ON 8/14/04 DID NOT CALL IN OR REPORT FOR WORK.8/18/04 EE WENT TO ANOTHER EE'S HOME TO FIND HIS PAYCHECK. OTHER EE ARRANGED FOR A DR VISIT 8/19/04 WHERE IT WAS DETERMINED THAT HIS LITTLE FINGER WAS BROKEN. EE TESTED POSITIVE TO DRUG TEST & WAS DISMISSED 8/20/04
HE STRAINED HIS ABDOMINAL WALL (UMBILICAL HERNIA) LIFTING A HEAVY STEEL FRAME. (HERNIA REPAIRED 9/29/04 PER CALL)
EMPLOYEE WAS TRAVELING TO HIS WORK LOCATION ON A TRACK MOUNTED JEEP. THE JEEP STRUCK A BROKEN RAIL AND STOPPED SUDDENLY CAUSING THE EMPLOYEE TO STRIKE HIS LEFT KNEE AGAINST THE INTERIOR COMPARTMENT OF THE JEEP.
Prior track conditions exacerbated by heavy equipment operation.
LIFTING A PUMP TO PLACE IT INTO A JEEP AND FELT PAIN IN HIS LEFT SHOULDER.
ON 8/13/04, A WASH PLANT MAINTENANCE WORKER WAS ASSISTING WITH REMOVING AND REPLACING THE GUIDE RAILS FOR THE DRAG CHAIN CONVEYOR IN THE VESSEL LOCATED ON THE FOURTH FLOOR OF THE WASH PLANT. AS HE ATTEMPTED TO REMOVE A PIECE OF GUIDE RAIL HE SMASHED HIS RIGHT INDEX FINGER BETWEEN THE GUIDE RAIL AND A CHAIN CONVEYOR FLIGHT.
Fractured and cut right index and middle finger while installing T-board to roof. Fingers were caught between roof and board.
Employee was a passenger in a Euclid Ch-120 haul truck riding in the jump seat. The loaded truck stopped to dump and when the brake was set the tractor frame failed breaking nearly completely through on both sides. The cab fell forward contacting the ground with significant impact. Employee struck head on air conditioner and his lower leg on the passenger door.
Ee while operating a scoop ran over a crib block. As scoop came off block it jarred the ee causing him to hit head on canopy of scoop.
EE ALLEGES THAT WHILE OPERATING A TRUCK HE BEGAN TO EXPERIENCE BACK PAIN/MUSCLE SPASMS. EE MISSED 4 DAYS OF WORK DUE TO ALLEGED BACK PAIN. (MISSED 8/18-21) NO MEDICAL TX PER PART 50. THROUGH CONVERSATION WITH EE'S MEDICAL PROVIDER EE HAS ONGOING/CHRONIC BACK PROBLEMS. HIS NOTES TO FOLLOW.
ROOF BOLTERS AND SUPPLYMAN WERE LOADING SUPPLIES ON ROOF BOLTER. THE OFF SIDE OPERATOR WAS PUTTING BOLTS FROM SUPPLY CART ONTO HIS SIDE OF THE MACHINE WHEN HE FELT A TWINGE IN HIS BACK. HE STATED THE AREA WAS LOW AND HE WAS LIFTING NORMAL WEIGHT / LOAD. THE EMPLOYEE FINISHED THE SHIFT AS USUAL.
WHILE LIFTING TOP STRUCTURE AND TURNING WITH IT EMPLOYEE'S RIGHT KNEE GAVE WAY.
The ee climbed from the operator's compartment on the roof bolt machine and was not aware of the distance to the ground. He stumbled, fell and struck his left rib on the side of the machine. The x-ray was negative, bruise.
Employee had been unloading timbers he parked the ram card and was walking to the intersection when he heard a popping and cracking sound. he turned around and saw the rib moving. employee tried to get out of the way of the rib role but was hit on the right leg and calf.
The ee was exiting the cab of the d-10 dozer. When his foot slipped off the step on the dozer arm blade. This cawed him to straddle the dozer arm blade, causing injury to his groin area.
FOLDING UP WET CANVAS BY THE FAN & STEPPED ON A ROCK IN A WATER HOLE & TWISTED HIS RT. KNEE, CAUSING A STRAIN TO HIS RT. KNEE.
MEMBER PULLING ON MINER CABLE FELT PAIN IN LOWER BACK. MEMBER HAD VACATION SCHEDULED FOR WEEK OF 8/16/04 PRIOR TO THIS ACCIDENT. MEMBER NOTIFIED SAFETY DEPARTMENT AUG 20, 2004 THAT HE WOULD NOT BE RETURNING TO WORK AFTER VACATION.
Ee was struck in the right side of head by the mrs cable which knocked him down into a mud hole.
Employee was putting a bearing on a shaft and got a small piece of metal in eye. Required surgery to remove particle.
SHOVELING AT #36 CROSSCUT ON NO #1 BELT LINE PICKED UP SHOVEL FULL OF COAL TO PUT ON BELT AND FELT A SHARP PAIN IN LOWER BACK, HE FINISHED OUT THE SHIFT AND WENT TO DOCTOR THE NEXT DAY.
Ee was roof bolting in last open crosscut between entry 1 & entry 2 on unit 2. As he was inserting roof bolt a piece of roof rock fell from between the roof bolts, striking him on rt forearm. The incident caused a laceration to his rt forearm that required sutures. He started missing time on Aug 13 & is still under dr's care at this time.
He was gathering a sand sample. He inserted sample tube into the the sample pad of sand, and it stopped abruptly, striking him very hard on his right forearm.
MECHANIC REPAIRING TAILGATE ON DUMP TRAILER & IT FELL ON HIM. HE RECEIVED MINOR CUTS & BRUISES.
WALKING ALONG THE STAGELOADER, EMPLOYEE STEPPED ON A ROCK & TWISTED HIS RT. KNEE, CAUSING A STRAIN TO THE KNEE.
EE WAS CLIMBING DOWN THE LADDER ON THE CAT END DUMP WHEN HE SLIPPED ON STEP AND FELL TO THE GROUND. FELL 15 FEET. FRACTURED NOSE.
EMPLOYEE WAS HANGING CURTAIN IN #1 ENTRY AND FELT PAIN IN LOWER BACK AND LEFT HIP.
ON 8/11/04 EE LIFTED AN 80 LB. SWIVEL HEAD ATTACHMENT SEVERAL TIMES WITHOUT PAIN BUT WHEN HE GOT OUT OF HIS VEHICLE AT HOME, HE FELT PAIN TO HIS LOWER BACK. HE REPORTED THE PAIN THE NEXT DAY. THE DOCTOR ALLOWED HIM TO CONTINUE WORKING BUT ON 8/25 WHILE STARTING TO CLIMB A HAUL TRUCK LADDER, HE FELT PAIN AGAIN. ON 9/2 AN MRI SHOWED DISC AND FACET DISEASE AND FACET ANTHROPATHY.
Employee was splicing a belt with a utility knife. After changing the blade on the utility knife, he cut his left hand.
The employee was jack legging the ribs when a large chunk of coal fell out of the rib and struck him on the head and shoulder. The employee did not have any broken bones or internal injuries. *First day out was 8/13/04.
Bent down to get rock, felt pain in lower back.
INJURED WAS STANDING ON A RAM CAR TO HANG A PIECE OF CABLE. WHEN HE STEPPED DOWN HE TWISTED HIS ANKLE.
EMPLOYEE WAS SHOVELING THE #1 TAILPIECE AND SLIPPED, FALLING BACKWARDS BRUISING HIS BACK AND KIDNEY.
Turned to walk and his foot hung in a turnbuckle and caused his left knee to twist. Did not start losing time until 8/16/04.
EE WAS CARRYING TOOLS, CROSSING 3RD FLOOR. HE STEPPED ON AN UNEVEN FLOOR DRAIN AND HIS RIGHT FOOT BENT TO THE SIDE. X-RAYS WERE POSITIVE.
EE was walking to his office at the beginning of his shift. He stepped on some slippery material on the roadway and landed on his right knee. Eventually had surgery on his right knee on 09/16/2004.
Employee was using a razor knife to cut an old radiator hose. His forearm came in contact with the open blade, resulting in a cut that required 14 stitches to close.
Employees were setting cribs. Finger (index, left) was crushed with crib. Finger was fractured.
SUBJECT WAS EXPLAINING TO AN EMPLOYEE HOW TO CHANGE THE SPEED ON THE DRILL PRESS WHILE THE PRESS WAS RUNNING. WHEN HE POINTED HIS FINGER, IT WAS TOO CLOSE TO THE DRILL PRESS AND HIS FINGER GOT PULLED INTO THE PRESS.
Employee had been working in the lower area of the BE1570 dragline. After climbing out of this area employee alleges he stepped on the edge of a manhole cover rolling & twisting his ankle.
EMPLOYEE WAS TALKING TO ANOTHER EMPLOYEE, TURNED AND HIT HIS HEAD ON THE SHUTTLE CAR CANOPY AND KNOCKED VERTEBRAE OUT OF ALIGNMENT - PINCHED NERVE. EMPLOYEE STATED THIS WAS A RESULT OF FALLING OUT OF TREE STAND SEVERAL YEARS BACK.
6H section, beltman was separating belt stands when a stand fell over & struck injured on the side of his foot causing fracture. Lost work days began 8/17/04.
