(3) MINERS WERE WORKING TO INSTALL & MOVE A NEW CONVEYOR BELT. PILAR WAS FEEDING THE BELT OFF THE ROLL TOWARD THE HEAD PULLEY. VICTIM WAS PULLING THE BELTING TOWARDS THE TAIL PULLEY WITH A 3/4" ROPE ATTACHED TO THE LACING HOLES W/3 STRANDS OF WIRE. THE PULL ROPE DETACHED FROM THE WIRE & HE FELL BACKWARDS ONTO A CONCRETE SLAB. HE WAS 12'ABOVE THE GROUND, FALL PROTECTION NOT WORN.

An electrician and a MSHA trained escort were in a crusher MCC, the electrician identified and then tried to remove a bad fuse when an Arc flash occured, the electrician was burned on both hands and a patch on the thigh, the MSHA escort was burned on the right hand.
Pulled fuse in fused disconnect, arc flash happened. Burns to both hands, and right thigh.

Dump Truck Driver was dumping load on the pit run pile and dislodged mud flap, driver got out to retrieve mud flat from pile and strained lower back while pulling on the mud flap

Worker said that he slipped while trying to grease the crusher at the end of the day, his left knee hit the crusher. This worker called in on Thursday 10/26/06, to say that he could not get out of bed because he had twisted his left knee. So he missed Friday the 27th of Oct. 2006. We beleive that this is a non employment injury. We did however fill out workers comp form 801.
Worker was greasing a digging bucket, when he stepped down off bucket the gravel under his footing gave way causing him to slip down. As a result of not checking his footing his chin caught the arm on the bucket.
Worker was repairing sand screw, while moving some parts with co-worker, his left thumb was pinched between two parts, smashing the tip causing fracture at the tip of his thumb.
