EMPLOYEE WAS ROOF BOLTING ON ONE SIDE OF A CROSSCUT. ON OPPOSITE SIDE EXPLOSIVE CHARGES HAD TEEBEEN SET. THEY WERE THEN UNINTENTIONALLY DETONATED
