INJURED WAS EITHER STANDING AT OR SITTING ON LONGWALL DISTRIBUTION BOX WHEN HE FELL ONTO THE RUNNING LONGWALL CONVEYOR BELT. HE WAS THROWN OFF BELT AGAINST A ROOF JACK AND THE TAILPIECE DRILL JACK.
