EMPLOYEE WAS DRIVING A HAUL TRUCK. AFTER DUMPING LOAD EMPLOYEE DID NOT DISENGAGE POWER TAKE-OFF. AS TRUCK WAS DRIVEN TOWARDS MINE ENTRANCE DUMP BODY WAS RISING AND BED STRUCK ENTRANCE CEILING OF MINE CAUSING EMPLOYEE TO STRIKE TRUCK DASH AND WINDSHIELD. AS A RESULT OF THE IMPACT THEEMPLOYEE INJURED HIS NOSE, CAUSING BLEEDING, LACERATIONS TO RIGHT EYE AND CHIN, X-RAYS REVEALED CRA
