THE INVESTIGATI2N DETERMED THAT THE EMPLOYEE HAD INSTALLED THE LAST ROOF BOLT IN PROPER SEQUENCE AND WHILE WAITING FOR OTHER OPERATOR TO FINISH HIS CYCLE, HE STEPPED BEYOND PERMENT ROOF SUPPOR T AND WAS STRUC5 BY A ROCK APPROXIMATELY 8X10X10 THICK AND WAS KILLED INSTANTLY.
