A MULTIPLE FATGAL ROOF ACCIDENT OCCURRED AS RAY DANIELS CONTINIOUS MINER OPERATOR AND DANNY MCGLOTHLIN MINER HELPER PARKED CONTINIOUS MINING MACHINE IN THE INTERSECTION AT THE NO 30 PILLAR AND WERE PREPARING TO SET TIMBERS FOR THE PUSHOUT WHEN THE ROOF FELL WITHOUT WARNING
